The last Annual Compliance visit was made on 9/5/2024. The current 1 star license was issued on 2/9/17. The April 2025 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a current compliance history of 97%. This facility's space was monitored for: Sign in/out, menu, schedule, activity plan, and staff/child ratio chart. A Sanitation inspection was conducted on 6/12/2025. A Fire inspection was conducted on 12/9/2024. The following was observed posted: current license, Summary of Law, Safe Arrival and Departure procedures, Emergency Care Plan, and no smoking signage. The bathroom, and outdoor area was monitored today. There was tissue and paper towels available and a working thermometer in the kitchen refrigerator. Facility files were monitored for Monthly Fire Drills, Playground Inspections, and EPR Drills. Children were observed having teacher directed and free play activities. Children's files were monitored for Application, Emergency Care authorization, Summary of Law, Immunization, Discipline Statement, No smoking statement, and Shaken Baby Syndrome Policy (if applicable). Staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. Medication storage and requirements were monitored. Feeding plans and nutritional requirements were monitored. This facility does not provide transportation. The following violations were cited during today's visit: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One (1) child care provider did not have a medical report on file prior to employment that was signed by a health care professional. 10A NCAC 09 .0701(a) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. The November 2024 Master Center Item Number Listing was used to complete today’s visit. I was greeted by teacher Haley Hayes. She was taking a group of six (6) children for outdoor play. She accompanied me back into the facility and made staff and facility files available to me. Owner Carolyn Lane was providing routine care for (3) children and supervised tummy time. Lunch was served during the visit. Interactions were positive and encouraging. Supervision, Staff Child Ratio, First Aid, Special Training, Storage of Hazardous Substances and Medication, Approved Space, Staff Records, Program Records, License Posted, and Permit Restrictions were monitored today. The following violations was cited during today's visit: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The March 2024 Master Center Item Number Listing was used to complete today’s visit. I was greeted by owner Carolyn Lane. She and additional caregiver Hailey Hayes were present with ten (10) children. Children were observed completing lunch and transitioning to free play activities. Interactions were positive and encouraging. Supervision, Staff Child Ratio, First Aid, Special Training, Storage of Hazardous Substances and Medication, Approved Space, Staff Records, Program Records, License Posted, and Permit Restrictions were monitored today. The following violations were cited during today's visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Two pack and plays were located at the entrance of the facility blocking the door. 10A NCAC 09 .0601(a) Compliance -Violations cited today must be corrected immediately. Technical Assistance -Even though infants can roll over and may do so when they're placed down on their backs to sleep, staff should be documenting (B) for "back to sleep" once the child is placed down. Even if they immediately change their sleep position, the new sleep position should be documented at the fifteen minute safe sleep check, (B) back, (T) tummy, (S) side. -Remember music should be played in fifteen (15) minute intervals unless played for a purpose such as music and movement or quiet music for rest or transition time. -Bouncy seats, "excersaucers", and other seating apparatus' should be used at a minimum as well.
