Upon arrival I observed several teachers and classrooms were on the playground interacting with children. I observed for a while then walked towards the door, I rang the doorbell. The door was opened and I was greeted by C. Schaal, one of the three administrative staff. I stated the purpose of my visit. When the door opened, Kelly Deal, Director, was in the hallway as well and I greeted her and explained the purpose of my visit. All administrators and the education specialist assisted us during today’s visit. The NC Secretary of State website was viewed before the visit and First Presbyterian Church Schools was current/active as of June 25, 1996. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. This facility currently operates with a five (5) star rated license effective 3/6/2020. Indoor Learning Environment: Upon arrival staff/child ratio was observed and was in compliance. Children were adequately supervised during today’s visit. Classrooms participating in varies activities with the children. Children were also observed napping and eating. Lunch for all children consisted of beef, broccoli, noodles, pineapples and milk. Infants were observed being fed bottles. Outdoor Learning Environment: Your playground is separated into three (3) different sections. The following items were observed in the outdoor learning environment: balls, animals, riding toys, blocks, swing sets, a composite structure, push toys and a picnic table. Additional materials were also locked in a chest outdoors. Children were actively in gross motor activities when observed. INSPECTIONS: The last sanitation inspection was conducted on 07/23/25. The last fire inspection was conducted on 1/24/25. The last documented fire drill was conducted 09/22/25. The last documented shelter-in-place or lockdown drill was conducted 9/10/25. The last documented playground inspection was conducted 09/17/25. The last compliance prior to this visit was 96%. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. The following items were also monitored: Supervision, Staff/child ratio, health, safety, program and management. The following items were posted in the classrooms and/or hallway: Daily schedules, activity plans, staff/child ratio worksheets, arrival/departure procedures, summary of law and menus. This facility does not provide transportation. During this visit we discussed the Pathway to the Stars options and discussed future plans to renovate the facility. The child care wing will be getting the floors replaced as well as new cabinetry and panting of the walls. You all stated that you are considering using the weekday preschool area that is not currently licensed. Violation Number Comment Rule 542 The written feeding plan was not modified as the child's needs changed. Older infant room had one child that did not have their form updated to reflect their current feeding needs. (E.). 10 NCAC 09 .0902(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In the Butterflies classroom there were two expired medication/forms, Dove lotion, and a prescription cream (Statin). .0803(12) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. There was a broken toy truck on the two/three year old playground. 10A NCAC 09 .0604(p) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. Centipedes had an Aquaphor with no child's name assigned. 10A NCAC 09 .0803(2)(b)(i-v) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a proposed timeline of the corrections. In some cases, the timeline may be extended. REQUIRED RESPONSE: (for any violations) You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Children were playing on the playground at arrival. I walked to the facility main door and rang the bell. I was greeted in the foyer by an administrator after I stated the purpose of my visit. Mrs. C. Schaal, Assistant administrator, open the door. The walk through of the facility was conducted with Mrs. Schaal and the staff education review was assisted by Mrs. G. Cawley, education specialist/administrator. At the close of the visit I spoke to all three administrators, including Kelly Deal, Director This facility operates as a five-star center. I observed the children engaged in the following activities: gross motor outdoor play, activity center play. Teachers read stories and sang songs in circle time. Infants were observed actively engaged in crawling, napping and interacting with teachers. In one toddler class, teachers were actively engaged with toddlers in a dump and fill ball play. The second toddler class was actively interacting with children in dramatic play as well as other centers. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure, and all required posted documents. Five new staff files were monitored today. This facility does not provide transportation. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan posted was dated for the week of March 31, 2025 in room 110, and the activity plan in room 104 was only posted outside of the classroom. Children were playing on the playground at arrival. I walked to the facility main door and rang the bell. I was greeted in the foyer by an administrator after I stated the purpose of my visit. Mrs. C. Schaal, Assistant administrator, open the door. The walk through of the facility was conducted with Mrs. Schaal and the staff education review was assisted by Mrs. G. Cawley, education specialist/administrator. At the close of the visit I spoke to all three administrators, including Kelly Deal, Director This facility operates as a five-star center. I observed the children engaged in the following activities: gross motor outdoor play, activity center play. Teachers read stories and sang songs in circle time. Infants were observed actively engaged in crawling, napping and interacting with teachers. In one toddler class, teachers were actively engaged with toddlers in a dump and fill ball play. The second toddler class was actively interacting with children in dramatic play as well as other centers. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure, and all required posted documents. Five new staff files were monitored today. This facility does not provide transportation. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan posted was dated for the week of March 31, 2025 in room 110, and the activity plan in room 104 was only posted outside of the classroom. Upon arrival I was greeted by the Education Specialist and I shared the reason for the visit. Ms. K. Deal Director, assisted me with today’s visit. Your program currently operates with a Five (5) Star License effective 3/6/2020. The license was posted, with restrictions to first (1st) shift care, meets enhanced ratios and enhanced space, and meets the highest voluntary enhanced ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety (90%) percent as of today’s visit. Ownership: The facility owner is who it is owned by First Presbyterian Church Schools. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The Fire Inspection has not occurred since the last visit and remains current. The following items above were found in/out of compliance. The following inspections are below. •Last fire drill was conducted on 3/29/24. •Last Shelter in Place/Lockdown Drill was conducted 2/21/24. •Last Playground Inspection was conducted on 3/13/24. •Last Sanitation Inspection was conducted on 02/13/24 and received a superior classification. •The Incident Log was in compliance and complete. The children were observed in the rooms that serves infant age children to be napping, participating in teacher led song and music/movement. The children in the rooms that serve toddler age children were observed to be engaging in self-directed center play including independent art, dramatic and dress up play and teacher interactions. The children in the rooms that serves preschool age children were participating in teacher direction circle time to include welcoming the children, interactive engagement with the children’s feelings and thoughts on the day. They were also observed to be participating in independent reading practice. All children in all rooms were actively attended to, had their needs met in an appropriate time-frame and seemed to be engaging with each other happily. Program Records: We reviewed all the required records and all were in compliance. Staff Records: The staff-training worksheet was completed at the time of the visit as there was one (1) new staff member. The staff file was in compliance, well organized and easy to review. Medication: Medication was monitored and all medication is in compliance. It is best practice to have any emergency medication permission forms in the rooms which the children are in to maintain compliance and safety. Nutrition: The menu and allergies were posted and lunch consisted of herbed baked chicken, broccoli, oranges, whole wheat rolls and milk. There were substitutions listed and provided for children with allergies. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. One (1) violation was observed today and was corrected during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the room that serves three (3) and four (4) year old there was an electrical outlet that was uncovered. There was some discussion on if the outlets in the rooms were self-protecting and didn't requiring a cover but a cover was provided to correct the violation. 10A NCAC 09 .0604(c) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Rated License •Reach out to your Child Care Consultant for the facility’s star rated cohort assessment and preparation year. Please make the Child Care Consultant aware of when and if you are ready to begin the Star Rated License process within the cohort timeline. There are several activities to prepare your facility for the preparation assessment year and the assessment year. •Also visiting www.ncrlap.org website for short videos and helpful documents is helpful in preparing for the rated license assessments. •Childcare Resources and The Alliance for Children are wonderful community resources to assist with classroom preparation for the assessment year. Technical Assistance •There is a new Summary of the Law on the DCDEE website that should be posted on the information board. •As a reminder, since the church is conducting fire inspections as a whole, the fire inspector is still responsible for completing the DCDEE Fire Inspection form upon a passing inspection and it must be submitted to the Child Care Consultant within seven (7) days. •In Room number four (4) there was a loose magnetic locking system on the diaper changing table. While the structure remains in compliance, please ask your maintenance team look on ways to fix it. •Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Family child care homes are required to test water for lead once. o DCDEE encourages you to utilize the Clean Classrooms for Children’s program. You can take the pre-enrollment webinars which is available at www.cleanwaterforUSkids.org/carolina then enroll – At www.cleanwaterforUSkids.org/carolina completing the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos. Keep mindful that once you evaluate potential hazards and receive results if there are recommendations and/or water mitigation support is needed they will provide direction in how to mitigate and ways to fund said mitigation. The Fire Inspection has not occurred since the last visit and remains current. The following items above were found in/out of compliance. The following inspections are below. •Last fire drill was conducted on 3/29/24. •Last Shelter in Place/Lockdown Drill was conducted 2/21/24. •Last Playground Inspection was conducted on 3/13/24. •Last Sanitation Inspection was conducted on 02/13/24 and received a superior classification. •The Incident Log was in compliance and complete. The children were observed in the rooms that serves infant age children to be napping, participating in teacher led song and music/movement. The children in the rooms that serve toddler age children were observed to be engaging in self-directed center play including independent art, dramatic and dress up play and teacher interactions. The children in the rooms that serves preschool age children were participating in teacher direction circle time to include welcoming the children, interactive engagement with the children’s feelings and thoughts on the day. They were also observed to be participating in independent reading practice. All children in all rooms were actively attended to, had their needs met in an appropriate time-frame and seemed to be engaging with each other happily. Program Records: We reviewed all the required records and all were in compliance. Staff Records: The staff-training worksheet was completed at the time of the visit as there was one (1) new staff member. The staff file was in compliance, well organized and easy to review. Medication: Medication was monitored and all medication is in compliance. It is best practice to have any emergency medication permission forms in the rooms which the children are in to maintain compliance and safety. Nutrition: The menu and allergies were posted and lunch consisted of herbed baked chicken, broccoli, oranges, whole wheat rolls and milk. There were substitutions listed and provided for children with allergies. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. One (1) violation was observed today and was corrected during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the room that serves three (3) and four (4) year old there was an electrical outlet that was uncovered. There was some discussion on if the outlets in the rooms were self-protecting and didn't requiring a cover but a cover was provided to correct the violation. 10A NCAC 09 .0604(c) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. While the structure remains in compliance, please ask your maintenance team look on ways to fix it. •Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years.
