Upon arrival I rang the doorbell and S. Brendle answered the door. I stated the purpose of my visit, and we proceeded with the walk through the facility. This facility currently operates with a five (5) star rated license effective August 29, 2019. Ownership information was confirmed as Bright Horizons Children’s Centers, LLC. Prior to today’s visit the Corporation was verified as being Active. The facilities address, and telephone number were verified during today’s visit. INSPECTIONS: The last sanitation inspection was conducted 01/08/26. The last fire inspection was conducted 11-11-2025. The last documented fire drill was conducted 01-30-2026. The last documented shelter-in-place or lockdown drill was conducted 12-05-2025. The last documented playground inspection was conducted 01/15/2026. The most recent compliance history was 91% prior to today’s visit. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management. The Emergency Preparedness Binder was reviewed. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. Ms. Sonya updated/completed the Staff and Training Worksheet while I was onsite. Children were observed playing in activity centers, outdoor gross motor play, transitioning from inside to outside play, handwashing, diapering and napping. Infants were having tummy time or napping. Children were served chicken /veggie strips, pineapples, mixed veggies, bread, and milk. Teachers were actively engaged indoors and during outdoor time. Many classrooms the children were creating Valentine crafts. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. Floor board is peeling, missing bumper guard. 15A NCAC 18A .2825(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. Facility was a month behind on one drill. .0604(u);.0302(d)(8) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. Suspected allergies did not have a plan for if the child comes in contact with allergen. .0801(b) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of childcare requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: Required Response: You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. The violations documented today must be corrected immediately. To verify correction of the above violations you shall submit a signed letter to me explaining how you have corrected the violations and how you will prevent these violations in the future. The letter should list each item using the item number located to the left of the violation descriptions. When submitting an email addressing corrections the email must be sent from the email address of the person designated to submit corrections (example: director) and must include the person’s name and title at the end of the email or letter. It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. You shall correct the violation(s) immediately. Send two (2) signed corrections letters or email to the address below by February 27, 2026. Rated License: We discussed the Reapplication of you Rated License (R.L.), you stated that you have been working with Child Care Resource and Referral to prepare. In February 2025 we discussed reapplying, however it was decided to wait so teachers can have time to prepare. Ms. Brendle expressed, and I discussed the environmental rating scales and the new Star system that allows facilities to use accreditation as an option. It was stated that your facility will start working on your National Association of the education of young children, (NAEYC), and when completed you will use your accreditation to maintain your five stars. NAEYC is currently being processed with a delay on family engagement verification. *Hold Harmless has ended. There is now three different pathways to obtain and maintain your Star Rated License. DCDEE has webinars and meetings to update providers of the new requirements on our website. The options are: Three Pathways to earn a 2-5 Star Rated License: Pathway #1 – Program Assessment using the Environment Rating Scales (ITERS-3, ECERS-3, FCCERS-3 and SACERS-U) Pathway #2 – Classroom & Instructional Quality focusing on curriculum and child assessment for demonstration of quality practices Pathway #3 – Accreditation & Head Start which allows a direct licensure pathway for accredited and Head Start facilities *Contact CCRI for assistance with preparing for the environmental ratings, if you change your mind. I was greeted at the door by S. Brendle, I stated the purpose of my visit and greeted Ms. S. Newsome, assistant administrator. Ms. S. Brendle assisted me with the walk-through of the facility. Children were engaged in nap time and nap time transitions to snack time. Diapering and toileting routines were observed as well. Several children were engaged in quite play on their cots. Children were served. raspberry, saltine crackers, and water. The following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. There was only one new staff, who started on September 16, 2024. Violation Number Comment Rule 614 Bed, cribs, playpens, cots or mats were not placed at least 18" apart or separated by partitions when in use. there were two cots that measured under 16" between the cots in space 7. 15A NCAC 18A .2821(e) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. in space 5 a teacher had a can of dr. pepper on the countertop that she was consuming. .0901(i) Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. MEYER Operation Type: Center Case Number: Visit Date: 6/17/2024 Number Present: 70 Completed Date: 6/17/2024 Age: From 0 To 5 Total Minutes: 105 Time In: 10:15 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an unannounced routine visit. Upon arrival I was greeted by Ms. S. Brendle. I shared the reason for the visit and she assisted me with today’s visit. Your program currently operates with a Five (5) star license. The license was posted, with restrictions to first (1st) shift, meets enhanced ratios, and meets enhanced space. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six (96%) percent as of today’s visit. Ownership: The facility owner is current and active. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The Sanitation and Fire Inspections remain the same since the last visit. The following items above were found in/out of compliance. The following inspections are below. •Last fire drill was conducted on 5/31/24. •Last Shelter in Place/Lockdown Drill was conducted 5/8/24. •Last Playground Inspection was conducted on 5/14/24. •The incident report(s) and log were in compliance. The children were observed in the room that serves infants participating in teacher directed self-soothing care such as feeding and napping. The children in the room that serves toddlers were separated in to smaller groups participating in teacher directed art projects and outdoor learning activities. The children in the room that serves younger and older pre-school children were participating in child directed free choice activities. Outdoor Learning Environment: The outdoor learning environment was divided into age-appropriate areas. The children in the area that serves toddler and infant aged children were participating in water play. The other children in the larger playground were participating in child directed free play. All three outdoor learning environments were all in compliance. Staff Records: The staff-training worksheets were completed before the visit. There was two (2) new files since the last visit to monitor. They both were in compliance. As a reminder before any new staff members begin, have a physical copy of their compliance letter in their file in addition to all other paperwork that is required on or before firt day of employment. Medication: Medication was monitored and all was well organized, all in compliance. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. One (1) violation were observed today and was corrected during the visit. The violations are as follows: Violation Number Comment Rule 1828 Jump ropes and rubber bands were accessible to children under five years of age without adult supervision. In the outdoor learning environment that serves older preschoolers there was a storage cabinet that was unlocked and jump ropes were accessible. .0604(q) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Rated License •Reach out to your Child Care Consultant for the facility’s star rated cohort assessment and preparation year. Please make the Child Care Consultant aware of when and if you are ready to begin the Star Rated License process within the cohort timeline. Technical Assistance •On May 13, 2024, DCDEE emailed the following information included in the Raise NC Newsletter Blast regarding a 2024 Legislative Session Update: Last week the Senate Health Committee unanimously passed and sent Senate Bill 876 QRIS Modernization to the Senate Appropriations Committee. The legislation authorizes the Child Care Commission to adopt rules to update and revise the Quality Rating Improvement System to include alternative pathways for licensed child care facilities to earn or maintain star ratings and extends the hold harmless provisions until the updated system is implemented. •The Senate also assigned Senate Bill 896 Investing in NC Act to the Senate Rules Committee. The legislation includes proposed funding to support expansion of the WAGE$ program statewide. •Reach out to your local CCRI and Partnership for Children for assistance with the Rated License process. •Make sure all cabinets in classrooms lock properly to minimize the chance of children opening and closing the cabinets. •Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Family child care homes are required to test water for lead once. o DCDEE encourages you to utilize the Clean Classrooms for Children’s program. You can take the pre-enrollment webinars which is available at www.cleanwaterforUSkids.org/carolina then enroll – At www.cleanwaterforUSkids.org/carolina completing the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos.
