The last Annual Compliance Visit was conducted on September 17, 2024. The center currently operates with a Five Star Child Care License that was issued on July 24, 2020. The Program Director, Ms. Perrin, as well as the Program Coordinator, Ms. Agnello, were present and assisted me during the visit. They accompanied me on a walk-through of the facility where attendance was documented, and observations made. Infants were observed engaging in floor play and routine activities. Older children were observed finishing breakfast and interacting in free play activities indoors. The last sanitation inspection was completed April 16, 2025, with an “Approved” classification. The last fire inspection was conducted on May 23, 2025, and violations were cited during that visit. The inspector has not returned to review the corrections. The program reached out to them in July and will call again to request that they come out to verify corrections. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. One infant in Space 1 did not have an infant feeding plan on file or in the classroom. 10A NCAC 09 .0902(a) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Staff person J.A. did not complete the required number of on-going training hours. .1103(a) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The EPR plan had not been reviewed since 2017 according to the plan cover page. .0607(e) The above violation must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by September 25, 2025. TECHNICAL ASSISTANCE: A violation was cited regarding the completion of annual ongoing in-service training hours. It is important to ensure that all employees meet their training requirements each year so that they are current with best practices. A violation was cited pertaining to feeding plans in the infant classroom. Each child in the infant classroom must have a completed feeding plan that is kept in the classroom and updated as their needs change so that anyone working in the infant classroom will know each child’s specific feeding needs. A violation was cited regarding the Emergency Preparedness Plan. Each year, the plan must be reviewed and updated as needed to ensure the most current information is in the plan. We discussed that once updated the updated portions should be printed and placed in the plan. The first page of the plan, (cover page), should also be printed to for verification of the update annually. We talked about the QRIS, including where to find information, as well as the upcoming in-person meeting that will provide information on QRIS. It is best that you review all of the QRIS information and the pathways and begin to determine which pathway might work best for your program. We also discussed beginning to ensure all staff members have current information in their DCDEE WORKS accounts. REMINDERS: -Contact the fire inspector immediately to have them come and reinspect the premises due to several violations noted on the inspection from May 2025. Please send me a copy of the approved inspection, preferably on the DCDEE Fire Inspection form. RESOURCES: -The Health and Safety Trainings are offered on the DCDEE Moodle format. The director was present and assisted me during the visit. She accompanied me on a walk-through of the facility where attendance was documented and observations made. Children were observed eating morning snack, participating in free play and teacher directed indoor play and outdoor play. The last sanitation inspection was completed March 31, 2025, with a “Provisional” classification. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were observed. Spaces 1, 3 & 8 contained diaper creams with expired or missing forms. 10A NCAC 09 .0803(1)(a & b) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. There was no documentation of orientation on file for one employee who has been employed for 6 months. .1101(a)(b) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. The medical report was missing for two employees. .0701(d) The above violations must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by April 15, 2025. We discussed the importance of all pre and post-employment requirements being completed for all staff members. This ensures that staff are medically able to care for children and are properly oriented to the program’s guidelines. The North Carolina Secretary of State website was used to check the status of the corporation. The facility’s owner is current/active. The director, Ms. Alston, was present and assisted me during the visit. She accompanied me on a walk-through of the facility where attendance was documented and observations made. Children were observed participating in indoor play, lunch and preparations for nap time. The last sanitation inspection was completed April 10, 2024 with an “Approved” classification. The last fire inspection was conducted September 5, 2023 and the facility was approved for daytime care only. The fire inspector came on August 30 and stated that he would return during the month of September since the inspection was not due until the month of September. The director will call him to have him return asap. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff member did not have a medical report on file. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff member did not have TB test or screening on file. .0701(a) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file.
