The last Annual Compliance visit was made on 11/16/24. The current 5 star license was issued on 4/29/2022. The March 2024 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 89% as of today. I was let into the facility by administrator Ms. Veronica Briscoe. She accompanied me on the walk through of the facility. Facility files were made accessible to me. Four (4) classrooms including a NC Pre-k classroom was monitored on today for: Sign in/out, allergy list, medical action plans (if applicable), menu, schedule, activity plan, and staff/child ratio chart. Storage of medication and permission to administer was monitored. Supervision, and storage of hazardous products was monitored. The NC Pre-k classrooms was monitored for parent involvement and communication. Children were observed having indoor free play, teacher directed activities, and having routine needs met. There were ample toys and activities available. The following was observed posted: current license, Summary of Law, Safe Arrival and Departure procedures, Menu, Emergency Care Plan, and no smoking signage. A Sanitation inspection was conducted on 06/13/2024 . A Fire inspection was conducted on 01/12/2024. Monthly Fire Drills, Playground Inspections, and EPR Drills were monitored. A sample of staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. A sample of children's files were monitored for Application, Emergency Care authorization, Summary of Law, Medical/Immunization, Discipline Statement, No smoking statement, Safe Sleep Policy, Infant Feeding Schedule and Shaken Baby Syndrome Policy (if applicable). This facility does not provide transportation. The following violations were cited during today's visit: Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Parent's medication authorization for one (1) child did not include the "to date" of which medication can be administered for a "from date" of 1/15/24. 10A NCAC 09 .0803(4)(6-9) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One (1) new staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. .1101(a) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. Emergency medical care information was not updated as changes occurred or at least annually for two (2) children. .0802(c) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The EPR Plan was not updated annually. .0607(e) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The depth of the loose surfacing under the climbing structures on the younger preschool playground was not based on critical height of the equipment. .0605(k)(1-4) 1947 An individual on a provisional criminal background check status was not supervised by a staff person who was determined to be qualified. An individual on a provisional criminal background check status in space two (2) was not supervised by a staff person who was determined to be qualified. G.S. 110-90.2(b) & .2703(f) Compliance -Violations cited today must be corrected immediately. Please send me a letter of compliance of how the violations cited were corrected by November 21, 2023. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Technical Assistance Please have M. Carrasco to check to see if an update criminal background letter has been issued. Her provisional expires on 11/14/2024. Name of Operation: CREATIVE PLAY PRESCHOOL Facility ID: 60003877 Consultant: DENISE WATSON Operation Type: Center Case Number: 1223-230L Visit Date: 1/3/2024 Number Present: 33 Completed Date: 1/3/2024 Age: From 0 To 5 Total Minutes: 70 Time In: 10:35 AM Time Out: 11:45 AM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance of all applicable child care requirements during a complaint visit. The complaint report alleges violations of child care requirements at this facility. During the visit, I discussed the allegations with the directors, Veronica Briscoe, and Debra Clark. This was reported to me on 12/20/2023. The incident took place on 12/19/2023. The reporter was directed to intake. The center was closed the week of 12/24/2023. Allegations: There are concerns that: a child was not adequately supervised. The child was left a lone and unsupervised in the classroom when the others went outdoors. Both directors accompanied me on the walk through of the facility. While documenting enrollment, eleven (11) children were observed present in space one (1) with one teacher. The youngest child present was three (3) years old. Staff:child ratio was not met for the youngest age present. The staff:child ratio chart was posted, and used to explain to the directors that the ratio for the youngest child present has to be met. Mrs. Clark stayed to maintain ratio in space one (1). Based on the self report, the allegation that a child was not adequately supervised was found SUBSTANTIATED. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On 12/19/2023, a child was left alone and unsupervised in the classroom when the others went outdoors. .1801(a)(1-5) 1756 Enhanced staff/child ratios and group sizes were not met. Eleven (11) children were present with the youngest being three (3) with one (1)teacher in space one (1). 10A NCAC 09 .2818 Compliance -Violations cited today must be corrected immediately. Please send me a letter documenting how violations cited today were corrected by January 17, 2024. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Because supervision and ratio violations were cited, a follow-up visit will be made in the near future to ensure compliance. Because a supervision allegation was substantiated, an administrative action may be taken against this facility. Mrs. Briscoe stated that appropriate supervision has been discussed with all teachers, and the teacher that left the child in the classroom was terminated. Please continue to move children immediately or place staff in classrooms to maintain ratio. Remind staff to use zone supervision when there is multiple staff in ratio. When there is one (1) staff in ratio, have staff position themselves where their vision and hearing is maximized for the entire group in care. Make sure staff are aware of the staff:child ratio for the group they are caring for and that they must follow the ratio for the youngest child present in their group.
