I was greeted by Tracey Harris, Owner/Operator. I stated the purpose of my visit and I conducted a walk-through of the facility unassisted. The program operates with a three-star child care license as a Center Located in a Residence issued December 5, 2019. Restriction: Fireplace/woodstove not used during operating hours; Children in care on ground level only; 1st shift (Daytime care); Meets enhanced ratios; and Meets enhanced space. Children were observed playing with blocks and coloring. The infant was observed drinking milk, sitting in the walker. Children were served peanut butter and jelly sandwiches, green beans, bananas and milk for lunch. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. Ms. Harris' son lives with her and she has two dogs at the facility. Ownership and phone numbers were verified and current. Provider currently does not provide transportation. During this visit we discussed that you are thinking about closing in 2026 after your son graduates. We also discussed being a family childcare home, (FCCH), to save on fees from required inspections. If you are interested in being a FCCH the application can be found on our website, the address will be in the resource section. I also emailed you a copy. Rated License: This facility will remain three stars, and has received the following points: five points in education, two points in program standards, and one quality point. Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. There was no evidence of the inspection being completed. .0605(q) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. There was no record of sleep documentation. .0606(g) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. I was greeted by, Tracey Harris, Owner/Operator I stated the purpose of my visit and we conducted a walk-through of the facility. The program operated with a three-star child care license as a Center Located in a Residence issued December 5, 2019. Children were observed actively playing with dramatic play. Children were served children were served chicken nuggets, peas, bananas, wheat bread, and milk. Children went outside for gross motor play and had access to the climbing structure, push toys, shovels, and a variety of other portable gross motor toys. The attached checklist was used to note the requirements I monitored today. The Operators Statement of Responsibilities, Verification of Required Information/Staff and Training worksheet, and children’s records were completed while I was on site. Ms. Harris reported that she is not currently participating in transportation services, and does not have any firearms onsite. INSPECTIONS: The last sanitation inspection was conducted 07-14-24. The last fire inspection was conducted 01-27-22. The last documented fire drill was conducted 10-14-24. The last documented shelter-in-place or lockdown drill was conducted 10-14-24 The compliance prior to this visit was at 85% Rated License: Ms. Harris has decided that she wants to reapply for rated license reassessment at this time. The application and Program Standards worksheet was completed during this visit. Based on the current information received, the facility will receive five points in education, two points in program standards, and one quality point. Violation Number Comment Rule 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. Children did not wash hands before eating lunch. 15A NCAC 18A .2803(c) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure, and all required posted documents. Due to children napping, interactions will be observed during a later visit. During this visit we discussed criminal background checks and finding alternative vendors to check the fire system. Rated License Summary: Ms. Harris has decided that she wants to reapply for rated license reassessment at this time. During the annual Compliance visit in October 24,2023, It was discussed that Ms. Harris wanted to reapply for stars. During this time the consultant had I sent Ms. Harris a self-study and did not receive it. After reviewing the procedure manual, Ms. Harris does not need to complete a self-study as she is considered a FCCH. The education and program Standards worksheet and other applicable forms were completed during this visit. Ms. Harris does not want to have the environmental rating scale completed. Based on the current information received, the facility will receive : Education: five points, Program Standards: two points, Quality point: One point Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. Restriction: Fireplace/woodstove not used during operating hours; Children in care on ground level only; 1st shift (Daytime care); Meets enhanced ratios; and Meets enhanced space. Children were read several books and were shown colors during circle time. Children were observed actively playing with dramatic play. Children were served children were served fish sticks, green beans, oranges, and milk. Children went outside for gross motor play and had access to the climbing structure, push toys, shovels, and a variety of other portable gross motor toys. The attached checklist was used to note the requirements I monitored today. The Operators Statement of Responsibilities, Staff and Training worksheet, and children’s records were completed while I was on site. Ms. Harris reported that she is not currently participating in transportation services, and does not have any firearms onsite. INSPECTIONS: The last sanitation inspection was conducted 07-20-23.
