A checklist was used and given to the caregiver/owner. Today’s visit was conducted with Ms. King. Ms. King currently operates a four-star facility and stated that she is interested in reapplying for the rated license (RL). In September 2022, we started the RL process, and I did not receive all required documents to properly complete the reassessment. Children were observed playing in centers with dramatic play, blocks and fine motor toys. Children were served noodles with ground beef, milk, and grilled cheese sandwich. Snack will consist of corn, crackers and pineapples. Routines for diapering, transitioning from eating to play and nap were all observed today. INSPECTIONS: The last documented fire drill was conducted 8-29-23. The last documented shelter-in-place or lockdown drill was conducted 8-29-23. The last documented playground inspection was conducted 8-28-23 The compliance percentage prior to this visit was 85%. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. Transportation was monitored, but transportation is currently not being provided. Rated License: The provider stated that she plans to. Provide transportation in the future. A copy of the RL reapplication was given, completed by the provider, and returned to the consultant. Once the self-study is received the provider will remain a four-Star facility. She will have two points in programming, seven points in education and one quality point. Violation Number Comment Rule 811 Baby food, snack items, and meal items sent from home, for each individual child, were not dated and labeled with the child's name. Infants bottle and food was not labeled with childs name. .1706(o) 912 Written feeding schedule was not on file for each child under 15 months of age and/or was not updated regularly to reflect changes in the child's needs. there were no feeding schedule onsite. .1706(i) 1887 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser were not in their original container or in another labeled container, used according to manufacturer's instructions. There was an air freshener/sanitation can on top of the children's cubby. .1719(a)(7) It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. REQUIRED RESPONSE: (for any violations) You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address below by 9/26/2022. The format to follow is: -Name of your facility, -ID number, -date of the visit, -date you are submitting the letter -citing each violation number, -when AND how it was corrected AND how you will prevent the reoccurrence of this violation, -Close with YOUR SIGNATURE REQUIRED RESPONSE: You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. On February 24th, NC opened vaccinations to child care staff. This means your staff are eligible, but it does not guarantee a vaccination as there is a limited supply. In view of this, DCDEE has collaborated with CCR&R and Smart Start to assist child care facilities. CCR&R staff will be contacting child care programs to ensure they have information about vaccination events in their respective areas and Smart Start staff are working with local vaccine providers to help make sure they’re ready to give shots to child care staff. This will provide more information and helpful resources as you prepare to get your spot to take your shot. Remember to have parents to label every item, bottle, food that is sent from home. 2.Place the sleep chart near the crib to make it is easier to remember. 3. Remember handwashing when entering the building, before and after meals and toileting as well as outside play. 4.You may want to revise your schedule to accommodate the different age groups. 5. Review the hazardous items storage for child care sheet that I gave you. 6. Remember to complete On-going training documentations and health & safety training records, FCCH and Center located in a residence verification of required information and operator and additional caregivers, and FCCH and Center located in a residence operator’s statements of responsibility prior to Annual Compliance visit. 7. We reviewed the self study and I will process the application once I receive the self study. 8. Use masking tape or another form of labeling to make sure that the labels do not come off of the cribs and cots. Resources: Visit the DCDEE website at https://ncchildcare.ncdhhs.gov/.
