The last Annual Compliance visit was made on 4/1/2025. The 3 Star Rated License was issued on 7/6/18. I was let into the facility by operator Crystal Anthony. No children were present during this visit. The April 2025 version of the Family Child Care Home Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 97% as of today. Children was observed having free play and routine needs met. There were toys and activities available. Attendance was monitored. Summary of law, schedule, summary of law, first aid sheet, and emergency numbers were posted. Monthly Fire Drills, Emergency Preparedness Drills, and Playground inspections were monitored. Children's files were monitored for Application, Emergency Care Authorization, Medical/Immunization, Discipline Statement, Summary of Law, No smoking statement, Shaken Baby Syndrome policy, and Outside fence permission. The provider's file was monitored for Criminal Qualifying Letters, First Aid and CPR coverage, Health and Safety Trainings, Shaken Baby Syndrome Policy, Special Trainings, On-going Training hours, and Annual Health Questionnaire. This facility does not provide transportation. Allergies and storage of medication was discussed. The outdoor, bathroom and kitchen areas were monitored. This facility has a compliance history 94% of as of today. The 3 star rated license was issued on July 6, 2018. Three (3) vehicles were present in the yard. I ranged the doorbell to the facility and there was no answer. I called the number listed for the facility and Crystal Anthony answered. She stated that she was on her way from the doctor and should would do the afterschool pick up and come to facility. I told her I would wait. Ms. Anthony called back and stated that her daughter was inside the house and would let me in. Ms. Anthony arrived shortly afterwards. She stated that her daughter/additional caregiver K. Anthony would bringing the children soon. Attendance was monitored. Summary of law, first aid sheet, and emergency numbers were posted. Monthly Fire Drills, Emergency Preparedness Drills, and Playground inspections were monitored. A sanitation inspection was completed in 02/21/2024. Children's files were monitored for Application, Emergency Care Authorization, Medical/Immunization, Discipline Statement, Summary of Law, No smoking statement, Shaken Baby Syndrome policy, and Outside fence permission (if applicable).
