    Name of Operation: CHANDLER CHRISTIAN FAMILY CHILD CARE HOME Facility ID: 60002005 Consultant: STEPHANIE PALMER Operation Type: Family CC Home Case Number: Visit Date: 3/11/2026 Number Present: 6 Completed Date: 3/11/2026 Age: From 1 To 6 Total Minutes: 120 Time In: 09:10 AM Time Out: 11:10 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of this unannounced visit was to monitor applicable child care rules. This family childcare home has a five-star rated license and the compliance history percentage prior to this visit was 95%. Upon arrival, I was greeted by Ms. Wanda Chandler-Tillman, owner/operator. I stated the reason for my visit. Ms. Chandler-Tillman assisted me with today’s visit. The following was monitored during this visit: supervision, capacity, safety, adequate/approved space, program records, license posted, permit restrictions, discipline and storage of hazardous products and medications. This family childcare home has a permit to operate from Mecklenburg Health department which is valid through 6/2026. The latest sanitation inspection was conducted on 7/3/25 with six (6) demerits and a status A code. There were six (6) children present during the visit. I observed the children engaged in choice activities, music and movement and table activity. Medication is not currently administered at this family childcare home. There are nine (9) children enrolled in this family child care home. Ms. Chandler-Tillman’s certifications for CPR and first aid are current and expire 5/2027. ITS-SIDS is valid through 9/2027. The operator’s qualification letter is valid expired 5/2026. The qualification letter for household member T. Tillman has expired, effective, 2/4/26. The ABCMS system was checked during the visit and the household member is qualified, but expired. I discussed with Ms. Chandler-Tillman that the qualification letter must be obtained within 15 days and to start the process on the CBC unit accessible through the DCDEE website. The violations cited during the visit were discussed with Ms. Chandler-Tillman and documented in the visit summary. The following violations were observed during the visit: Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. The outdoor area had scrap metal pieces along the pathway to the family childcare home entrance. .1719(a)(1)&(17) 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The qualification letter for household member, T. Tillman expired on 2/4/26. G.S. 110-90.2 & .2703(n) & (o) 1831 A valid qualification letter was not on file and available for review at the facility. A valid qualification letter was not on file for household member, T. Tillman. The grass in the outdoor learning environment needs to be cut and weeds removed. 15A NCAC 18A .2831 (i) Grass, fruit and vegetable gardens, vines on fences, and other vegetation shall be maintained to prevent the harboring and breeding of pests. All violations documented above must be corrected immediately. A letter needs to be sent to me stating how each violation was corrected and how compliance will be maintained in the future. Please include the violation number, statement of compliance, license ID and your signature in the letter. Your compliance letter must be received, via email or mail by March 25, 2026.
