Today’s visit was partially assisted by Mrs. Chapman group leader. This facility currently operates with a five-star rated license effective February 22, 2019. This facility has changed from an elementary after school to a middle school afterschool program. The facility has two groups and there was a total of five children present. The following activity centers: science, math, art, reading as well as social studies. Children were observing talking, chrome books, and homework. INSPECTIONS: The last sanitation inspection was conducted 04/04/25. The last fire inspection was conducted on 10/08/25. The last documented fire drill was conducted 10/08/25. The last documented shelter-in-place or lockdown drill was conducted 9/19/25. The last documented playground inspection was conducted 10/18/25. Last compliance history was 99% I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. Daily schedules, activity plans, staff/child ratio worksheets, arrival/departure procedures, summary of law and menus. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. Violation Number Comment Rule 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. children used Chromebook and it was not documented. .0510(d)(2)(A-C) If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a proposed timeline of the corrections. In some cases, the timeline may be extended. You are required to always maintain compliance with all applicable childcare rules and regulations. NC GS 110-90(4)(d) requires all childcare facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Your compliance history was 97% prior to today’s visit. Upon arrival, I went to the afterschool program entrance and had to walk around the building to get into the building. I was greeted by a school staff member who retrieved Mrs. Kennedy. We greeted each other and I stated the purpose of my visit. Today’s visit was conducted with you, Lavonda C. Kennedy, Program coordinator/site administrator. This facility currently operates with a five-star rated license effective February 22, 2019. The facility has two groups, who were combined today due to low attendance. Children were observed participating in a contracted group activity in the library. The following activity centers: Dramatic play, science, math art, reading as well as social studies and gross motor physical play. Children were also observed transitioning to and from the bathroom. Children were served sun butter sandwiches, fruit cup, milk, and orange/vegetable melody juice. Children do not use the playground due to safety issues (tree stumps and mulch). Children use the open field and blacktop(basket ball court). If the playground is not repaired, it can keep the facility from maintaining the appropriate score on the Environmental rating scale assessment. INSPECTIONS: The last sanitation inspection was conducted 9-20-2023. The last fire inspection was conducted.6-30-2023 The last documented fire drill was conducted 10-28-2023. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The coordinator did not have and did not send the required checklist and it was not available for review during the visit. 10A NCAC 09 .0304(a) Comments: Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. REQUIRED RESPONSE: You shall correct the violation(s) immediately.
