Upon arrival I walked into the entrance and rang the bell. I was greeted by the Administrator assistant, and I stated the purpose of my visit. Today’s visit was conducted with Ms. Avion Edgeworth, Co-director This facility currently operates with a five (5) star rated license effective July 13, 2018. I observed the children engaged in the following activities: quite play, large Lego play, eating /feeding, and nap time. Prior to this visit I received the application for Pathway one, and your Rated License assessment review form. You stated that you would like to have the scales ordered later this month due to several staff being on vacation in July. I reviewed the application, and we discussed that I needed a copy of the excel version of the Staff information worksheet. I conducted a walk-through of the facility and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. Prior to this visit I received your fire inspection, and it is submitted with this visit. All violation was corrected during the visit. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. A childs cup in the Fireflies room was not labled and three teachers cups throughout the building were unabled. 15A NCAC 18A .2804(d) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. A outlet was uncovered in the 4/5 year old room near the book center. 10A NCAC 09 .0604(c) You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. This center has a five-star license effective July 13, 2018. The NC Secretary of State website was viewed before the visit, the business was current and active. Upon arrival I was greeted by the administrative assistant who opened the door for me. I walked in the office and was greeted by Ms. S. Mungo, administrator who directed me to Ms. A. Edgeworth, Co-administration, I stated the purpose of my visit and I conducted the visit with S. Mungo and Ms. Edgeworth assisted at the end. Teachers were observed actively engaged with children during center play as well as gross motor play. Infants were engaged in napping and active tummy time and play, diapering and toileting and feeding. One year old’s were observed in active gross motor play and transitions from outdoor to indoors. and two were engaged in circle time and the teacher read several books. The twos played with fine motor toys after seating at the table for circle time. Threes were observed in circle time and eating lunch. Children were served milk, buttered noodles, meatballs in marinera sauce, and mixed vegetables for lunch. The four/five year old children were observed in circle time discussing animals that live in trees. INSPECTIONS: The last sanitation inspection was conducted 12-13-2025. The last fire inspection was conducted 01-16-2025. The last documented fire drill was conducted 2-12-25. The last documented shelter-in-place or lockdown drill was conducted 1-16-2025. The last documented playground inspection was conducted 02/20/2025. The last Annual Compliance was conducted 3/28/2024. Compliance History prior to today’s visit is 86%. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program, and management. The facility does not currently provide transportation. Staff/child ratio and supervision, capacity, group size and space were observed in compliance today. The center’s license and NC childcare law were prominently posted. Violation Number Comment Rule 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. The Dragonflies, (one and two year olds) room had a plastic bag under the changing table. .0604(q) 892 The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily. Tadpole room did not have evidence of this polocy in the room. .0606(b) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. You are required to always maintain compliance with all applicable childcare rules and regulations. NC GS 110-90(4)(d) requires all childcare facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Your program currently operates with a Five Star (5) Center License The license was posted, with restrictions to first (1st) shift care, meets enhanced space and meets enhanced ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-eighty (88%) percent as of today’s visit. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. Fire and Sanitation Inspections have not occurred since the last visit and remain current. The following items above were found in/out of compliance. The following inspections are below. •Last fire drill was conducted on 9/24/24. •Last Shelter in Place/Lockdown Drill was conducted 7/15/24 •Last Playground Inspection was conducted on 8/21/24. Indoor Learning Environment: The children were observed in the rooms that serves infant age children were participating in self-guided free play and teacher assisted self help routines such as feeding. The children in the younger toddler classrooms were participating teacher and group led music and movement time and transitioning back into the classroom. The children in the rooms that served preschool age children were participating in self-directed and free choice center time and transitioning from center time to teacher directed circle learning time. Outdoor Learning Environment was not monitored due to Hurricane Helene’s inclement weather. Staff Records: The staff-training worksheets were completed before the visit. There were seven (7) new staff files to monitor. All files were well maintained and organized. The Administrators were aware of all letters and trainings that were expiring. Medication: Emergency medication was monitored and was in compliance. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. Nutrition: The menu and allergy listing were posted in each classroom and the children were having pizza, mixed veggies, and milk for lunch. The allergy listing was not posted in the main food preparation area but this was corrected. Two (2) violations were observed today and both were corrected during the visit. The violations are as follows and no follow up is required. There were two (2) fans in space number three (3) and one (1) in the music and movement room that were within reach of the children and did not have a mesh guard to prevent access. 10A NCAC 09 .0604(d) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Rated License •Hold Harmless has been extended until the new QRIS is implemented (SB 425). Providers in Cohort #1 (number one) are not required to go any further with a rated license assessment unless they want to, and providers in Cohort #2 (number two) do not need to start their preparation year unless they want to. Upon request, a childcare facility may be awarded a star-rated license based on accreditation from a national childhood education accreditation organization provided the facility maintains its accreditation and remains in good standing. •Use this time for staff to update their information in the WORKS database. Technical Assistance •Make sure all safe sleep check logs are current and easily accessible for daily use. Continue to safe older safe sleep check logs for the appropriate amount of time. •When a staff member removes anything from an outlet, please replace it with an outlet cover immediately. •As with most older buildings, they can be humid, and the temperature can vary from room to room. It would be beneficial to investigate a dehumidifier and temperature gauges or thermometers in each room to maintain the correct temperature rage. Also, keep in mind that safe sleep policy requires the infant room temperature does not exceed seventy-five (75) degrees. •As a reminder, staff should model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. •There are some effective videos about positive language and interactions in the classroom that might be helpful for new staff and as a refresher for more seasoned staffed. Positive language and interaction with children at this critical age, is imperative to help children feel safe to learn, explore and grow. Visiting www.ncrlap.com is a good source to not only prepare staff for the rated license assessment but for interaction videos and hand outs. Child Care Resources offers trainings on “Praise or Encouragement, and Zoning in on Behavioral Prevention as well demand training for new employee training; o A+ Supervision o Keep It Clean o Positive Guidance •Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Family child care homes are required to test water for lead once. o DCDEE encourages you to utilize the Clean Classrooms for Children’s program. You can take the pre-enrollment webinars which is available at www.cleanwaterforUSkids.org/carolina then enroll – At www.cleanwaterforUSkids.org/carolina completing the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos. Keep mindful that once you evaluate potential hazards and receive results if there are recommendations and/or water mitigation support is needed they will provide direction in how to mitigate and ways to fund said mitigation. Upon arrival I was greeted by Avion Edgeworth and Shakira Mugo, Co-Directors. I shared the reason for the visit and they both assisted me with today’s visit. Your program currently operates with a Five (5) Star License effective 07/13/2018. The license was posted, with restrictions to day time care only and meets enhanced ratios and space. I monitored the following items today: supervision, staff/child ratio, space capacity, licensed spaces, staff and children’s records, health, safety, and program records. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three (93%) percent as of 10/31/2023. Ownership: The facility owner is First United Methodist Church Child Development Center, Inc. SoS ID #0053109 which is current and active. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, CPR/First Aid certifications, ITS/SIDS certifications, Emergency Medical Care Plan, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The following inspections are below. • Fire Inspection was conducted on 6/1/23. • The last fire drill was conducted on 10/11/23. • Last Shelter in Place/Lockdown Drill was conducted 10/8/23. • The Sanitation Inspection was completed on 6/21/23 with a Superior rating. • The last Playground Inspection was conducted on 10/5/23. The children were observed in bottle feeding, tummy time, circle time, bubble play, and center time. The staff and teachers were engaging with the children and all interactions observed were nurturing and appropriate. Program Records: We reviewed all the required records and all were in compliance. For future reference, both sides of the playground safety sheet must be filled out each month. Staff Records: The staff-training worksheets were completed before the visit. There was one (1) new staff file to monitor. Medication: Medication was monitored. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. Five (5) violations were observed today. One (1) violation was corrected during visit. One (1) violation was a repeat violation. The violations are as follows: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In room #2 there was not a current plan posted for this week or previous weeks. This is a repeat violation. GS 110-91(12); .0508(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. In the room that serves toddlers, the prescription medication did not have the original label with instructions. .0803(2)(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. New staff member I.T. did not have results indicating they were free of TB before beginning work. .0701(a) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One new staff member did not have a signed document of the Shaken Baby Syndrome and Abusive Head Trauma on file. .0608(d)(1-4) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. In rooms #3 and #5 the medical administration permission form and record were not present for one child in each room. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) On or before 11/13/23 I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected.
