This facility currently operates with a four (4) star rated license effective June 19, 2019. Upon arrival Mr. Campbell was outside, I greeted him and he walked inside. After I gathered my things I walked to the door and Mr. Campbell opened the door. As I walked in I greeted Mrs. Doris Campbell, owner and stated the purpose of my visit. I proceeded with the monitoring visit partially assisted by Mrs. Campbell. There were four (4) children present today. Children were observed engaged in painting/art activity, blocks and playing in activity centers. The caregiver was observed reading and singing with children. Mr. Campbell assisted Mrs. Campbell with the children during this visit while the provider prepared lunch. Lunch consisted of wheat bread, pinto beans, green beans, apple sauce, and milk. INSPECTIONS: The last documented fire drill was conducted on 4-13-2026. The last documented shelter-in-place or lockdown drill was conducted 4-13-2026. The last documented playground inspection was conducted 05-01-2025. The attached checklist was used to note the requirements we monitored today. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility does not provide transportation. The provider stated when asked that there are no firearms on the premises. Prior to the visit Case Work Assist and ABCDMS was monitored . Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. Outdoor had two piles of leaves, and grass in the corner of the fence, and near the shed. .1719(a)(1)&(17) Violation: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. You have stated that you do not want to participate in the environmental rating scale and that you are not interested in the new pathways. Please note that despite your five points in education you will not be able to maintain your current four star level and will be at risk of becoming a two or three star. In the meantime please complete your application and send to me before October 1, 2026. You will need to complete you Continuous Quality Improvement Plan (CQI), and decide which Family and Community Engagement Foundational practices you will/are doing. Pathway one Program Assessment: If you will do this pathway you will also need to complete a three month Program assessment self study using the FCCERS and have an assesment to be used for goals for your CQI to become a three star facility. Pathway Two Classroom and Instructional quality: You will have to identify the curriculum that you will use, start conducting child observations and document coaching/training that you have participated in to become a three star facility. To become a four star you will need to purchase, and get trained in a approved curriculum and assessment for all ages served. And start conducting child assessments annually in addition to the things needed to become a three star. Pathway three accreditation: To become three star you will need to apply and pay for accreditation with the following: National Early Childhood Program Accreditation (NECPA) National Association for Family Child Care (NAFCC) o American Montessori Society (AMS) International Montessori Council (IMC) You will automatically become a three star facility and we will use your education to maintain five stars. Lunch consisted of wheat bread peanut butter and jelly, beans, mixed vegetables, apple sauce, and milk. After lunch children transition to diapering and toileting and preparing for nap. INSPECTIONS: The last documented fire drill was conducted on 5-13-2025. The last documented shelter-in-place or lockdown drill was conducted 4-11-2025. The last documented playground inspection was conducted 05-01-2025. The attached checklist was used to note the requirements we monitored today. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility does not provide transportation. The provider stated when asked that there are no firearms on the premises. Rated License: I received the full Rated License reassessment application and the self-study today. Mrs. Campbell will not participate in the environmental rating scale. Program standards: The facility will receive two points. Education: the facility will receive seven points. Quality point: The facility will receive one point. Once the application is processed you will receive your four (4) stars license based on seven points in education, two points in program and one point in quality point. Violation Number Comment Rule 811 Baby food, snack items, and meal items sent from home, for each individual child, were not dated and labeled with the child's name. Infant bottle did not have a name or date present. .1706(o) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. I was greeted at the door by Mrs. Doris Campbell, the owner and operator. I stated the purpose of my visit and conducted a walkthrough of the facility unassisted. This facility currently only operates one approved shift. On arrival there were four children present. Children were observed transitioning from lunch to toileting and napping. Children will be served peas, apple sauce, wheat crackers with peanut butter, and milk for lunch. The infant was observed being held/cuddled during this visit. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure, and all required posted documents. It was stated that no firearms are in the facility. Mr. and Mrs. Campbell are the only ones who currently reside in the home. We discussed the new Stars system and the new environmental rating scales. We will complete the Rated License during the next visit. Violation Number Comment Rule 1889 Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. The bedroom in the kitchen area was unlocked and had lotions and other items in potential reach of children. .1719(a)(7) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately.
