The five-star licensed DPI licensed afterschool program continued to operate meeting the highest voluntary enhanced ratios and enhanced space requirements. Upon arrival I was greeted by the PC, Ms. Davis. The child care item listing dated April 2025 was used to identify and determine non-compliance. The following sections were monitored for compliance: A1, B1, C4, C5, C6, E1, E2, F3, G1, G3, G4 and G5. The children and staff were observed outside playing on the field. None of the stationary play equipment is used by the children. A walk through of the following spaces was completed: cafeteria and the outdoor learning environment. Lesson plans with at least four different activities were monitored posted. Children were observed eating their PM snack and engaged in center free play. The DPI verification forms were monitored completed, and current. Staff training with complete training logs was monitored for compliance. There have not been any new staff hired since the last visit completed in 2025. The staff and training worksheet were provided prior to the visit. The ABCMS roster report was run prior to the visit. The center’s ABCMS roster report was reviewed and monitored current. A quarterly lock down drill or shelter in place drill was not documented completed on the tracking tool. Monthly outdoor inspections, incident log, monthly fire drills and quarterly safety drills were monitored. The visit summary could not be completed due to program closure and time of day the school day program dismisses. The visit summary will be completed and emailed to Ms. Davis. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A shelter in place or lock down drill was not documented completed, once every three months. .0604(u);.0302(d)(8) Technical Assistance Provided and General Discussion: 1. We discussed the option for Pathway #1 or Pathway #2. The administrator for CMS is waiting to determine if the curriculum has been approved by DCDEE. If the curriculum is approved, CMS ASEP will pursue Pathway #2. The Pathway to the Stars form was completed and reviewed during the visit. 2. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. It was recommended to Ms. Davis: plan the four required quarterly drills on her corporate calendar and fill in the tracking tool as soon as each required drill is completed. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before March 5, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Name of Operation: ELIZABETH TRADITIONAL ELEMENTARY ASEP Facility ID: 60002238 Consultant: LISA EDDINS-SMITH Operation Type: Center Case Number: Visit Date: 6/6/2024 Number Present: 17 Completed Date: 6/6/2024 Age: From 6 To 11 Total Minutes: 150 Time In: 07:00 AM Time Out: 09:30 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for applicable child care requirements during the routine unannounced visit. Andrea Anderson, Child Care Consultant accompanied me today. The facility currently has a Five Star Rated License with an effective date of May 7, 2019. The last annual compliance visit was conducted on February 27, 2024. The facility’s 18-month compliance history before today’s visit was 96%. The facility license and NC Summary of the Child Care Law was posted on the ASEP bulletin board near the office. All information required to be posted was observed posted on the ASEP bulletin board. Upon arrival, we were greeted by Tameka Davis, Program Coordinator and C. Navarro, caregiver. We stated the reason for the visit. As we arrived Ms. Davis informed us that it was the end of the year and she had packed up most of the supplies. She informed us that they are stripping the cafeteria and will be in the Media Center this afternoon and tomorrow. Five interest centers were set up for use. A Canadian Dry Ginger Ale soda was observed next to Ms. Davis as she was playing cards with the children. The facility uses the cafeteria as their primary space. I walked with T. Davis to view the ASEP bulletin board as well as all program records. As children arrived, they were greeted with nurturing and positive interactions. The children were engaged in learning centers to include manipulatives, reading, technology, art and board/card games. The cafeteria staff began setting up during our visit. All cleaning supplies were observed stored properly. The following items were monitored: supervision, staff/child ratio, first aid, CPR, special training, criminal background checks, BSAC, emergency medical care plan, administering of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted and permit restrictions. There have been no new staff hired since the annual compliance visit was conducted on February 27, 2024. We reviewed he Staff and Training Worksheet to confirm staff were current with CPR, First Aid, BSAC training, and criminal background qualifying letters. The DPI Public School verification form for staff records was revised. All current staff were listed. The last fire inspection was conducted on February 2, 2024. The last sanitation inspection was conducted on March 13, 2024, with zero demerits and a Superior rating. The Emergency Drill Log and Report was reviewed today. The last fire drill was conducted on May 28, 2024, and shelter-in-place drill on April 30, 2024. The monthly playground inspections were reviewed and found meeting compliance. No incidents have occurred however an incident log is on file to use when applicable. We were informed that no children currently require medication. The following violations were observed today: Violation Number Comment Rule 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. Upon entry we observes one staff seated playing cards with children. A Canadian Dry Ginger Ale soda was on the table next to her. .0901(i) Corrective Action Plan: All violations were corrected immediately therefore I am not requiring a compliance statement be submitted. Technical Assistance Provided and General Discussion: 1.Access to Moodle was discussed with Ms. Davis and Ms. Navarro. Andrea Anderson was able to help Ms. Navarro log into and register for classes in Moodle. We also informed staff that other Health and Safety Training is available such as Community Healthcare Nurses, North Carolina Health and Safety Resource Center healthychildcare.unc.edu/, and CCRI are other approved agencies. 2.It was suggested that the ASEP program board be moved to the ASEP program space. Parents do see the board in the afternoon at departure. A fold up board to keep in the cafeteria will be implemented for the next school year. 3.It was discussed that while caring for children staff must model appropriate eating behaviors by consuming only food or beverages that meet nutritional requirements per 10A NCAC 09.0900 (j). 4.It was discussed that all materials need to be accessible during all operating hours including the last days of school for before school and after school per facility license.
