The facility’s last annual compliance visit was conducted September 22, 2025, however, the facility is closed during the summer and the 2026-2027 school year does not begin until August 25, 2026. An annual visit was conducted today to ensure the annual compliance is conducted before September 22, 2026. Upon arrival, I checked into the school’s main office. I was greeted a few minutes later by P. Zeigler. Ms. Zeigler explained that she was substituting for the facility’s Program Coordinator, T. Merritt. Ms. Zeigler assisted me with today’s visit. The children use the cafeteria as their primary space. Activity areas set up for the children today included arts and crafts, writing, math, science, reading and global world studies. Today, I observed the children participating in arrival procedures, personal care routines, and transitions. The Emergency Drill Log was reviewed today. A monthly fire drill was conducted on April 22, 2026. A shelter-in-place drill was conducted May 12, 20206. Public School Off-Site Records Verification forms for staff and children for the 2025-2026 school year were reviewed today. The Staff and Training Worksheet was received. One new employee has been hired since the last annual compliance visit conducted on September 22, 2025. The new employee’s file, the substitute’s file and 10% of existing staff were monitored. The following violation was observed and corrected during the visit. Violation Number Comment Rule 1757 A valid qualification letter was not on file and available to review at the facility. A qualification letter was not on file for a substitute that was present today. G.S. 110-90.2(b) & (d) & .2703(e) Technical Assistance/General Information: Criminal Background Qualification Letters- Qualification letters for all staff must be on file for review. A copy of the substitute's letter was emailed to her during the visit and placed in the file. Activity areas set up for the children today included reading, writing, math, science, art, global world studies, homework activities and career development. Today, I observed the children participating in arrival procedures, personal care routines, and transitions. A shelter-in-place drill was conducted August 29, 2025, and the last fire drill was conducted on September 12, 2025. Public School Off-Site Records Verification forms for staff and children for the 2025-2026 school year were reviewed today. The Staff and Training Worksheet was received. One new employee has been hired since the last temporary time period visit conducted on February 10, 2025. The new employee’s file was monitored and 10% of existing staff. A substitute was present today. Required documentation was on file for the substitute. The following violations were observed. Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. Documentation was not on file that the EMC plan had been reviewed annually with staff. 10A NCAC 09 .0802(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Verification was not on file that the new staff member had completed First Aid training. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Verification was not on file that the new staff member had completed CPR training. .1102(d) 1432 Within the first 6 weeks of assuming responsibility for supervising group of children, each employee did not complete at least 3 additional clock hours of training on topics outlined in this rule. Verification was not on file that the new staff member completed orientation. 10A NCAC 09 .2510(i)(2) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. Documentation was not on file that the School Risk Management plan had been reviewed annually with staff. .0607(e) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before October 6, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. Both training modules can be found on the DCDEE Moodle Learning Platform at https://www.dcdee.moodle.nc.gov/. In-service training hours are received upon completion of the training sessions.
