You, Ms. T. Tillman, assistant director, assisted me with the visit today. Mrs. Shannon Wilson, owner, arrived during today’s visit. Upon arrival, I stated the purpose of today’s visit and discussed the temporary monitoring requirements and to go over QRIS options for her to make the best decision for her program. I discussed with Ms. Wilson Pathway #1 and Pathway #2 requirements. I explained to Ms. Wilson that I need for her to review the information that I showed her during the visit. We discussed in detail Pathway #2 option. The Administrator/Owner provided information regarding the approaches for the program is taking in selecting an approved curriculum that is most favorable for the program. We also discussed that they are required to select and receive training for all lead teachers for the program’s selected formative assessment. 10A NCAC 09 .3205 CLASSROOM AND INSTRUCTIONAL QUALITY PATHWAY FOR CHILD CARE CENTERS (e) For a child care center to earn a four-star rated license, the following requirements shall be met: (1) The child care center shall meet the enhanced ratio requirements set forth in Rule .3208 of this Section. The child care center may choose to meet the enhanced space requirements set forth in Rule .3210 of this Section. (2) At least fifty percent of lead teachers and at least fifty percent of educators shall meet the individual education standards at the four-star level as set forth in Rules .3211, .3212, .3213, .3214, .3215, .3216 and .3218 of this Section or higher. (3) The child care center shall implement the family and community engagement standards set forth in Rule .3219 of this Section for a four-star license. (4) The child care center shall implement the continuous quality improvement standards set forth in Rule .3221 of this Section. (5) The child care center shall implement a curriculum that has been approved by the Commission as defined in Rule .0102(11) of this Chapter for all ages served. A list of curricula that has been approved by the Commission is available at http://ncchildcare.ncdhhs.gov/Services/Licensing/Star-Rated-License.Curriculum. (6) In curriculum planning, the child care center shall ensure modifications and adaptations are made to involve all children with special health and developmental needs. (7) The child care center shall conduct on-going formative assessments to gather information about each child's growth and skill development based on individual development and learning needs and maintain evidence of each child's on-going assessment. The child care center shall use a formative assessment(s) that has been approved by the Commission, available at https://ncchildcare.ncdhhs.gov/Services/Licensing/Star-Rated-License/Curriculum. Evidence of Effective July 1, 2025 formative assessments shall include documentation required or contemplated by the approved formative assessment and may also include: (A) anecdotal notes; (B) portfolio of child's work samples; (C) developmental screenings; or (D) photographs, audio, or video recordings with permission from the child's family. (8) The lead teacher shall share the results of the formative assessment with families at least once annually. The manner in which the results are shared shall be convenient for the family such as by telephone, email, virtually, or in-person. (9) The administrator shall complete training related to the approved curriculum and formative assessment tool that is used by center staff with children, in addition to applicable requirements in Rule .1103 of this Chapter. Verification of this training shall be maintained in the administrator's personnel file. (10) The administrator shall participate in one of the following activities regarding classroom and instructional quality practices: (A) annual completion of five hours of coaching or mentoring by another administrator of a licensed child care facility with a star-rating of four or five stars or a community resource partner; (B) annual completion of five on-going training hours, in addition to applicable requirements in Rule .1103 of this Chapter and Rules .3211, .3212, .3213, .3214, .3215, .3216 and .3218 of this Section; or (C) annual completion of 0.5 continuing education unit, in addition to applicable requirements in Rule .1103 of this Chapter and Rules .3211, .3212, .3213, .3214, .3215, .3216 and .3218 of this Section. In regard coaching/training for administrator and her lead teachers. I stated that she would just identify this individual on the approved documentation for the CQI forms. Currently this center operates with a Temporary License, issued from 03/05/2026 to 09/05/2026. The restrictions include daytime care only, with the building capacity of 135 for first shift and 135 on second shift; Age ranging from 0-12 years of age. Indoor Learning Environment: There were 51 children present and 68 children enrolled in building. The children were engaged in playing in their separate areas within the learning area, bottle feeding, and transitioning to the outdoor learning environment. The outdoor learning environment: During today’s visit the outdoor learning environment was monitored and no violations was cited. The fire drill report was discussed today and was observed in compliance with the latest date of 5/21/2026. The playground inspection was observed completed on 6/01/2026. The most recent sanitation inspection was completed on 04/02/26. During the visit, the Child Care Center Item Number Listing (DCD 0357) was used as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to this facility. In addition to monitoring for health and safety, 10 percent of children’s records, center files and required postings. The following items were posted in the designated environment within the facility: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, arrival/departure procedures, summary of law, and the Temporary License. Records: Staff and training worksheet was completed prior to today’s visit. The administrator/owner has had one (1) staff hired since my last visit in May 2026. Weapons: Your facility is in compliance with Child Care Requirements regarding firearms. I inquired about the hours Ms. Wheeler is onsite to carrying out her administrator’s duties if she is the administrator onsite. Ms. Shannon, the owner is not required to be onsite but at time also plays an important administrator’s role when on site. 10A NCAC 09 .0714 OTHER STAFFING REQUIREMENTS (a) Each child care center shall have a child care administrator who shall be responsible for monitoring the program and overseeing administrative duties of the center. This requirement may be met by having one or more people on site who meet the requirements for a child care administrator as set forth in G.S. 110-91(8) and according to the licensed capacity of the center. The child care administrator shall be on-site in accordance with the following chart: Licensed Capacity Weekly On-Site Hours Less than 30 children 20 30-79 children 25 80-199 children 30 200 or more children 40. The child care administrator's required weekly hours may include those hours that he or she is off-site due to administrative duties, illness or vacation. Ms. Wheeler hours are Monday through Thursday, 10:00 a.m. to 4:00 p.m. with some days, extended to 5:30 p.m. One violation was cited and corrected during today’s visit. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. One medication was observed without being in the original labeled container during the visit. .0803(2)(a) We discussed Early Head Start Staff Child Ratios versus Minimum Requirements. I indicated that while on a temporary license, minimum staff/child ratios are expected to be always followed. We discussed that the program went through a change of ownership in March 2026. The previous owner, understanding of Early Head Start Requirements in regard to Staff/Child Ratios, was to maintain ratios as best as they could or use State ratios when EHS staff called off sick or were on vacation. I stated that once approved as an Early Head Start program, they must follow all EHS staff/child ratios throughout the day. If an EHS staff member is out sick, they must communicate with EHS Grantee Management for proper guidance. Pathway recap: We discussed the pathway to the stars requirements towards obtaining a 5-star license. You stated that you do not want to apply for the ERS Assessment. I discussed the documentation needed to submit with the application for the 5-star license and asked that she complete and return all requested documents by June 18, 2026. I also explained, discussed enhanced ratios that will be required and the optional of enhanced space with Ms. Wilson. I also reiterated that the building inspection will be required prior to the expiration of the temporary permit. I need the certificate of compliance or certificate of occupancy as soon as possible. Ms. Wilson stated that the building inspection should be completed by next week. Your temporary license will expire on September 5, 2026, and no extension of time will be given. WORKS: I will review the information for you on the DCDEE WORKS system from the staff and training worksheet that was printed for me during today’s visit. Technical Assistance: ERS Application, Facility CQI, and Family Engagement forms were all emailed to Ms. Doberstein prior to today’s visit. I also recommend that they use food prep sink for only food prepping (bottle feeding and plating children’s food). I also recommend that teachers or administrator review over the counter medication such as diaper creams or sunscreen, making sure the amount to place on a child is appropriate. I reviewed several forms that indicated all over the child’s body. The amount was not indicated on the form. I also discussed any medication that is prescribed to a child will need that have the medication in the original container with the child’s name and date. Ms. Shannon, the owner is not required to be onsite but at time also plays an important administrator’s role when on site. 10A NCAC 09 .0714 OTHER STAFFING REQUIREMENTS (a) Each child care center shall have a child care administrator who shall be responsible for monitoring the program and overseeing administrative duties of the center. This requirement may be met by having one or more people on site who meet the requirements for a child care administrator as set forth in G.S. 110-91(8) and according to the licensed capacity of the center. The child care administrator shall be on-site in accordance with the following chart: Licensed Capacity Weekly On-Site Hours Less than 30 children 20 30-79 children 25 80-199 children 30 200 or more children 40. The child care administrator's required weekly hours may include those hours that he or she is off-site due to administrative duties, illness or vacation. Ms. Wheeler hours are Monday through Thursday, 10:00 a.m. to 4:00 p.m. with some days, extended to 5:30 p.m. One violation was cited and corrected during today’s visit. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. One medication was observed without being in the original labeled container during the visit. .0803(2)(a) We discussed Early Head Start Staff Child Ratios versus Minimum Requirements. I indicated that while on a temporary license, minimum staff/child ratios are expected to be always followed. We discussed that the program went through a change of ownership in March 2026. The previous owner, understanding of Early Head Start Requirements in regard to Staff/Child Ratios, was to maintain ratios as best as they could or use State ratios when EHS staff called off sick or were on vacation. I stated that once approved as an Early Head Start program, they must follow all EHS staff/child ratios throughout the day. If an EHS staff member is out sick, they must communicate with EHS Grantee Management for proper guidance. Pathway recap: We discussed the pathway to the stars requirements towards obtaining a 5-star license. You stated that you do not want to apply for the ERS Assessment. I discussed the documentation needed to submit with the application for the 5-star license and asked that she complete and return all requested documents by June 18, 2026. I also explained, discussed enhanced ratios that will be required and the optional of enhanced space with Ms. Wilson. I also reiterated that the building inspection will be required prior to the expiration of the temporary permit. I need the certificate of compliance or certificate of occupancy as soon as possible. Ms. Wilson stated that the building inspection should be completed by next week. Your temporary license will expire on September 5, 2026, and no extension of time will be given. WORKS: I will review the information for you on the DCDEE WORKS system from the staff and training worksheet that was printed for me during today’s visit. Technical Assistance: ERS Application, Facility CQI, and Family Engagement forms were all emailed to Ms. Doberstein prior to today’s visit. I also recommend that they use food prep sink for only food prepping (bottle feeding and plating children’s food). I also recommend that teachers or administrator review over the counter medication such as diaper creams or sunscreen, making sure the amount to place on a child is appropriate. I reviewed several forms that indicated all over the child’s body. The amount was not indicated on the form. I also discussed any medication that is prescribed to a child will need that have the medication in the original container with the child’s name and date. Upon arrival, I was greeted by Ms. Tracy Tillman, assistant director and Ms. Angela Wheeler, Administrator. I asked if the owner, Ms. Shannon Wilson, was present and it was stated she would be arriving shortly. I shared the reason for my visit. I asked if the cook, Ms. Cindy Anderson, was present. It was shared that Ms. Anderson’s last day of employment was April 10, 2026. After the departure of Ms. Anderson, the Administrator, Administrator’s Assistance, and Owner take turns preparing meals. I interviewed four (4)staff members and observed the April 2026 Menu, and it was shared that on April 28, 2026, lunch consisted of lunch meat and mashed potatoes; April 29, 2026, lunch was chicken, mixed vegetables, and tater tots; and April 30, 2026, lunch was cheeseburger and tater tots. During today’s visit, lunch consisted of Mac and Cheese, Mixed Vegetable, fruit, milk and roll. I also inquired about the telephone connected to the childcare facility. What I have documented in our Regulatory system, 704-392-4446. I asked the administrator(s), if the telephone number has changed since the request was made to make the above telephone number the facility’s number. The administrator stated that the number that I stated is the current number. Prior to the start of visit, I called the number to make sure the number is working properly. The phone number was verified as working properly. There were 66 children present during today’s visit. I observed the children participating in free play, outdoor play, routines, and lunch routines. Supervision, staff/child ratios, group size, capacity and space were observed in compliance today. Findings: -I shared that children must be served all meal components. -It was shared that meal components and portion size may not be accurate. I explained to all four (4) interviewed staff the meal patterns for children, listed above, were also posted in the kitchen area for easy reference. I highly recommend to educate your staff and families on portion sizes to make sure everyone is aware of what the requirement is for all children according to their age. - I shared that an administrative action may be issued because I am citing one (1) allegation regarding a child care requirement, regarding 10A NCAC 09 .0901 GENERAL NUTRITION REQUIREMENTS (b) When food is prepared by or provided by the center, menus for nutritious meals and snacks shall be planned at least one week in advance. At least one dated copy of the current week's menu shall be posted where it can be seen by parents and food preparation staff when food is prepared or provided by the center. A variety of food shall be included in meals and snacks. Any substitution shall be of comparable food value and shall be recorded on the menu prior to the meal or snack being served. On April 29, 2026, Meatballs, w/gravy, mixed veggies, diced pears, brown rice, and milk that was written on the menu wasn’t the same meal that was observed on the Brightwheel app that showed the children were given baked chicken, tater tots, mixed veggies, and milk. Based on interviews and documentation, the allegations of violations of child care requirements are confirmed. Ms. Shannon Wilson, Owner, arrived during the visit. There was (1) one violation and substantiated during today’s visit. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. On April 29, 2026, the lunch menu components could not be verified with the pictures that were viewed on BrightWheel app on this same date. 10A NCAC 09 .0901(b) Technical Assistance: -I shared with Ms. Wheeler, Ms. Tillman, and Ms. Wilson regarding the menu, please revise (making substitution) the menu to what the children will be eating prior to serving the food to children. -I shared that children must be served all meal components. -It was shared that meal components and portion size may not be accurate. I met Jacqueline Hargette, Administrator, Tracy Tillman, Administrator Assistant, Shannon Wilson, Owner, upon arrival and discussed this visit as the center’s first temporary time period visit. We conducted a walk-through of the facility. Currently this center operates with a Temporary License, issued on March 5, 2026. The restrictions include first and second shift care; Restricted age 0-12 years of age and children less than 2 ½ years in age are in rooms with direct exits. The facility licensing capacity is 135 for first and second shift. The NC Secretary of State website was viewed before the visit and Shannon’s Little Angels of the Carolinas, LLC, active/current May 10, 2018. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. Indoor Learning Environment: As we began the walk-through of the facility, I spoke to all individuals mentioned about required documents that should be posted for families to view. The bulletin board was filled with documents that were not required and/or needing the new owner’s information to be posted. This building completed a change of ownership on March 5, 2026, and the previous owner’s information was observed in some postings. The building has eight (8) identified learning spaces that were previously approved for use. There is a total of 96 children enrolled on first shift. A total of 56 children were observed present. No children are enrolled on second shift. Outdoor Learning Environment: There are three (3) designated spaces for children to utilize for gross motor play. We discussed having someone in the early morning going through each outdoor learning environment to make sure the outside public doesn’t throw trash on to the playground. It was told to me that only two (2) outdoor learning environments are being utilized daily. A fire drill took place on 3/19/2026. Sanitation Inspection took place on 3/19/26 and received a Provisional Status with 21 Demerits. On 4/2/2026 with 11 Demerits and a Superior Rating. Transportation: This program currently does participate in Transportation. The 2013 Private Passenger Vehicle was monitored during today’s visit. Insurance: Progressive Southeastern-870766162 expiration date 3/6/27. Tag: MCK 1363 Records: Three children’s records were reviewed during today’s visit, with no violations found. All staff records were reviewed during today’s visit. Nutrition: This program offers breakfast, lunch and afternoon snacks. Lunch met the nutritional guidelines during today’s visit. Lunch consisted of Meatballs with gravy, green beans, rice, slice apples, and milk. Weapons: Your facility is in compliance with Child Care Requirements regarding firearms. The following violations were observed and corrected during today’s visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In space #4, the activity plan was dated for May 2026. GS 110-91(12); .0508(a) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In space #4 I observed plastic bags accessible to children that are under the age of three. .0604(q) I spoke to Ms. Wilson about making sure that the program is obtaining required Building Inspection prior to the expiration date of September 5, 2026 date to prevent Administrative Action with Provisional Status. I highly recommend they contact each entity immediately to get all inspections completed. I sent a letter to all parties mentioned indicating that a change of ownership took place on March 5, 2026. Compliance History: The next two Temporary Time Period visits will be unannounced. During those visits a full assessment of the center's ability to maintain compliance with all applicable Child Care Requirements will be conducted. Please note that any violations cited today or during future visits will negatively impact your compliance history. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. As discussed, at the end of your temporary license period you must be at least a three (3) star facility in order to continue to provide care for children who receive subsidies. WORKS: If you haven’t done so as of today, please make sure your staff establish an NCID for entering their information in the DCDEE WORKS system. Once all staff have completed entering their information in the DCDEE WORKS system, then mail the official transcripts to the Workforce and let me know when they are mailed. Refer to the information on the Division’s website for detailed instructions regarding how to use DCDEE WORKS. To help child care providers meet the new training requirements, the Division of Child has free Moodle online training modules. Click on the Course Catalog tab at the top of the page on the website to login to your training. Please make sure you are marking minimum standards on the staff/ratio worksheet for all classrooms in operation. Technical Assistance: I suggest organizing the staff files by using the checklist offered by the Division. Due to this being a change of ownership, there were several duplicates and or change of owner names on paperwork making it challenging during the review of staff records. The staff medical information should be kept separate and up-to date for the health questionnaire that is required to be completed annually. I highly recommend that you spend time reviewing the ncchildcare.dhhs.gov website, visit QRIS Modernization information regarding the Rated License Assessment, Education Requirements for (Administrator, Lead Teacher, Teacher, Group Leader). NCRLAP.org website for additional information regarding interactions, self-study, and required materials. Make sure you go through the entire building to change the previous owner’s information to the new facility’s name and information. Staff and Children paperwork: Please go through and reduce unnecessary paperwork in each staff and child’s file. It is important for licensed providers to be knowledgeable of all of the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time.
