The last Annual Compliance visit was made on 8/27/2024. The 4 Star Rated License was issued on 11/08/18. I was let into the facility by operator Arlesha Kendrick. She was present with five (5) children. One (1) family member was visiting during today's visit. One (1) household member was present. The April 2025 version of the Family Child Care Home Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 96% as of today. Children were observed having free play. There were toys and activities available. Attendance was monitored. Summary of law, schedule, summary of law, first aid sheet, and emergency numbers were posted. Monthly Fire Drills, Emergency Preparedness Drills, and Playground inspections were monitored. Children's files were monitored for Application, Emergency Care Authorization, Medical/Immunization, Discipline Statement, Summary of Law, No smoking statement, Shaken Baby Syndrome policy, and Outside fence permission. The provider's file was monitored for Criminal Qualifying Letters, First Aid and CPR coverage, Health and Safety Trainings, Shaken Baby Syndrome Policy, Special Trainings, On-going Training hours, and Annual Health Questionnaire. This facility does not provide transportation. There are no children enrolled with allergies or medication present. The outdoor, bathroom and kitchen areas were monitored. The November 2024 Master Family Child Care Item number was used to complete today's visit. Provider Arlesha Kendrick was present with three (3) children. One (1) household member was present during today's visit. The childcare room, kitchen, bathroom, and outdoor play area was monitored. The following was observed posted: Current license, Summary of Law, and Emergency Care Plan. Monthly playground inspections and fire drills were monitored. Emergency Preparedness Drills were monitored. One (1) staff file was monitored for Criminal Record Checks, Emergency Info/Health Questionnaire, First Aid/CPR training, and special trainings. The following violations were cited during today's visit: Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. The provider's dog rabies vaccination expired 2/2/25. .1719(b)(1) 908 Health questionnaire was not completed annually. Health questionnaire was not completed by 1/15/25. .1703(a)(1) Compliance -Violations cited today must be corrected immediately. Please use Only the forms provided by DCDEE found under provider documents on the website. 10A NCAC 09 .1721 REQUIREMENTS FOR RECORDS (e) The family child care home operator shall complete and maintain other records that include: (1) documentation of the operator's Emergency Preparedness and Response Plan on a template provided by the Division of Emergency Management at http://rmp.nc.gov/portal/#; (2) documentation that monthly fire drills are practiced. The documentation shall include the date each drill is held, the time of day, the length of time taken to evacuate the home, and the operator's signature; -Be careful about playing music during free play or other group activities without an intended purpose. Sometimes when music is played in the background as a filler, it can take away the opportunity for meaningful conversations and interactions to take place. Try limiting times when music is played to music/movement time, as a transitional prompt or quiet music during rest periods. -Please be aware that N. Kendrick's Criminal Background Check expires 7/13/25. "Cookie cutter" print outs were used today for children to color. It's best practice and developmentally and age appropriate to use them sparingly. I suggest having plain paper and drawing materials for them to use freely or placing the print outs in the play room as a free choice activity for children to use if they want. Materials for play were observed in the playroom and in the closet. I suggest introducing more materials to the children gradually and using shelves, and bins to store the materials to encourage cleaning up and autonomy. A checklist was used to monitor compliance. Prior to today's visit; your compliance history was 94%. Upon my arrival the operator was present with four children ranging in age of 1-2 years of age. The fifth child arrived during the visit. The child was greeted upon arrival and joined the other children as they played with material in the child care space of the home. The kitchen and stairs to the upper level were made inaccessible to children in care. Material and equipment was observed in good repair and developmentally appropriate of the age served. A current activity for the months of August and September were posted. The outdoor area was observed and found meeting compliance. Supervision and staff child ratio were found meeting compliance. I reviewed program records. All information required to be posted were observed posted as you enter the home. The emergency drill log was reviewed and found meeting compliance. I reviewed monthly playground inspections and were found meeting compliance. I reviewed the children's records; one child's medical action plan will need to be updated by 9/7/23. I reviewed your information, you and your two family members living in the home have current DCDEE qualifying letters on file. You have current CPR/FA certification and current ITS/SIDS training. You had 4 hours of training hours to carry forward, you completed 9 for the 2022-2023 annual year, and have 4 hours to carry forward. You do not offer transportation. The following violations were cited during today's visit. Violation Number Comment Rule 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. One Auvi-Q for a child enrolled does not have written permission to administer. 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) Compliance Statement: Childcare providers are expected to maintain compliance with all applicable child care requirements at all times. All violations cited today shall be corrected immediately. Arlesha Kendrick, Operator, will email a compliance letter explaining how each violation cited today has been corrected. Once you have reviewed or updated it you must Publish it (which will have the cover page with the current date it was published), then print and place the current plan in your Ready To Go File. You need to update it annually so first responders know you are still in operation and have current information. Nutrition – We discussed requirements for Special Diets and the Opt-Out option. 10A NCAC 09 .1706(c) A child's parent may opt out of the supplemental food provided by the operator as set forth in G.S. 110-91(2) h.1. When a child's parent opts out of the supplemental food provided by the family child care home, the operator shall obtain the parents signature acknowledging the parental decision and shall maintain the acknowledgment in the child's file at the home and provide a copy to the parent. A child's parent may opt out of the supplemental food provided by the family child care home, subject to the following: (1) the operator shall not provide any food or drink so long as the child's parent or guardian provides all meals, snacks, and drinks scheduled to be served at the program's designated times; (2) the opt out ability is not available for specific meals or days based on menu options; (3) if a child requests specific foods being served to other children, but the parent has opted out, the operator shall not serve supplemental food; and (4) if the child's parent has opted out, but does not provide all meals and snacks for the child, the operator shall replace the missing meal or snack as if the child's parent or guardian had not opted out of the supplemental food program. (d) The food required by special diets for medical, religious or cultural reasons, or parental preferences, may be provided by the operator or may be brought to the program by the parents. If the diet is prescribed by a health care professional, a statement signed by the health care professional shall be on file at the program and written instructions must be provided by the child's parent, health care professional or a licensed dietitian/nutritionist. If the diet is not prescribed by a health care professional, written instructions shall be provided by the child's parent and shall be on file at the program. Medication: I reminded you that the Medical Action plan you have on file for the child with Auvi-Q on site, needs to be updated by 9/7/23. We also discussed that whenever you have medication (creams/lotions, medication for medical conditions) are on premise for you to administer you must have current written permission to administer. Creams and Lotion medication permission is valid for on year. Medication for medical conditions is valid for 6 months. I have provided medication forms for you to have for future use. These documents can be found on the DCDEE website under Provider Documents, click on the Family Child Care Home Documents. Rated License Reassessment: The current 4 Star rated license was issued 11-8-18. The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024. Reassessment for your facility will be during Cohort 1. Cohort 1: Planning year runs from July 1, 2023 to June 30, 2024. Reassessment year runs from July 1, 2024 to June 30, 2025. If you are interested in the Family Child Care Home Environmental Rating scale (FCCERS-R), you can request one during the planning year as a practice run, or if you score above a 5.0 it can be used during the reassessment year. If you score below a 5.0 during the planning year, you can decide if you want to complete the assessment during the reassessment year at no charge and that score will be used to determine the program points of the rated license to earn more than 2 points in Program Standards. NCRLAP has resources and training available on their website NCRLAP.org. In order to maintain 2 points in program standards of the rated license you will be required to complete the FCCERS-R self-study over a 3-month time period prior to your rated license reassessment. 10A NCAC 09 .2823(b)Operator provides documentation of self-study and self-assessment using the Family Child Care Rating Scale over a 3-month time period. Documentation such as an ERS book with score sheets, self-assessments and program adjustments shall be reviewed by the Division as part of the rated license assessment. A checklist was used to monitor compliance. Prior to today's visit; your compliance history was 94%. Upon my arrival the operator was present with four children ranging in age of 1-2 years of age. The fifth child arrived during the visit. The child was greeted upon arrival and joined the other children as they played with material in the child care space of the home. The kitchen and stairs to the upper level were made inaccessible to children in care. Material and equipment was observed in good repair and developmentally appropriate of the age served. A current activity for the months of August and September were posted. The outdoor area was observed and found meeting compliance. Supervision and staff child ratio were found meeting compliance. I reviewed program records. All information required to be posted were observed posted as you enter the home. The emergency drill log was reviewed and found meeting compliance. I reviewed monthly playground inspections and were found meeting compliance. I reviewed the children's records; one child's medical action plan will need to be updated by 9/7/23. I reviewed your information, you and your two family members living in the home have current DCDEE qualifying letters on file. You have current CPR/FA certification and current ITS/SIDS training. You had 4 hours of training hours to carry forward, you completed 9 for the 2022-2023 annual year, and have 4 hours to carry forward. You do not offer transportation. The following violations were cited during today's visit. Violation Number Comment Rule 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. One Auvi-Q for a child enrolled does not have written permission to administer. 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) Compliance Statement: Childcare providers are expected to maintain compliance with all applicable child care requirements at all times. All violations cited today shall be corrected immediately. Arlesha Kendrick, Operator, will email a compliance letter explaining how each violation cited today has been corrected. Technical Assistance was provided on the following: EPR Plan: Annually, and anytime within the year anything changes to your EPR plan, you need to go into the NC Risk Management portal; sign in with your NCID username and password. Review your EPR plan and update it if needed. Once you have reviewed or updated it you must Publish it (which will have the cover page with the current date it was published), then print and place the current plan in your Ready To Go File. You need to update it annually so first responders know you are still in operation and have current information. Nutrition – We discussed requirements for Special Diets and the Opt-Out option. 10A NCAC 09 .1706(c) A child's parent may opt out of the supplemental food provided by the operator as set forth in G.S. 110-91(2) h.1. When a child's parent opts out of the supplemental food provided by the family child care home, the operator shall obtain the parents signature acknowledging the parental decision and shall maintain the acknowledgment in the child's file at the home and provide a copy to the parent. A child's parent may opt out of the supplemental food provided by the family child care home, subject to the following: (1) the operator shall not provide any food or drink so long as the child's parent or guardian provides all meals, snacks, and drinks scheduled to be served at the program's designated times; (2) the opt out ability is not available for specific meals or days based on menu options; (3) if a child requests specific foods being served to other children, but the parent has opted out, the operator shall not serve supplemental food; and (4) if the child's parent has opted out, but does not provide all meals and snacks for the child, the operator shall replace the missing meal or snack as if the child's parent or guardian had not opted out of the supplemental food program. (d) The food required by special diets for medical, religious or cultural reasons, or parental preferences, may be provided by the operator or may be brought to the program by the parents. If the diet is prescribed by a health care professional, a statement signed by the health care professional shall be on file at the program and written instructions must be provided by the child's parent, health care professional or a licensed dietitian/nutritionist. If the diet is not prescribed by a health care professional, written instructions shall be provided by the child's parent and shall be on file at the program. Medication: I reminded you that the Medical Action plan you have on file for the child with Auvi-Q on site, needs to be updated by 9/7/23. We also discussed that whenever you have medication (creams/lotions, medication for medical conditions) are on premise for you to administer you must have current written permission to administer. Creams and Lotion medication permission is valid for on year. Medication for medical conditions is valid for 6 months. I have provided medication forms for you to have for future use. These documents can be found on the DCDEE website under Provider Documents, click on the Family Child Care Home Documents. Rated License Reassessment: The current 4 Star rated license was issued 11-8-18. The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024. Reassessment for your facility will be during Cohort 1. Cohort 1: Planning year runs from July 1, 2023 to June 30, 2024. Reassessment year runs from July 1, 2024 to June 30, 2025. If you are interested in the Family Child Care Home Environmental Rating scale (FCCERS-R), you can request one during the planning year as a practice run, or if you score above a 5.0 it can be used during the reassessment year. If you score below a 5.0 during the planning year, you can decide if you want to complete the assessment during the reassessment year at no charge and that score will be used to determine the program points of the rated license to earn more than 2 points in Program Standards. NCRLAP has resources and training available on their website NCRLAP.org. In order to maintain 2 points in program standards of the rated license you will be required to complete the FCCERS-R self-study over a 3-month time period prior to your rated license reassessment. 10A NCAC 09 .2823(b)Operator provides documentation of self-study and self-assessment using the Family Child Care Rating Scale over a 3-month time period. Documentation such as an ERS book with score sheets, self-assessments and program adjustments shall be reviewed by the Division as part of the rated license assessment. You may request reassessment at any time, if it’s not requested by the assigned reassessment year, I will complete an Annual Compliance Rated License Assessment visit during the 2024-2025 annual year and complete the reassessment at that time.
