Natasha Asbury, the owner, assisted us with the visit. This facility issued a temporary license on November 15, 2024, with a restriction of daytime care only. During this visit, I monitored the indoor and outdoor environment. Children were observed playing in teacher directed activities, circle time and preparing for lunch. Ms. Nellie Williams, the provider’s mother, was present preparing lunch for the children. Ms. Natasha Larysa was training two (2) substitutes during my arrival Ms. Fannie Rachord and Ms. NiAsia Alexander. Teachers were engaged in a positive nurturing way with the children. Program records were reviewed. The menu was not current, this was corrected during the visit. New staff files were reviewed, one (1) violation was observed and corrected during the visit. The EPR plan was completed February 12, 2025. Two (2) violations were cited and corrected during today’s visit. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu that was posted was not current. 10A NCAC 09 .0901(b) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One staff member did not have a signed Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy on file prior to working with the children. .0608(d)(1-4) Rated License Process : We discussed the rated license process with you today. You stated that you decided not to have the assessment done at this time. Comments Section: - It was discussed today that if the provider is using a rotating menu, make sure the correct menu is posted for parents to view. - Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. - Today I measured the living room, which was where Ms. Asbury had her FCCH, and then the new addition. This will help Ms. Asbury if she decided to change her capacity to 15 in the future. Natasha Asbury, the owner, assisted me with the visit. This facility was issued a temporary license on November 15, 2024, with a restriction of daytime care only. During this visit, I observed the indoor and outdoor environment, and completed a full assessment of the licensed space. Children were observed in circle time, singing Christmas Carols, playing outside, and transiting to lunch. Five (5) children were signed in on the attendance sheet today, six (6) children were present. Ms. Asbury corrected this violation during the visit. Ms. Asbury was engaged in a positive nurturing way with the children. Program records, including fire drills and monthly outdoor area inspections were complete and current. Staff and children’s records were reviewed to ensure compliance. Ms. Asbury had taken the EPR training and has until March 15, 2025, to complete the EPR plan. One (1) violation was cited and corrected during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. Six children were present, only five children were signed in on the attendance sheet. 10A NCAC 09 .0302(d)(4) Rated License Process : When you transfer from a FCCH to a CLIAR, you will be issued a temporary license for 6 months as you are changing the operational type for providing child care. Your current subsidy payment rate as a FCCH provider will transfer with you to your new operational status (CLIAR) during the 6-month temporary time period. However, because this change of operating type is considered a new facility with a new ID#, hold harmless does not apply. The hold harmless statute is related to currently operating facilities and not new facilities. I discussed the rated license process with you today. Ms. Asbury stated that she is working with Ms. Pender from CCRI. Ms. Asbury is not sure if she is going to have the scales completed Ms. Asbury understands that I will need to request the Rated License Assessment, but February 15, 2025. I encouraged Ms. Asbury to attend the January 14, 2025, QRIS meeting at CCRI. Ms. Asbury was given the information sheet today. Ms. Asbury stated she has children enrolled who receive subsidized care. She understands the facility must hold a 3-5 Star Rated License at the end of the temporary time period to continue providing child care for sponsored children. Education Standard Points: Ms. Asbury has a current WORKS letter. Ms. Asbury has a BA/BS Degree in EC/CD and over 15 years of experience. She will earn 7 points education. Quality Point: you can choose to earn one extra point toward your star rated license by meeting one of the quality point options. I reviewed the quality point options with you today. Comments Section: - Ms. Asbury and I discussed that daily records of arrival and departure times at the center for each child which shall be maintained as children arrive and depart. I explained that if the parent does not mark the child’s time and departure that Ms. Asbury can do it so that her attendance is accurate. - Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent.
