Upon arrival, Mrs. Glover greeted me at the door. I stated the purpose of my visit and proceeded with the visit. Children were observed transitioning from nap, toileting, snack and outdoor play. Snack consisted of graham crackers and milk. There were two children that remained sleep while a teacher took the other children outside to play. I conducted a walk-through of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. When asked about firearms, the provider stated that she does not own any firearms. During this visit it was discovered that Mrs. Glover changed her mind about which pathway she wanted to proceed with so we discussed pathway one and two again and will do a rated licensed assessment later in the year. Details of what was discussed will be found below. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The lesson plan posted was dated for the week of 6-8-2026. GS 110-91(12); .0508(a) Violations: All violations were corrected on site. Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be incompliance with all the applicable laws and rules. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. Technical Assistance: NC Pathway to Stars: There are Three new pathways, you initially want to go with pathway one. You are now thinking about pathway two. Below are some similarities and what is needed for each one. Pathway one: Similar to the old rated assessment except you WILL have to have the environmental rating scale completed along with the self-assessment. This path you will have to do self-study, CQI for facility and one for teachers. Documentation of the selected family and community engagement activities. Pathway two: You have to show/ have proof that you have met two to three categories in Family and Community Engagement Standards. Refer to the handout left with you. (Family and Community Engagement Standards). Complete a CQI for center and one for Staff For Five stars you and teachers must have the approved curriculum and documentation/proof of training certificate, And evidence that you are using the curriculum. This facility currently operates with a five (5) star rated license effective April 28, 2019. I rang the doorbell and was greeted by Ms. K., lead teacher. I stated the purpose of my visit and was led downstairs to Mrs. Mattie and the children. The construction is finished on the new addition and was measured and approved today. You stated that you will transition all of the childcare to the lower level and the first space/classroom will be converted back into a family/living room. The main interest will be the second classroom space. The main playground with the climbing structure is currently closed due to a broken fence. The new room has a bathroom, kitchen, and an area for the children to have gross motor play when they are unable to go outside. Children currently use a fenced area with a basketball goal for gross motor play while the mail playground is being repaired. Children were observed in active gross motor play, center play, transitions from indoor to outdoor, as well as toileting and hand washing. Children were served chicken patty and bread, broccoli, carrots, and cauliflower, strawberries, and milk for lunch. After lunch children were observed napping. INSPECTIONS: The last sanitation inspection was conducted 1-15-2026. The last fire inspection was conducted 7-08-2025. The last documented fire drill was conducted 1-13-2026. The last documented shelter-in-place or lockdown drill was conducted 1-12 -2026. The last documented playground inspection 1/20/26. The last documented compliance was at 97%. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program and management. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. Facility Emergency Preparedness & Response were also reviewed. Space three was also measured and approved for fourteen (14) children. Remember your capacity is twelve (12) children. At arrival I rang the doorbell, Mrs. Glover greeted me at the door. I stated the purpose of my visit, and I conducted a walk-through of the facility. There were only two (2) children present today. Children were sitting, one was at the table and one in the high chair. Mrs. Glover stated that children took naps earlier today and after outdoor play she served them lunch. Lunch consisted of fish filet with bun, apples, salad, and milk. Children then transitioned to wash their hands and playing in activity centers. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. During this visit I observed the new constructions. It is currently 696 square feet, if this remains the space will hold 23 children, however remember building and zoning will assist in the determination.. When construction is completed, email me the building, fire, zoning, and sanitation inspections. Today’s visit was conducted with you, Mattie Glover. This facility currently operates with a four (4) star rated license effective April 28, 2019. Upon arrival I was greeted by the owner/director of the facility. I stated the purpose of my visit and proceeded to monitor the facility partially unassisted. We discussed that your playground is currently closed due to construction on the house. The playground is being rebuilt and you are adding an additional room to be used for the childcare. You stated that this room will have a bathroom, kitchen, and an area for the children to have gross motor play when they are unable to go outside. INSPECTIONS: The last sanitation inspection was conducted 1-10-2024. The last fire inspection was conducted 3-13-2023. The last documented fire drill was conducted 1-19-2024. The last documented shelter-in-place or lockdown drill was conducted 1-22 -2024. The last documented playground inspection was conducted 1-15-2024. The last documented compliance was at 98%. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program and management. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. Facility Emergency Preparedness & Response were also reviewed. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There was a can of Lysol disinfect on the steps leading to the upstairs area behind a gate. .2820(b) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Several staff files did not have medical information in a separate file. .0701(d) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of childcare requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: As outlined in Session Law 2021-127 - Senate Bill (SB); facilities who have been unable to hire/retain staff with the same educational levels in the current star rating are in a hold harmless state. The Executive Order No. 116 Declaration of a State of Emergency to Coordinate Response and Protective Actions to Prevent the Spread of COVID-19 has ended, and you are in the first cohort. You received and email in June 2023 with more details on the timeline. It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. REQUIRED RESPONSE: You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less.
