I greeted by Ms. F. Beckham, assistant administrator. I stated the purpose of my visit and we proceeded with the monitoring visit. Children were observed participating in gross motor activities, children were also observed eating snack. Snack consisted of diced pears, and rice crispy treats. Transition from the bus and departure was monitored as well. I conducted a walk-through of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. There were no new staff files to review, however I reviewed a floaters file due to her being in the classroom alone. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The mulch around the play surface measured under three inches for some places, mainly under the slide. .0605(j) 1041 Prior to employment a Criminal Background Check was not completed. One staff was alone with the children and did not complete the process to get her out of state CBC to get her NC CBC completed. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. Staff letter was a falsification of the official qualification letter. Administrators were not aware of the validity of the letter. G.S. 110-90.2(b) & (d) & .2703(e) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. REQUIRED RESPONSE: You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address below by June 1, 2022. The format to follow is: -Name of your facility, -ID number, -date of the visit, -date you are submitting the letter, -citing each violation number, -when AND how it was corrected AND how you will prevent the reoccurrence of this violation, -Close with YOUR SIGNATURE Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. Technical Assistance: In one staff member’s file there is a DCDEE qualifying letter showing this staff member is qualified under N.C.G.S 110-90.02 and dated October, 29, 2025. This letter is valid for five (5) years, but it did not have an expiration date. Upon checking the ABCM System, there was not a record of this letter being processed by the Criminal Background Unit of DCDEE and there was no record of an CBC account. During today’s visit, it was confirmed by the CBC department that the letter was invalid and the account was closed due to the staff not following the required steps to have an out of state CBC completed. •As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. CBC Provider Portal: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. The facility operated with a four-star license, issued June 15, 2022. Upon arrival I observed a teacher standing outside of the building with several children. I greeted them and walked to the door and Ms. B. opened the door for me. We greeted each other and I stated the purpose of my visit. Mrs. Courtney Dukes, the administrator, was in her office and we greeted other. I was assisted with the monitoring visit by both Ms. B and Mrs. Courtney. Mrs. Dukes, owner, arrived later near the end of the visit. Children were observed actively engaged in center play, outside gross motor play. And the older school aged children were doing homework, reading, or having quite time. Children were served a snack that consisted of apple sauce, French toast, cheese crackers, and water. Departure was also observed. The following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu posted was applesauce, french toast, and water. Many children had cheese crackers and water. 10A NCAC 09 .0901(b) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Space three had a missing cover by the shelf by the entrance near the hallway. 10A NCAC 09 .0604(c) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. During the visit, we spoke with Mr. Morrison. Limited monitoring of child care requirements occurred during today’s visit. This facility currently operates with a five (5) star rated license effective June 15, 2022. Children were observed were looking at books and engaged in center activity play. Children were served cookies, ritz cheese crackers, and water. INSPECTIONS: The last sanitation inspection was conducted 10-17-2023 The last fire inspection was conducted on 1/11/24. The last documented fire drill was conducted 2-07-2024. The last documented shelter-in-place or lockdown drill was conducted 12-11-2023. The last documented playground inspection was conducted 1/7/ 2024. I monitored the following items today: Supervision, Staff/child ratio, health, Safety, program and management. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Consultant did not receive the required fire or sanitation inspection, one week after the inspection was conducted. 10A NCAC 09 .0304(a) 808 The outdoor premises were not clean, drained and free of litter and hazardous materials grass and other vegetation in a manner which does not encourage vermin. Mrs. Dukes was not present at arrival, however, I spoke with her via telephone discussing environmental rating scales and Cohorts. Mrs. Dukes arrived later near the end of the visit. This facility is in the second cohort and does not need to reapply for rated license assessment until 2025. If you are interested, you can apply prior to your cohort. Children were observed actively engaged in center play, another class was engaged in screen time, and space four were reading books at the table and one child was engaged in screen time. Snack consisted of pears, cereal, and milk. The following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure.
