Name of Operation: BEVERLY WOODS ASEP Facility ID: 6055643 Consultant: DENISE WATSON Operation Type: Center Case Number: Visit Date: 1/12/2026 Number Present: 21 Completed Date: 1/12/2026 Age: From 5 To 11 Total Minutes: 95 Time In: 01:40 PM Time Out: 03:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during an annual compliance visit. The last annual compliance visit was made on 1/16/2025. The current 5 star rated license was issued 4/17/2024. The April 2025 version of the Child Care Center Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 99% as of today. I was let into the school by the front office staff. Program coordinator Ms. Cathey was not present today. Group Leader Ms. Green was present. She stated that a substitute was arriving shortly. Program records, staff records, and children's records were made accessible to me. The following was observed posted: Emergency Care Plan, Safe Drop off/Pick up, Current License, Summary of Law, Menu, Activity Plan, and Schedule. Attendance and Sign In/Out was monitored. An Offsite Staff Verification form, Off site transportation form, and Offsite Children's Verification form was monitored. The cafeteria and bathroom area used by the children was monitored. Attendance/Sign in and out was monitored. Children arrived at the cafeteria as school let out. They used the restroom and washed hands. Both groups went to outdoor play. After outdoor play they will come back in, have snack, center time, and homework time. A substitute Ms. N. Perry arrived during the visit. The following violations were cited: Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. A file was not available for review for the substitute staff present. The substitute staff present was not on the staff and training worksheet or off site records documentation form. G.S. 110-91( 9) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. A photograph was not present for every child being transported for field trips. 10A NCAC 09 .1003(d) Compliance -Violations cited today must be corrected immediately. Please send me a letter documenting how violations cited today were corrected by January 26, 2026. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. The March 2024 Child Care Center Item number listing was use to monitor this facility. I was let into the school by the front office staff. Upon my arrival I stated to them the reason for my visit. I walked to the cafeteria and ASEP office. I was greeted by group leader Ms. Green. She stated that site coordinator Ms. Cathey was not present today. Files for this facility were made available to me. Substitute site coordinator Ms. Kenny-Boddie arrived shortly during the visit. The following was observed posted: Current license, Summary of Law, and Safe Arrival and Departure procedures, Staff Child ratio chart, and Medical Care Plan. The current schedule, menu, and lesson plans were observed posted. Monthly Playground Inspections and Fire Drills were monitored. Emergency Preparedness Drills were monitored. Three (3) staff files were monitored for Criminal Record Checks, Emergency Info/Health Questionnaire, First Aid/CPR training, and BSAC training. An Offsite Staff Record Verification form was observed. The cafeteria, gym, and bathroom, area used by the children was monitored. Children arrived at the cafeteria as school let out. Attendance was taken, then they went to the restroom and outside for free play. Attendance/Sign in and out was monitored. The following violations were cited during today's visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference.
