I was greeted at the door by Cameo Womack, I stated the purpose of my visit and I conducted a walk-through of the facility. I observed the children engaged in the following activities: activity center play, transition from indoor and outdoor, outdoor gross motor play. Children played with fine motor toys as well. During this visit we discussed that you were thinking about only providing care on second shift. When you decide please send me an email of your start day. I conducted a walkthrough of the facility and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. There were no new staff. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan was dated for the week of April 10, 2026. GS 110-91(12); .0508(a) Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. This facility currently operates with a 5 star rated license effective 10-20-2017, can provide care on first and second shift, and meets all enhanced requirements as well as ratios. Upon arrival at today’s visit, I was greeted by the Owner/Administrator. We discussed the purpose of the annual compliance visit. I proceeded with the visit. Children were observed playing in engaged with both teachers in activity learning center. Children participated in art, songs, reading and many conversations. There was one infant present. The infant was observed napping and having tummy time play. Lunch which consisted of Cheese quesadilla, green beans and mixed fruit, and milk. Staff were engaged with children in conversation, feeding and holding infants. Handwashing and diaper/toileting was also monitored. During naptime teachers were observed monitoring children’s sleep and comforting them to help them relax. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last sanitation inspection was conducted: 6/2/25 The last fire inspection was conducted 6/2/25. The last documented fire drill was conducted 06/03/25. The last documented shelter-in-place or lockdown drill was conducted 04/31/25. The last documented playground inspection was conducted 05/22/25. Last compliance prior to this visit was % I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. Transportation is not provided at this facility. Staff and children’s records were reviewed today. I selected two (2) children’s records for review and one Staff. The staff and training worksheet was completed while on site. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. .0803(2)(a) 847 Parent's medication authorization did not include required information. There were no instructions for the children with 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. .0803(12) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. The last documented was for 2024. .0701(a) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. One staff only had two of the fifteen required hours. .1103(a) 1054 Documentation of staff's on-going training was not on file and/or was not current . This requirement was not current for the past year trainings. 10A NCAC 09 .1106(a) 1308 Application did not include the names of individuals to whom the center may release the child, as authorized by the person who signs the application. This facility currently operates with a 5 star rated license effective 10-20-2017, can provide care on first and second shift, and meets all enhanced requirements as well as ratios. Upon arrival at today’s visit, I was greeted by the Owner/Administrator. We discussed the purpose of the annual compliance visit. I proceeded with the visit. Children were observed playing in engaged with both teachers in activity learning center. Children participated in art, songs, reading and many conversations. There was one infant present. The infant was observed napping and having tummy time play. Lunch which consisted of Cheese quesadilla, green beans and mixed fruit, and milk. Staff were engaged with children in conversation, feeding and holding infants. Handwashing and diaper/toileting was also monitored. During naptime teachers were observed monitoring children’s sleep and comforting them to help them relax. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last sanitation inspection was conducted: 6/2/25 The last fire inspection was conducted 6/2/25. The last documented fire drill was conducted 06/03/25. The last documented shelter-in-place or lockdown drill was conducted 04/31/25. The last documented playground inspection was conducted 05/22/25. Last compliance prior to this visit was % I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. Transportation is not provided at this facility. Staff and children’s records were reviewed today. I selected two (2) children’s records for review and one Staff. The staff and training worksheet was completed while on site. The center is current and active with NC Secretary of State website. Upon arrival at today’s visit, I was greeted by the staff and then the Administrator. We discussed the purpose of the visit. I proceeded to conduct the visit. Children were observed playing in engaged with both teachers in activity learning center. Children participated in art, music and movement, and many conversations. The outdoor space was observed, and children were actively engaged with gross motor play. Children were served cereal, bananas, and milk for breakfast. Lunch consisted of meatballs alfredo, pasta, corn, and blueberries, and milk. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. I selected two(2) children’s records for review and one Staff. . I discussed the purpose of the children’s record sheet and how to complete the Staff and training worksheet. INSPECTIONS: The last sanitation inspection was conducted: 11/30/23. The last fire inspection was conducted 10/13/2023. The last documented fire drill was conducted 05/31/24. The last documented shelter-in-place or lockdown drill was conducted 04/30/24. This facility currently operates with a 5 star rated license effective 10-20-2017. Supervision, Staff/child ratio, group size, and space were observed and found in compliance today. The center is current and active with NC Secretary of State website. Upon arrival at today’s visit, I was greeted by the staff and then the Administrator. We discussed the purpose of the visit. I proceeded to conduct the visit. Children were observed playing in engaged with both teachers in activity learning center. Children participated in art, music and movement, and many conversations. The outdoor space was observed, and children were actively engaged with gross motor play. Children were served cereal, bananas, and milk for breakfast. Lunch consisted of meatballs alfredo, pasta, corn, and blueberries, and milk. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. I selected two(2) children’s records for review and one Staff. . I discussed the purpose of the children’s record sheet and how to complete the Staff and training worksheet. INSPECTIONS: The last sanitation inspection was conducted: 11/30/23. The last fire inspection was conducted 10/13/2023. The last documented fire drill was conducted 05/31/24. The last documented shelter-in-place or lockdown drill was conducted 04/30/24. The last documented playground inspection was conducted 06/23/23. Last compliance prior to this visit was 92 % I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. Transportation is not provided at this facility. Provider stated that there are no fire arms on the premises. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence), provider threw the form away due to child reaching one year old. 10A NCAC 09 .0902(a) 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. the left side of the fence on the second/third panel from the back had a space that measured over 4 inch. .0605(g) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Staff last form was dated 2020. .0701(a) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. two children did not have updated information. .0802(c) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. REQUIRED RESPONSE: You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address or email below by July 09,2023. The format to follow is: -Name of your facility, -ID number, -date of the visit, -date you are submitting the letter, -citing each violation number, -when AND how it was corrected AND how you will prevent the reoccurrence of this violation, -Close with YOUR SIGNATURE REQUIRED RESPONSE: If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a proposed timeline of the corrections. In some cases, the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. As outlined in Session Law 2021-127 - Senate Bill (SB); facilities who have been unable to hire/retain staff with the same educational levels in the current star rating are in a hold harmless state. Your facility meets these criteria and therefore will retain your current star rating until six months after Governor Cooper issues an order to rescind Executive Order No. 116 Declaration of a State of Emergency to Coordinate Response and Protective Actions to Prevent the Spread of COVID-19. The state of emergency ended August 15, 2022. Technical Assistance: Remember that emergency information for staff and children, and health questionnaires must be updated yearly as well as staff development and evaluations. It is time to start retaking your Health & Safety trainings, they are due every five years. We discussed the Cohorts and that you are in the first phase and you should start preparing for assessments. Rated License: •We discussed being reassessed and the new cohorts. You have an option not to be reassessed for environmental rating scales, if you chose this route you will decrease to four stars based on your current education. If you decide to be reassessed contact CCRI to assist with preparing you for the Family Child Care environmental rating scale assessment (FCCERS). NCRLAP.org has many resources that can prepare you for the FCCERS. During this interim time I suggested that you take the following steps: *contact your local CCRI for support. *add the outline of shapes of the blocks on the block shelf *add more diversity pictures *Label materials *Have all staff update their information in the WORKS database. The WORKS database is designed to streamline education. evaluation, using a web-based process designed to increase efficiency with which staff can process child care provider applications.
