Sugey Alcivar, the owner, assisted me with the visit. The facility was issued a temporary license on July 28, 2025, with a restriction of Daytime care only, Maximum of 5 preschool children at any time, children in care on ground level only, Fireplace/woodstove not used during operating hours. Kaye Dunlap Child Care Consultant accompanied me on today’s visit. Ms. Alcivar currently has six (6) children enrolled. When we walked into the FCCH there was a space heater in the corner of the room, sitting on a shelf that was mounted into the wall. The shelf is 5’5” from the ground and out of reach of the children. The heater was being held secured by a hard plastic thick cable. The heater was plugged into a surge protector, and the manufacturers stated that the plug needed to plug into the outlet directly. The heater was not hot to the touch, but the plastic cable that securing the space heater was hot to the touch. During this visit, I observed the indoor and outdoor environment, and completed a full assessment of the licensed space. Five (5) children aged 0-2 were present. Children were observed eating snacks and playing freely. Two (2) infants were sleeping. Safe Sleep Charts were not completed for today. Ms. Alcivar was very nurturing with the children. The child care room has several appropriate age materials for the children. In the bathroom paint was stored in the closet, and the closet was not locked. In the kitchen area two (2) aerosol cans were sitting on a shelf that was over five feet. One (1) new child had enrolled in the programs since my last visit. The file was reviewed and found in compliance. Three (3) violations were observed and discussed with the provider during today’s visit. Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. there was a space heater in the corner of the room, sitting on a shelf that was mounted into the wall. All violations cited today must be corrected immediately. Ms. Alcivar will submit a letter of compliance explaining how and when today’s violations were corrected to Amy Italiano, Lead Child Care Consultant on or before January 20, 2026. Failure to correct the violations and/or submit the compliance letter to me by the date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Ms. Alcivar is working with Quality Every day. Ms. Alcivar is going to continue to work with Quality Every day and once she is ready, she will reach out to her consultant Kaye Dunlap, to request the scales to be completed. Ms. Alcivar stated she has no children enrolled who receive subsidized children care. She understands the facility must hold a 3-5 Star Rated License at the end of the temporary time period to continue providing child care for sponsored children. - We discussed that when children under 12 months are sleeping that the provider needs to check on the infant at least every 15 minutes and document that the infant was visually checked. I recommend that you set a timer to ensure that you are checking on the infant and documenting the safe sleep chart. - Ms. Alcivar has been working on her lesson plan and is making good progress. Today I emailed you a sample of a FCCH lesson plan to have another example. - You were reminded today that all hazardous products and aerosol cans must be kept under lock and key. Locked means combo lock, lock and key or magnetic locks. - Ms. Alcivar has two rooms that infants sleep in, however she only has one posted policy, I gave her the phone number to the recourse center to order another poster. 1-800-367-2229. - You must maintain at least a 75% or higher Compliance History to be eligible to apply for a child care license. Your Compliance History will be built over an 18-month period. The compliance history is based on violations that are cited during visits to your facility. If you have an acceptable compliance history at the end of your six-month temporary license you will be eligible to apply for a star rated license. You must maintain at least a 75% or higher compliance history to maintain your star license. If your facility's compliance history drops below 75% you may receive an administrative action. - Health and Safety Training must be completed within one year of licensure and every five years thereafter. You can access these training courses for free on DCDEE’s website www.ncchildcare.ncdhhs.gov by going to Provider, Professional Training, and DCDEE Moodle. Ms. Alcivar needs to complete her Recognizing and Responding to child Mal treatment before October 28, 2025. - New facilities have one year from licensure to complete EPR training, and 4 months to complete the EPR plan once the training is completed. Your EPR plan needs to be completed no later than July 8, 2026. Once the rated license is issued, the working file will be passed to Kaye Dunlap, the child care consultant assigned to this zip code. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Pathway 1 Program Assessment This pathway is the most familiar to the previous system and uses a North Carolina Rated License assessment tool to measure the overall quality of the program. This assessment will focus on standards for both the learning environment and the education qualifications of staff. Pathway 2 Classroom and Instructional Quality This pathway will emphasize a program use of specific high-quality curriculum and instructional strategies. Quality is demonstrated by a program curriculum choice which must be based on the child's developmental needs. This pathway can also include coaching and training for administrators to ensure the effective amplification of the current curriculum. Pathway 3 Accreditation and Head Start This option allows programs with specific national accreditations or head start status to automatically qualify for a three- or five-star rating. During today’s visit I was told that the facility had been accredited during previous ownership. Administrative, Operational and Personnel Policies – Ms. Alcivar is working on her operational polices, Quality Every day is working with Ms. Alcivar. Ms. Alcivar stated she has no children enrolled who receive subsidized children care. She understands the facility must hold a 3-5 Star Rated License at the end of the temporary time period to continue providing child care for sponsored children. - It was discussed today that you need to do your fire inspection and playground inspections monthly. I encouraged Ms. Alcivar to set an alarm on her calendar to ensure she if fulling this requirement. - We discussed that when children under 12 months are sleeping that the provider needs to check on the infant at least every 15 minutes and document that the infant was visually checked. - It was discussed that activity plans need to be current and implemented into your daily activities. - Ms. Alcivar and I discussed scheduling a TA visit to help her get her paperwork organized. - You must maintain at least a 75% or higher Compliance History to be eligible to apply for a child care license.
