Upon arrival I was greeted by the receptionist and stated the purpose of my visit. After I signed in and receive my visitor’s sticker the Princpal stated that the stated that their was a substitute and a new hire and that Mrs Chanda is at a training today. The substitute contacted the group leader and she come to the office and walked me to the trailers. Children were actively engaged in gross motor activity. I restated the purpose of my visit and we proceeded to walk through the facility partially conducted with you, Ms. Nelson. The following spaces were observed: outdoor playground, cafeteria, gym, and the mobile classroom was observed today. Children were observed transitioning from gym to the trailer, and departure. Children were engaged in gross motor activities and were provided several types of sports balls as well as other portable materials. There was a teacher whose firsts day as group leader was today. Several Children were observed transitioning from the trailers to active gross motor play in the gym. I conducted a walk-through of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure, and all required posted documents. There was one new staff and a substitute present. I was unable to review the new staff file because, it was not available for monitoring. Pathways: Prior to this visit I received the application for Pathway two. I reviewed the documents associated with Pathway two. Once I receive the education worksheet I will be able to process your new star level. Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. New staff information was not available for review. G.S. 110-91( 9) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last drill was conducted on 12/17/26, and there is no documentation of the next drill. (drill kept on the computer). .0604(u);.0302(d)(8) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of childcare requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. REQUIRED RESPONSE: (for any violations) You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address or email below by June 10, 2026. The format to follow is: -Name of your facility, -ID number, -date of the visit, -date you are submitting the letter, -citing each violation number, -when AND how it was corrected AND how you will prevent the reoccurrence of this violation, -Close with YOUR SIGNATURE REQUIRED RESPONSE: If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a proposed timeline of the corrections. In some cases, the timeline may be extended. You are required to always maintain compliance with all applicable childcare rules and regulations. NC GS 110-90(4)(d) requires all childcare facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. Upon arrival I was greeted by the receptionist and stated the purpose of my visit. After I signed in and receive my visitor’s sticker the receptionist called Ms. Nelson to the front office. When Ms. Nelson arrived at the office, I restated the purpose of my visit and we proceeded to walk through the facility Today’s visit was conducted with you, Ms. Nelson. This facility currently operates with a five (5) star rated license effective June 27, 2018. Ownership information was confirmed as, CHARLOTTE-MECKLENBURG BOARD OF EDUCATION, a Department of Public Instruction program. All contact information was reviewed, and it was reported that no changes had been made. The following spaces were observed: outdoor black top, cafeteria, gym, and the mobile classroom was observed today. Children were observed transitioning from school into the cafeteria for afterschool and snack. Snack consisted of a cheese pizza pocket, carrots, juice, milk and apple juice. Outside children were engaged in gross motor activities and were provided several types of sports balls as well as other portable materials. Today there was a sub assiting Mrs. Nelson. The last sanitation inspection was conducted 10-02-2024 The last fire inspection was conducted 07-14-202. The last documented fire drill was conducted 4--2023. The last documented shelter-in-place or lockdown drill was conducted 01-20-2025. The last documented playground inspection was conducted 02/03/2025. The last Annual Compliance was conducted 2/20/2024. Compliance History prior to today’s visit is 98%. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. The required public school off-site verification forms for children’s records, Staff records, and transportation were reviewed and in compliance. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program, and management. Violation Number Comment Rule 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. The cafeteria, and multipurpose room did not have the required posting. .0713(a)(10), (c) & (f)(3); .2818(e) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First aid expired in 1/2025. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file.
