The center operates with a Four-Star license and an 18-month compliance history score of 90%, effective July 11, 2019. The Secretary of State website was checked and FRAN, INC. was current and active. The restrictions include: First and Second Shift; School age children only; Meets enhanced space. Upon arrival, I was greeted by P. Holit, Adminstrator. I discussed with Ms. Holit the Annual Compliance Monitoring guidelines. Ms. Holit assisted me with the walkthrough of the center and the visit. The following items were posted within the center/ classrooms: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/ departure procedures, summary of law and menus. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. Children were observed during transitions. There were two (2) children present today. There are fourteen (14) children enrolled for first shift, and there are two (2) children enrolled for second shift. Outdoor Learning Environment: The outdoor learning environment was monitored today and in compliance. Inspections: The outdoor inspections and fire drills were monitored. The last fire drill was conducted on 6/2/25. The centers last shelter in place/ lockdown drill was conducted on 4/17/25. The last monthly playground inspection was conducted on 5/21/25. The center last had a sanitation inspection on 1/17/25. The center received four (4) demerits and received a superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 7/11/24. Transportation: The center does provide transportation. The vehicle used to transport children was monitored. Ford #PFE9732 registration expires: 4/30/26 Insurance expires 10/1/2025. Nutrition: Meals met nutritional value during today’s visit. Files: Children records were monitored today and in compliance. Staff files were monitored today and in compliance. There were violations cited during today's visit. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. I observed three (3) ceiling tiles along the wall where the couch is. 15A NCAC 18A .2825(a) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. In space #1, I observed a piece of art with a broken frame on top of a shelf. Under that, on the shelf, I observed a broken toy bin. 10A NCAC 09 .0604(p) Violations must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 6/17/2025, stating how each violation was corrected and how compliance will be maintained in the future include the violation number, statement of compliance, license ID and your signature in the letter or e-mail. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. The center currently has a Four-Star rated license, the 18-month compliance history score is 95%. The center license and Summary of the Law was prominently posted. I was greeted by P. Holit, Administrator. I informed Ms. Holit the reason for my visit. I began the walkthrough of the center alone and Ms. Holit assisted me with the visit. Permit information-the program operates with a Four- Star license, issued July 11, 2019. The center has seven (7) points for education, two (2) points for program standards and one (1) quality point. License restriction include: First shift and Second Shift; School age children only; Meets enhanced space. The program is still located at: 2600 West Trade Street, Charlotte, NC 28208. The phone number is:(704) 714-4840. The center had three (3) children present during today’s visit and twenty (20) children enrolled. The following items were monitored: Supervision, S/C Ratio, Storage of Hazardous Substances, Storage of Medication, Adequate Approved Space, Program Records, License Posted and Permit Restrictions. The center has not hired any new staff since the last visit. The center’s last fire drill was documented on 1/3/25. The center’s last lockdown drill was on 1/7/25. The last sanitation inspection was completed on 1/2/25 with four (4) demerits, receiving a superior rating. The center playground inspections were monitored and in compliance. There were two (2) violations cited during todays visit. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. Transportation: The vehicle used to provide transportation was not on site, therefore I was unable to monitor the vehicle today and will monitor it at the next visit Nutrition: Meals met nutritional value during today’s visit. Peeling paint- In space #1- I observed peeling paint on the boarders throughout the classroom. Peeling paint is considered hazardous, there is a concern that children can eat it. Ms. Peggy stated that they just painted the classroom and that the children must have picked at it. Ms. Peggy stated that they will try to find another paint, one that doesn't peel as easily. 2. Electrical outlets- In space #1- I observed an electrical outlet not in use without covers. When not in use, electrical outlets and power strips located in space used by children shall have safety outlets or be covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Ms. Peggy stated that someone must have moved the furniture that was in front of the outlet. Ms. Peggy will move the sofa, which is a heavier piece of furniture, in front of the outlet as well as make sure there are safety covers in the plugs. Violations must be corrected immediately.
