The center operates with a Four- Star license and an 18-month compliance history score of 94%, effective December 4, 2024. The Secretary of State website was checked and the status of Gleaton's Learning Immersion Academic Center was current and active. The restrictions include: First and Second Shift; Meets enhanced ratios. Upon arrival, I was greeted by P. Gleaton, Provider. I informed Ms. Gleaton the reason for my visit. Ms. Gleaton assisted me with the walkthrough of the center and the visit. The following items were posted within the center/ classrooms: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/ departure procedures, summary of law and menus. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. Children were observed during indoor free play, teacher directed activities and transitions. There were four (4) children present today. There are six (6) children enrolled in the center. Outdoor Learning Environment: The outdoor learning environment was monitored and in compliance. Inspections: The outdoor inspections and fire drills were monitored. The last fire drill was conducted on 5/11/26. The centers last shelter in place/ lockdown drill was conducted on 5/7/26. The last monthly playground inspection was conducted on 3/5/26. The center last had a sanitation inspection on 4/23/26. The center received four (4) demerits and received a superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 6/2/2026. Transportation: The center does not provide transportation. Nutrition: Meals met nutritional value during today’s visit. Files: Children records were monitored today and in compliance. Staff files were monitored today and in compliance. There was one (1) violation cited during today's visit. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Family member's (DG) last qualifying letter expired 1/2026. The next qualifying letter was issued on 5/1/2026. G.S. 110-90.2(b) & .2703(n)&(o) Violations must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 6/9/2026, stating how each violation was corrected and how compliance will be maintained in the future include the violation number, statement of compliance, license ID and your signature in the letter or e-mail. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Name of Operation: Gleaton's Learning Immersion Academic Center Facility ID: 60004355 Consultant: RASHEKA WHEELER Operation Type: Center Case Number: Visit Date: 11/15/2024 Number Present: 4 Completed Date: 11/15/2024 Age: From 1 To 4 Total Minutes: 75 Time In: 09:30 AM Time Out: 10:45 AM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of my unannounced visit was to monitor for compliance with applicable childcare requirements for a third temporary time-period and review the results of your Environment Rating Scale Assessment. You achieved the following score on the FCCERSS assessment was 4.81. Lashawnda Berry, Child Care Consultant, accompanied me during today’s visit. Currently this center operates with a Temporary License, issued on June 3, 2024, to December 3, 2024. The restrictions include first shift and second shift only; Age Range 1-12. I used the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to this facility. In addition to monitoring for health and safety, I reviewed program files and required postings. During the walk-through we observed a large art activity was taking place at the table area. We spoke with Ms. Gleaton to resume the activity after our visit to give us some time to discuss the assessment report. The following items were posted in the classrooms: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/departure procedures, summary of law and menus. There was a total of four (4) children present. Age ranging from 1 to 4 years of age. The program has eight (8) children enrolled. Currently no children are enrolled on second shift. Indoor Learning Environment: Staff/child ratio was observed and in compliance. The outdoor inspections and fire drill report forms were reviewed during today’s visit. The forms were current and up to date. The last fire drill was observed documented was completed on 10/10/24. The last monthly playground inspection observed documented was completed on 10/11/24. Sanitation Inspection: The last sanitation inspection was made on 6/04/24 by the Environmental Health Specialist. A “Superior” classification was issued with 10 demerits noted on the grade card. Fire Inspection 5/03/24 and approved for daytime care only. Nutrition: Meals met nutritional value during today’s visit. Chicken nuggets, wheat bread, fries, cucumber, oranges and milk. Weapons: Your facility is in compliance with Child Care Requirements regarding firearms. Rated License: A review of each item on the Environment Rating Scale assessment was discussed with you and documented on the form attached to the visit summary. Curriculum: You stated you already use the Fun shine Preschool accompanied with NC Foundations in your four-year-old classrooms. Your temporary license will expire on December 3, 2024. The rated license packet will be processed with the points you have earned. We discussed that you must meet enhanced ratios and enhance space. The provider stated she wanted to think about the enhance space. As discussed, the staff/child ratio form will need to be updated in each classroom by placing a check mark in the box for voluntary enhanced ratio requirements. The facility had the Family Child Care Environment Rating Scales (FCCERS) completed on 10/17/2024. I also reviewed the indicator items that scored less than 5 and gave suggestions on how the staff could improve those scores. I reviewed the results with Ms. Gleaton to discussed potential licensing violations that were observed during the assessments. She understands that should these concerns be observed during future licensing visits violations of childcare requirements could be cited. The ERS Review Form was completed and will be attached to the Rated License Packet. Rated License Assessment Education Component The Provider qualifies at the 4-point level as the Lead Teacher. Program Component Enhanced Ratios and the FCCERS average score of 4.81. FCCERS 4.81 The average ERS score is 4.81 The facility has earned 5 points in Program Standards. Quality Point Option The facility has earned the quality point by meeting: The lead teacher have at least 5 years of documented early childhood work experience that can be verified by the Division and have been employed by no more than 2 different employers. Compliance History: Please note that any violations cited today or during future visits will negatively impact your compliance history. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Rated License: I reminded you that if you receive a four or five-star license, then you will be required to have an approved curriculum for children four years of age. As discussed, at the end of your temporary license period you must be at least a three (3) star facility in order to continue to provide care for children who receive subsidy. The following violation were observed today: Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. On 11/13/24, one (1) child was not signed out on the sign in and out document for the program. 10A NCAC 09 .0302(d)(4) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. We observed small pieces of dramatic play items (sushi pieces) accessible to children younger than three years of age. .0604(q) The Consultant assigned to this program after December 3, 2024, will be Lashawnda Berry. I greeted Ms. Gleaton, the Owner/Provider and stated the purpose of today’s visit. Center operates with a Temporary License, issued on June 3rd, 2024. The restrictions include daytime care only. Age ranging from 1-12 years of age; First and Second Shift. The NC Secretary of State website was viewed before the visit, Gleaton’s Learning Immersion Academic Center LLC, which is current/active as of 10/18/22. If any changes to the company need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. Indoor Learning Environment: Upon arrival, staff/child ratio was observed and was in compliance. This program operates in a designated space designed for her child care program. Currently, the facility has six (6) children enrolled on first shift; six (6) children were present during today's visit. Currently, the provider stated that she does not have anyone enrolled on second shift. Children were observed participating in a large group time upon my arrival. Large group time consisted of Alphabets, numbers, shapes, colors, and nursery rhyme songs. At 10:00, children used the bathroom and went into their learning centers for free choice play. I reviewed all program records requirements. I discussed the conversation the provider and Ms. Teresa Pender, QED Specialist at Child Care Resources, about preparing for the Environment Rating Scales. I re-iterated the Child Care Specialist can assist the classroom teachers and with room arrangements/suggestions, staff/child interactions and necessary materials needed to score well on the FCCERS assessment for Centers Located In a Residence. Outdoor Learning Environment: Was monitored upon arrival and in compliance with Child Care Requirements. Records: The Staff and Training Worksheet was completed prior to today’s visit. No changes were needed since the first temporary visit. Children: No new children were added since the first temporary visit that was conducted on July 10, 2024. Program: I reviewed all the required records. The fire drill report was completed monthly, and the last documented drill was 9/05/24. The last outdoor environment inspection was completed on 9/04/2024. Inspections: Sanitation Inspection was conducted on 06/04/2024 with an Approved classification and 10 demerits. Fire Inspection was completed 0n 5/03/2024 and the facility was approved for daytime only. Nutritional Guidelines were met during today’s visit. Lunch was comprised chicken nugget, string bean and potatoes, pears, wheat bread and milk. Weapons: Your facility reported that they were in compliance during today’s visit with Child Care Requirements regarding firearms. The following violations were cited during today's visit. Compliance History: North Carolina General Statute 110-90(4)(d) requires all childcare facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Rated License: I discussed the two-component rated license process prior to today’s visit. I discussed the three components: Program, Education and Quality Point. I provided the quality point document during today’s visit for Ms. Gleaton to read over and select the option that is best for their program. I will process your rated license packet with the staff’s education that has been previously evaluated. I explained and discussed enhanced ratios and enhanced space with you. I explained that if you receive a four or five-star license, then you will be required to have an approved curriculum for children four years of age. The facility has already adopted the Funshine Curriculum for Preschool. As discussed, at the end of your temporary license period you must be at least a three (3) star facility in order to continue to provide care for children who receive subsidy. You must have earned enough points for 3-5 stars in order to continue receiving subsidy dollars. I reviewed that this may be difficult for some programs without an Environment Rating Scale. You complete the application for the Rated License Request during today’s visit for me to request the ERS Scale. Health and Safety Trainings: The Child Care Development Block Grant (CCDBG), also called the Child Care Development Fund (CCDF), is a federal grant which provides funding for childcare subsidies for low income working families and improvements in childcare quality. The recent reauthorization of CCDBG, Child Care and Development Block Grant Act of 2014, brought for the first time in 18 years new state requirements. These requirements promote the health and safety of children in childcare, create continuity of access to subsidies for low-income families, better educate parents and the general public about the childcare choices available to them, and improve the overall quality of early care and education programs. States must have pre-service or orientation and on-going minimum health and safety training requirements in place for childcare providers. The trainings must cover the following content areas: * Administration of medication * Appropriate precautions in transporting children * Building and physical premises safety * Pediatric First Aid and CPR appropriate for the ages of children in care * Prevention and control of infectious diseases, including immunizations * Prevention of and response to emergencies due to food and allergic reactions * Prevention of shaken baby syndrome and abusive head trauma * Prevention of SIDS and use of safe sleeping practices * Recognizing and Responding to child abuse and neglect * Handling and storage of hazardous materials and the appropriate disposal of bio-contaminants * Emergency preparedness and response planning for emergencies To help childcare providers meet the new training requirements, the Division of Child Development and Early Education has Moodle Trainings set up and can be accessed through the Divisions website: ncchildcare.ncdhhs.gov. Violation Number Comment Rule 427 Children three years old or older were in care and the schedule did not include times and/or activities that were developmentally appropriate for the children in care. During today's visit, a current activity plan available/posted for review. GS 110-91(12); .0508(d)(2) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. While on the outdoor learning environment, I observed a bag of charcoal, bleach container and another product that stated keep out of reach of children accessible to children on the porch area. .2820(b) 1301 Center did not maintain a record of daily attendance. The attendance was not maintained or current during today's visit. I observed the attendance record was not completed on yesterday's date (9/23/24). GS 110-91(9) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Th provider did not complete the Child Maltreatment Training prior to August 3rd date of this year. .1102(g) Technical Assistance: Remember due to the multiple ages you are caring for to cater or limit the amount of time spent in large group time. The younger children can still engage in large group time but still be allowed to leave for free choice activities nearby. Make sure you are signing up for Recognizing and Responding to Child Maltreatment Training and Playground Safety Training. I introduced myself and explained to Priscilla Gleaton, Owner/ Administrator/Lead Teacher, and stated the reason for my visit. Currently this center operates with a Temporary License, issued on June 3rd, 2024, and will expire December 3rd, 2024. The restrictions include daytime care only and no children in diapers, no frying of foods and no laundry done on site. The capacity of the facility is 12 children on first shift and 5 children on second shift. Ownership: The NC Secretary of State website was viewed before the visit and the corporate owner is Gleaton’s Learning Immersion Academic Center LLC. The corporation is current/active as of 10/18/2022. Keep mindful that if any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. During today first Temporary Time Period visit, the one classroom was monitored for meeting all minimum licensing requirements, including staff-child ratios, supervision, materials, equipment, required posted items, activity plans, daily schedules, nutrition, and sanitation. Indoor Learning Environment: Upon arrival staff/child ratio was observed and was in compliance. The children were inside playing upon my arrival, participating in free play in their center areas and preparing for a bathroom routine. I observed handwashing and bathroom procedures, expressive language skills, and large group time with the toddlers and preschool age children. The children were very verbally interactive, and I enjoyed the stories and information that they shared. Outdoor Learning Environment: The following items were observed in the outdoor learning environment: there is a designated space for children to engage in play. The children also have permissions to play outside the gate. I reminded Ms. Gleaton that lawn equipment can not be accessible to children if they are playing outside of the designated play area. Program Records: The fire drill log was reviewed during today’s visit. The fire drill log and Playground Inspections were current and up-to-date. The last fire drill took place on 6/26/24. The last playground inspection, took place on July 5, 2024 Transportation: You stated that currently your program does provide transportation. If you begin to provide transportation you must contact me in order for your vehicle to be monitored and a review of the transportation child care requirements will be conducted. Children’s Records: Three children’s records were reviewed, with three violations observed during today’s visit. Staff Records: staff files and husband were reviewed during today’s visit. I explained that she will be responsible for keeping this document up to date. I discussed transitioning forms from FCCH to Center documents as well as keeping all medical forms and documents separated from the staff files. I discussed with Ms. Gleaton to update the staff and training worksheet as she hires new staff for her Center Located in a Residence. I discussed completing the orientation and documenting it, ensuring that all hazardous items are locked up, ensuring safety for all children including children under three years of age and access to plastic bags, and adding gross motor activities to the activity plan. Nutrition: Ms. Gleaton is currently working with the Child and Adult Food Program, through observation of the menu, she is meeting meal pattern requirement for Breakfast, Lunch and PM Snack. Weapons: Your facility is in compliance with Child Care Requirements regarding firearms. Rated License: I discussed the two-component rated license process. Ms. Gleaton stated that they wish to apply for the rated license and have the Environment Rating Scale assessment. North Carolina Rated License Project (NCRLAP) is now completing standard Environmental Rating Scales. I explained that she would complete the paperwork for the Rated License during the next visit. I encouraged Ms. Gleaton to contact CCRI to begin working with them in preparation for the assessment. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One child enrolled and present, did not have documentation stating the child's parent was given The Summary of the NC Child Care Law. GS 110-102 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child, enrolled since being license, did not have a completed medical on file completed by the child's parent during today's visit. GS110-91(1) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. Ownership: The NC Secretary of State website was viewed before the visit and the corporate owner is Gleaton’s Learning Immersion Academic Center LLC. The corporation is current/active as of 10/18/2022. During today first Temporary Time Period visit, the one classroom was monitored for meeting all minimum licensing requirements, including staff-child ratios, supervision, materials, equipment, required posted items, activity plans, daily schedules, nutrition, and sanitation. Indoor Learning Environment: Upon arrival staff/child ratio was observed and was in compliance. The children were inside playing upon my arrival, participating in free play in their center areas and preparing for a bathroom routine. I observed handwashing and bathroom procedures, expressive language skills, and large group time with the toddlers and preschool age children. The children were very verbally interactive, and I enjoyed the stories and information that they shared. Outdoor Learning Environment: The following items were observed in the outdoor learning environment: there is a designated space for children to engage in play. The children also have permissions to play outside the gate. I reminded Ms. Gleaton that lawn equipment can not be accessible to children if they are playing outside of the designated play area. Program Records: The fire drill log was reviewed during today’s visit. The fire drill log and Playground Inspections were current and up-to-date. The last fire drill took place on 6/26/24. The last playground inspection, took place on July 5, 2024 Transportation: You stated that currently your program does provide transportation.
