Prior to today’s visit, the facility’s 18 month compliance history was 94%. Upon my arrival I met with Jennifer Swart, Program Coordinator. Children had not arrived for the day, so I was able to review the staff files and program records. I reviewed the public school off site verification form, you informed me that you have one sub and one staff that comes from Polo Ridge ASEP to work the first hour, until her program begins. I reviewed all required training information. The sub was able to show that she completed BSAC through the CMS portal however the date is not showing, and she cannot print a certificate. It shows she completed the training. Program records I found meeting compliance were the monthly playground inspections, emergency care plan, incident log, and emergency drill log. The last fire inspection was conducted on 10/10/23. The last sanitation inspection was conducted on 11/20/23. Information required to be posted was observed posted on the ASEP Bulletin Board in the cafeteria. Each group, had a current activity plan posted. One child has medication to be administered for a chronic condition. The medication is stored in the program coordinator’s office, it was stored properly today. Current written permission to administer was last signed in September 2023, and should have been updated by March 2024. I observed cleaning supplies stored properly. I did not observe any hazards accessible to children. I monitored general safety requirements and found them meeting compliance. As the children arrived, I observed them place their belongings down, sit with the group, attendance conducted, and then they began preparing for snack. I did not observe any discipline issues. Staff were heard engaged in conversations with the children, each group had six interest centers set up to choose activities. Each group was observed in adequate/approved space. The current license was observed posted and all permit restrictions were observed meeting compliance. The following violation was observed today. Violation Number Comment Rule 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. One child has an epi pen on site, the medication authorization to administer was last signed in September 2023, and is required to be updated every 6 months. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) Compliance Statement Childcare providers are expected to maintain compliance with all applicable child care requirements at all times. All violations cited today shall be corrected immediately. Jennifer Swart, Program Coordinator, will email a compliance letter explaining how each violation cited today has been corrected. The compliance letter must be received by the consultant, andrea.anderson@dhhs.nc.gov, on or before June 7, 2024. Technical Assistance was provided on the following: Medication Permission: You have a medication notebook, I encourage you to start using it again or come up with some procedure to ensure you maintain compliance with medication requirements. Medical action plans are valid for 1 year, the parent still needs to give you permission to administer medication for the child's chronic condition, and it must be updated every 6 months. Substitutes: 10A NCAC 09 .0102(47) "Substitute provider" means any person who temporarily assumes the duties of a staff person for a time period not to exceed two consecutive months and may or may not be monetarily compensated by the facility. Any substitute provider must be at least 18 years of age and literate. 10A NCAC 09 .1102(3) substitute providers who provide services for less than 10 days in a 12-month period. This means any substitute used by CMS ASEP, not per school, but ASEP overall must maintain compliance with this requirement. They should have a traveling file that can provide each site on their first day subbing at a CMS ASEP to maintain requirements for staff files. There is a substitute file checklist on the Division website under Provider Documents. You will contact your HR to ensure they have all required paperwork on file that is listed on the Public School Off-Site verification form prior to allowing them to work with you, add them to the form once you verify with HR they have the information, then review your emergency care plan, have them complete their annual health questionnaire and emergency information and place on file. A checklist was used to monitor the facility today, along with the current Child Care Requirements effective July 1, 2023. I met with Jennifer Swart, Program Coordinator, during today's visit. Upon my arrival I sat in your office and reviewed the Licensing Notebook you have organized with information I need to review during visits as well as staff files. The last fire inspection was conducted on 10/10/23. The last sanitation inspection was conducted on 3-2-23. The monthly fire drill log was reviewed, you did not document the fire drill conducted in September 2023 on the emergency drill log. All information required to be posted was observed posted on the ASEP board in the cafeteria. Daily attendance and current activity plans were also reviewed today. You are not logging screen time and you don't have a technology activity listed on the current activity plans. The current Public School off site verification forms for children and staff files were signed 7/1/23 and current. I reviewed the current staff and training worksheet you provided and found staff files meeting compliance today. The Public School off site verification form for Transportation was also reviewed and current. I observed children's arrival, center free choice play and snack time. Staff were observed moving about the cafeteria monitoring their group of children. Each group had a variety of material and 6 centers set up for play. Supervision and staff/child ratio were observed being maintained. Outdoors you use the soccer field, black top any of the three stationary equipment areas when adequate depth of surfacing is maintained. Currently the furthest area is not being used because the resilient surfacing isn't maintained. The following violations were observed and documented today: Violation Number Comment Rule 539 When screen time was provided to school-aged children, it was not offered as a free-choice activity; not used to meet a developmental goal; was not limited to 30 minutes per day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. I observed school age children using tablets today, playing activities, non related to homework. A Cumulative Log has not been maintained. You did not have technology activities listed on any of the three group activity plans. .2508(e)(1-5) 805 Fire drills were not practiced monthly and/or the drill record was incomplete.
