The last annual compliance visit was conducted on 9/14/23. The compliance history percentage for this family childcare home is 90% for the previous 18-month period. This family childcare home currently holds a 4-star rated license. Upon arrival, I was greeted by Ms. Tamara Butts, owner/operator. I shared the reason for the visit. Ms. Butts assisted me with today’s visit. I used the “Annual Compliance Monitoring Checklist for Family Child Care Homes” as a monitoring tool during today’s visit. I monitored supervision, capacity, group size, indoor/outdoor spaces, safety, program records and permit requirements. Observations: Two (2) children were present during the visit. The children were engaged in choice activities and transitioning to outdoor play time. Transportation is not currently provided at this family childcare home. Medication is not being administered. Twelve (12) children are enrolled in the family childcare home. There were three (3) new children enrolled since the last AC visit. The files were reviewed for compliance with the NC Childcare Rules. The operator’s file was monitored as well. The operator shared that she is taking the CPR/first aid trainings today, 9/13/24. The family childcare home’s EPR plan was not available for review during the visit. The Ready To Go file was available for review. The FCCH has a permit to operate issued from the Mecklenburg County Health Department that is valid through 4/2025. The following documents were given to you prior to today’s visit: -VERIFICATION OF REQUIRED INFORMATION FOR OPERATOR AND ADDITIONAL CAREGIVERS. -The OPERATOR’S STATEMENTS OF RESPONSIBILITY. The documents were completed during today’s visit. The child care consultant completed the verification form and reminded the operator that this document must be completed by the operator moving forward. A copy of the computer-generated visit summary was given to you along with a copy of the enrollment/staff/child worksheet and the “Annual Compliance Monitoring Checklist for Family Child Care Homes”. The violations observed during the visit were discussed with Ms. Butts at the end of the visit and documented in the visit summary. The following violations were observed: Violation Number Comment Rule 508 Operator did not successfully complete a first aid course as referenced in Rule.1702(b)(2) First aid training was not renewed on or before the expiration of the certification. The first aid certification for the operator expired 7/2024. .1703(a)(2) 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. The CPR certification for the operator expired 7/2024. .1703(a)(3) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A child's file did not contain the required medical assessment. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1857 The operator did not develop an Emergency Preparedness and Response Plan within four months of completing the Emergency Preparedness and Response in Child Care training and/or the Plan was not completed on the template provided by the Division. The EPR plan was not available for review during today's visit. .1714(c) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. The operator's health and safety trainings were due 7/2024. .1703(d)(2) All violations documented above must be corrected immediately. A letter of compliance needs to be sent to me stating how each violation was corrected and how compliance will be maintained in the future. Clean Classrooms for Carolina Kids will assist FCCH’s with this requirement. Pre-enrollment is required for this service and can be done at www.cleanwaterforUSkids.org/carolina. • During the visit, I discussed with Ms. Butts that the verification form sent to her prior to today’s visit is to be completed by the operator and returned to the child care consultant via email prior to the annual compliance visit. • During today’s visit, a copy of the FCCH outdoor monthly inspection was provided. • During today’s visit, I provided instructions for accessing the EPR template online to update. I observed children engaged in choice activities and outdoor play time. Positive interactions were observed between the operator and the children. Transportation is not provided. Medication is administered. Diaper cream was monitored. This family childcare home holds a permit to operate from the Mecklenburg County Health Department valid through April 2024. Twelve (12) children are enrolled in the family child care home. Two (2) files were reviewed today for compliance with NC Childcare Rules. The operator’s file was monitored as well. The following documents were given to you and completed during the visit: -VERIFICATION OF REQUIRED INFORMATION FOR OPERATOR AND ADDITIONAL CAREGIVERS. -The OPERATOR’S STATEMENTS OF RESPONSIBILITY. A copy of the computer-generated visit summary was given to you along with a copy of the enrollment/staff/child worksheet and the “Annual Compliance Monitoring Checklist for Family Child Care Homes”. The violations observed during the visit were discussed with Ms. Butts at the end of the visit and documented in the visit summary. The following violations were observed: Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. Mushrooms were observed in the outdoor play area. .1719(a)(1)&(17) 1301 Operator did not complete the required number of on-going training hours as specified in rule. The operator did not complete the required on-going training hours. Six additional hours are needed. GS 110-91(11); 10A NCAC 09 .1705(b)(5) 1403 Prescription or over-the-counter medication and topical, non-medical ointment, repellant, lotion, cream, fluoridated toothpaste, or powder was administered to any child without the required information. A parent's medication administration permission form to administer diaper cream was not completed. 10A NCAC 09 .1720(c)(1&2) All violations documented above must be corrected immediately. A letter of compliance needs to be sent to me stating how each violation was corrected and how compliance will be maintained in the future.
