This family child care home has a Four-star license and an 18-month compliance history score of 96%. The center’s license and NC child care law were prominently posted. The Summary of Law was prominently posted. I was greeted by L. Smalls, Owner. I discussed with Ms. Smalls the Annual Compliance Monitoring guidelines. I completed the walkthrough of the center alone and Ms. Smalls assisted me with the visit. Permit Information: The program operates with a Four-star license, issued November 11, 2017, earning seven (7) points for education, two (2) points for program, and one (1) quality point for complying with a programmatic option. The facility is still located at: 5903 Lawnmeadow Dr, Charlotte, NC 28216 and the phone number is: (704) 910-1155. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. Five (5) children were present during todays visit. The center has six (6) children enrolled for first shift. The center is currently only providing care for first shift only. Inspections: The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required, the centers last fire drill was documented on 5/18/26. Shelter-in-place and lockdown drills were in compliance and documented on 4/15/26. The playground safety inspections were reviewed, the last playground inspection was documented on 5/19/26. The facility last had a sanitation inspection on 10/25. The inspection was completed by the Mecklenburg County Health Department. Outdoor Learning Environment: The outdoor learning environment was monitored today . I observed many tree branches in the areas where children play which could be a safety hazard. Safety: The center is in compliance with the firearm safety requirements. Transportation: The center does not provide transportation. Nutrition: Meals met nutritional value during today’s visit. Files: Children records were monitored today and in compliance. Staff files were monitored today and in compliance. There were two (2) violations cited during today's visit. Violation Number Comment Rule 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. I observed one child's file without documentation of immunization record. GS 110-91(1); .1721(a)(2) 1920 Indoor and outdoor area was not checked daily for debris, vandalism, broken equipment, and animal waste. I observed a lot of debris/ tree branches on the Outdoor Learning Environment. .1719(a)(17) Violations must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 7/1/2026, stating how each violation was corrected and how compliance will be maintained in the future include the violation number, statement of compliance, license ID and your signature in the letter or e-mail. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Today we reviewed the Application for Assessment for a Rated License for FCCH and a copy was sent to Ms. Smalls via email. Ms. Smalls is currently working with CCRI preparing for QRIS. Ms. Smalls plans to apply by August 2026, but the center may be ready sooner. Ms. Smalls and I discussed the following: 1. Family and Community Engagement Standards 2. Curriculum and Formative Assessments 3. Family Child Care Home (FCCH) Operator Continuous Quality Improvement (CQI) Plan and Professional Development (PD) Plan Next steps: 1. This family child care home has a four-star license and an 18-month compliance history score of 93%. The center’s license and NC child care law were prominently posted. I was greeted by Lisa Smalls, Owner. I discussed with you, Ms. Smalls, the Annual Compliance Monitoring guidelines. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. The caregiver was observed during nurturing and engaging interactions with children. There were five (5) child present today. There are five (5) children enrolled in the center. Inspections: The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required, the centers last fire drill was documented on 6/17/24. Lockdown drills were in compliance and documented on 5/20/24. The playground safety inspections were reviewed, the last playground inspection was documented on 6/17/2024. The facility last had a sanitation inspection in November 2023. The inspection was completed by the Mecklenburg County Health Department. I observed a power strip with outlets that were not in use without safety covers. 10A NCAC .1719(a)(27) 2048 Products that are labeled “keep out of reach of children” with an additional warning(s) on the label, were not kept in locked storage while children were in care. I observed a bottle of vitamins on the kitchen counter, accessible to children. 10A NCAC 09 .1719(a)(7) Technical Assistance: 1. I encouraged the Administrator, to stay up to date with the Division of Child Development and Early Education by visiting www.ncchildcare.ncdhhs.gov. I encourage you to click on the “What’s New” tab for important updates impacting childcare in North Carolina. 2. Today we discussed electrical outlets not being covered. Mrs. Smalls stated that she was unaware that the outlets were uncovered. I reminded Mrs. Smalls the importance of making sure the electrical outlets that are not in use are covered with a safety cover to prevent a child from getting electrocuted. Violations must be corrected immediately.
