Upon arrival I was greeted by Ms. Nekita Morrison. Today’s visit was conducted with you, Ms. Morrison, administrator. The Family Child Care operates with a four-star license issued March 10, 2020. Restriction: Max 5 preschool children at any time, fireplace/wood stove not used during operating hour; children in care on ground level only; 1st shift (Daytime care); 2nd shift, serves NMT 2 infants under age 1. The age range is 0-12 years of age, and the capacity is 8 on 1st shift and 5 on 2nd shift. Nekita Morrison is the owner/operator. Remember if any changes to the ownership need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. In Mecklenburg County the FCCH operator must live in the Family Child Care Home and be the primary caregiver. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. I verified there have been no changes to the facility telephone number or email address in the regulatory system. Prior to the visit, this facility experienced damage from Hurricane Helene. Observvations: Children were observed actively playing with dramatic play, block center, transitioning to morning snack and eating. Morning snack consisted of pop tarts (toasted pastries with strawberry fillings),milk; and fresh oranges and apples. The attached checklist was used to note the requirements we monitored today. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility currently does not provide transportation. The Administrator reported that the facility is in compliance with Child Care Requirements regarding firearms and weapons. INSPECTIONS: The last documented fire drill was conducted on 10-10-2024. The last documented shelter-in-place or lockdown drill was conducted 9-07-24. The last documented playground inspection was conducted 10-10-24. The last documented compliance was at 94%. Rated License: Ms. Morrison has decided that she wants to continue to apply for rated license reassessment at this time. I received her application prior to 2023 and it was determined that she would decrease in stars due to education not being in Works. An updated self-study was received today’s visit. Ms. Morrison was advised to submit her transcripts to WORKS in order to have all of her education in the system before processing starts. Based on Ms. Morrison’s education and completion of the self-study and has earned one quality point, Blessed Beginnings will remain a Four Star facility, if her education is reviewed by WORKS. Capacity, facility changes: We discussed transportation and being re-approved. She is currently seeking a new vehicle/van to assist with transporting children. Ms. Morrison would like for me to review and approve her vehicle for transportation at the next visits, if it is purchased. It was also discussed that she is interested in increasing her capacity to ten children. While discussing it was determined that she already has the required exits to become R2/3 and if she wanted to be a center in a residence, she needs to have an handicap accessible exit. It was stated that she is interested and that if she could use her back door as an interest would be ready to convert to Center in a residence. If you are ready prior to the consultants’ next visit you may contact me, and I will provide a courtesy visit to approve capacity or next steps. Note if you are interested in becoming a center in a residence you may have to go thru the temporary license process again. Violation Number Comment Rule 711 Outdoor stationary play equipment was not anchored and/or equipment was installed over concrete or asphalt. Equipment was not anchored and posed a potential hazard due to the proximity to the fence. 10 A NCAC .1719(a)(22) 1544 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The fenced measured under 48 inches in several spaces. GS 110-91 1927 Openings in equipment, steps, decks, and handrails were not smaller than 3 1/2 inches or greater than 9 inches to prevent entrapment. Several sections of the fence where the sides met measured 3.5 inches up to 6.0 inches. .1719(a)(23) 1931 Telephone numbers for the fire department, law enforcement office, emergency medical service, and poison control were not posted in a location visible in the home. This document could not be located as being posted. .1719(a )(12) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. REQUIRED RESPONSE: (for any violations) You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address below by November 05, 2023. The format to follow is: -Name of your facility, -ID number, -date of the visit, -date you are submitting the letter, -citing each violation number, -when AND how it was corrected AND how you will prevent the reoccurrence of this violation, -Close with YOUR SIGNATURE REQUIRED RESPONSE: If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a proposed timeline of the corrections. In some cases, the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Technical Assistance: Water testing: You stated that you have received your water test kit. To receive this test for free and to be in compliance, you must have this done prior to January 24, 2024. Stars/education: Remember to update and submit your transcript into Works , contact me as soon as you are done. Outdoor space: Because you are interested in becoming a CIAR., Review the childcare rules before placing your swings on the playground, make sure you properly mount the footing in the ground and measure to prevent tipping and hazards. Based on Ms. Morrison’s education and completion of the self-study and has earned one quality point, Blessed Beginnings will remain a Four Star facility. We discussed transportation and being reapproved. Ms. Morrison would like for me to review and approve her vehicle for transportation at the next visits. The last documented shelter-in-place or lockdown drill was conducted 9-14-23. The last documented playground inspection was conducted 10-11-23. The last documented compliance was at 98%. The Administrator reported that the facility is in compliance with Child Care Requirements regarding firearms and weapons. Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. Children could potentially have access to a lawn mower, propane tank, a power tool (for cutting, and a leaf blower). These items were all located when you walk outside to go to the playground. .1719 (a) (1) 1804 Safe sleep policy and poster about safe sleep practices was not posted in a prominent place in the infant sleeping room or area where parents and caregivers are able to view daily. Provider did not have any evidence of the required document posted in the area that the infant slept. .1724(b) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. Documentation was not kept on file, provider stated that she gave the documentation to the infant's parent. .1724(a)(8)&(f) 9999 All other rules must be followed even if there is not a specific violation number listed on this form. Provider has an infant and did not have a dirty toy bucket for mouthed toys.
