Upon arrival I was greeted by Ms. Maria Ortega. I shared the reason for the visit. Permit Information: The program currently operates with a Four-Star center license effective 10/7/21. The license was posted, with the following restrictions: - 1st shift - Meets enhanced space I monitored the following items: supervision, staff/child ratio, licensed spaces, space capacity, health, safety, and program records. Ownership: This facility is owned by you, Ms. Maria Ortega, the individual owner, and you reported that you do live in the home and no other individuals reside in the home. Inspections: The program's last fire inspection on file with DCDEE was completed on 10/8/24. Requirements were discussed, I obtained a current fire inspection today completed for 10/3/25. The program's recent sanitation inspection on file with DCDEE was conducted on 10/23/25. The program received zero (0) demerits and received a superior classification. Indoor Learning Environment: I observed children in the indoor learning environment and found supervision and staff/child ratios in compliance. I observed children participating in nap/quiet time. Children were adequately supervised, spaced accordingly and each had their own linens. Outdoor Learning Environment: The outdoor learning environment was monitored and in compliance. Program Records: The last fire drill was completed on 12/2/25 and the last emergency drill was completed on 11/7/25. Staff Records: There were no new staff files to review. You are required to complete eight (8) ongoing training hours each year. There were no training hours that carried over from the previous year. Please complete the eight (8) hours on or before 3/10/26. Nutrition: The facility was in compliance with Child Care Meal Patterns Requirements. Medication: Medication is not administered at this facility. Transportation: The provider does not provide transportation. Weapons: It was reported that your facility was in compliance with child care requirements regarding firearms. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Upon arrival I was greeted by Ms. Maria Ortega. I shared the reason for the visit, and you were able to assist me. Permit Information: The program currently operates with a Four-Star center license effective 10/7/21. The license was posted, with the following restrictions: -1st shift - meets enhanced space I monitored the following items: supervision, staff/child ratio, licensed spaces, space capacity, staff and children’s records, health, safety, and program records. I observed children participating in large group time where a book and gross motor movement activities took place. Children were also observed in the outdoor learning environment. The kitchen is separated by a plastic/wooden gate with simple lock mechanisms. I observed an aerosol spray can on the kitchen floor, we re-reviewed requirements to lock all aerosol containers. Outdoor Learning Environment: The outdoor learning environment was monitored and in compliance. Program Records: The last fire drill was completed on 3/3/25 and the last emergency drill was completed on 2/25/25. The EPR plan was reported to have been last updated in May 2024, I observed the ERP plan with a draft watermark and date of 10/28/17. I reviewed to keep this updated annually and to track it by logging into the Risk Management Portal to update your EPR plan including your annual enrollment numbers. I observed infants and toddlers being selected as enrolled and this is no longer current. Information with links and requirements will be emailed to you. Please complete this on or before 5/1/2025. Staff Records: There were no new staff files to review. I reviewed and verified the operator’s verification form. The statements of responsibility were completed during the visit. You are required to complete eight (8) ongoing training hours each year. There were two (2) training hours that carried over from the previous year. The remaining six (6) hours were recorded with certificates attached. Children’s Records: A selection of children’s files was reviewed. Please refer to the children’s records worksheet. Nutrition: The facility was in compliance with child care meal pattern requirements. Medication: It was reported that medication is not administered. Transportation: Transportation is not provided. Weapons: It was reported that the facility was in compliance with child care requirements regarding firearms. The violation cited was corrected during the visit therefore no further action is required. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. I observed an aerosol spray can on the kitchen floor. .2820(b) Compliance History: The program’s compliance history was ninety-three percent (93%) prior to today’s visit. Rated License: Hold harmless has been extended until the new QRIS (Quality Rating and Improvement System) is implemented, this is also known as SB 425 (Senate Bill 425). Providers in Cohort #1 are not required to go any further with a rated license assessment unless requested, and providers in Cohort #2 do not need to start their preparation year unless a rated license assessment is requested. Technical Assistance: Please remember that rule requirements list that children’s emergency information section on their application needs to be updated annually. The recognizing and reporting child abuse, child neglect, and child maltreatment training had two (2) dates of completion. After further review, you shared that you also had two (2) dates. This training is part of the health and safety training topics that need to be completed every five (5) years. Once you log in to the website, please print the most current certificate of completion. If it has been more than five (5) years, please complete the training to have it current with rule requirements. Your Fire Marshal reviewed the difference between both documents and on 10/16/23 an inspection was completed. We reviewed to send me a copy once it’s completed. To prevent this violation from re-occurring we discussed having your fees and inspection consistent and completed in the same month and completed prior to the due date. We also reviewed adding this inspection and reminders to a calendar to see when it is due. Standing water can create risks of injury to children, the standing water observed in the trays of the two (2) play structures was dumped out during the visit. To prevent this from re-occurring complete daily walkthrough of your outdoor learning environment especially after any inclement weather. First Aid and CPR training needs to be renewed on or before the expiration of the certification. For the future make sure to schedule and have a visual reminder on a calendar to complete your First Aid and CPR training before it is due.
