The November 2024 version of Child Care Center Master Item Number Listing, DCD-0357 was used to complete monitoring today. I was let into the facility by the administrator Tammy Drew. She accompanied me on the walk through. Program records were made available to me. Supervision, Staff Child Ratio, First Aid, Special Training, Storage of Hazardous Substances and Medication, Approved Space, Program Records, License Posted, and Permit Restrictions were monitored today. The bathrooms used by children, and thermometers in the refrigerator's were monitored today. The following violations were cited during today's visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Electrical outlets in space three (3) and eight (8) did not have safety outlets in them. 10A NCAC 09 .0604(c) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. Within the first two weeks of employment, one (1) employee's six clock hours of orientation was not documented. .1101(a)(b) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. Starbucks cups were located on a shelf in space two (2), and space three (3). .0901(i) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One (1) staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) Compliance -Violations cited today must be corrected immediately. Please send me a letter documenting how violations cited today were corrected by March 12, 2025. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Technical Assistance -Ditto sheets and teacher directed art was observed in space one (1). For the age group present, I recommend more "free art", for example have paper and art supplies available to create freely. I also recommend putting some of the child created artwork on child eye level. -Remind staff to lift children around the torso and under the arms to prevent injury to the children's joints. -Today we discussed the ABCMS. You stated that you have taken the training but have no access to it yet. You stated that you assume it's because you hadn't used it in over 90 days and have the contact number and email to get it reset. You're also aware that going forward you will have to have a fourteen (14) character password. Please keep me updated on your progress with this. -Please remind staff that music should be played for a purpose such as movement times, transitional cues, singing, or quiet music for rest time. - You stated that a college student substitute may be coming back for the summer. Please ensure that all of their required trainings are up to date before they return. V. Cunninham, C. Neeley, A. Pfeiffer, L. Escobar, and Z. McKoy's CPR and First Aid are due for renewal on 10/10/25. V. Cunningham's SIDS expires 10/6/25. -We discussed QRIS Modernization and the Accreditation pathways. The last Annual Compliance visit was made on 08/30/24. The current 1 star license was issued on 5/20/2020. The March 2024 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 94% as of today. I was let into the facility by teacher Ms. Chardon. I told her the reason for my visit. She alerted the director Tammy Drew that I was here. Ms. Drew accompanied me on the walk through of the facility. Facility files were made accessible to me. Ten(10) classrooms were monitored on today for: Sign in/out, allergy list, medical action plans (if applicable), menu, schedule, activity plan, and staff/child ratio chart. Storage of medication and permission to administer was monitored. Supervision, and storage of hazardous products was monitored. Children were observed having indoor and outdoor free play, teacher directed activities, and having routine needs met. There were ample toys and activities available. The following was observed posted: current license, Summary of Law, Safe Arrival and Departure procedures, Menu, Emergency Care Plan, and no smoking signage. A Sanitation inspection was conducted on 06/05/2024 . A Fire inspection was conducted on 10/5/2023. Monthly Fire Drills, Playground Inspections, and EPR Drills were monitored. A sample of staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. A sample of children's files were monitored for Application, Emergency Care authorization, Summary of Law, Medical/Immunization, Discipline Statement, No smoking statement, Safe Sleep Policy, Infant Feeding Schedule and Shaken Baby Syndrome Policy (if applicable). This facility does not provide transportation. The following violations were cited during today's visit: Violation Number Comment Rule 532 All children were not held or placed in feeding chairs or other appropriate apparatus to be fed. One (1) child was lying on the floor drinking a bottle. 10A NCAC 09 .0902(b) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Diaper Cream was present in space 1 for one child without written authorization. 10A NCAC 09 .0803(1)(a & b) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care.
