The last Annual Compliance visit was made on 10/10/2024. The current 5 star license was issued on 6/22/2020. The April 2025 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 98% as of today. Program records were made available to me. The following was observed posted: current license, Summary of Law, Safe Arrival and Departure procedures, Menu, Emergency Care Plan, and no smoking signage. A Sanitation inspection was conducted on 5/12/2025. A Fire inspection was conducted on 9/15/2025. Monthly Fire Drills, Playground Inspections, and EPR Drills were monitored. A sample of staff files including five (5) new staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. A sample of children's files were monitored for Application, Emergency Care authorization, Summary of Law, Medical/Immunization, Discipline Statement, No smoking statement, Safe Sleep Policy, Infant Feeding Schedule and Shaken Baby Syndrome Policy (if applicable). This facility does not provide transportation. The following violations were cited: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted in space two (2). GS 110-91(12); .0508(a) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Vaseline was observed on a shelf in the bathroom. 15A NCAC 18A .2820(d) A walk through was not conducted during today's visit due to it being picture day for the staff and children and because of illness. A follow-up visit will be made in the near future to complete today's visit. Rated License A technical assistance visit was made my me on May 5, 2025. The environment rating scales were requested for this facility on May 15, 2025. The results were received on July 3, 2025. The results were discussed with the director on July 14, 2025. The facility submitted a grievance on July 17, 2025. The results of the grievance were received on August 13, 2025. On September 15, 2025, a discussion was had with the director to wait until the QRIS Pathways came into effect before we moved forward with the Rated License process. The last Annual Compliance visit was made on 10/24/24. The current 5 star license was issued on 6/22/2020. The March 2024 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 88% as of today. I was let into the facility by administrator Ms. Selina Watts. She accompanied me on the walk through of the facility. Facility files were made accessible to me. Eleven (11) classrooms were monitored on today for: Sign in/out, allergy list, medical action plans (if applicable), menu, schedule, activity plan, and staff/child ratio chart. Storage of medication and permission to administer was monitored. Supervision, and storage of hazardous products was monitored. Children were observed having indoor and outdoor free play, teacher directed activities, and having routine needs met. There were ample toys and activities available. The following was observed posted: current license, Summary of Law, Safe Arrival and Departure procedures, Menu, Emergency Care Plan, and no smoking signage. A Sanitation inspection was conducted on 10/01/2024 . A Fire inspection was conducted on 09/12/2024. Monthly Fire Drills, Playground Inspections, and EPR Drills were monitored. A sample of staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. The August 2023 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 91% as of today. I was let into the facility by director Selina Watts. A walk through of the facility was completed. Eleven (11) classrooms were monitored on today for: Sign in/out, allergy list, medical action plans (if applicable), menu, schedule, activity plan, and staff/child ratio chart. Children were observed having indoor/outdoor free play and having routine needs met. There were ample toys and activities available. The kitchen, bathrooms, and outdoor play areas used by children were monitored today. The following violations were cited during today's visit: Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. There was not thermometer in the refrigerator in space nine (9). The thermometer in space ten (10) displayed a temperature of 50 degrees during the walk through. It was checked during the final walk through and it displayed 40 degrees before my departure. 15A NCAC 18A .2806(j)(2) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Prescription medication in space five (5) was not in locked storage. 15A NCAC 18A .2820(d) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Three (3) staff did not have an annual health questionnaire on file. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually.
