The last Annual Compliance Visit was conducted on October 9, 2025. This center located in a residence currently operates with a Four Star Child Care License that was issued on October 31, 2017. The owner and her daughter were not present during the visit. I did speak briefly with the owner by telephone. Infants were observed engaging in routine care and older children were observed engaging in indoor free play and playing outdoors. The last sanitation inspection was completed May 12 2026, with a “Superior” classification. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Wood paneling/particle board has been put up on outdoor fence and has sharp/rough areas that may cause splinters or injury to children. 10A NCAC 09 .0601(a) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. No sleep checks were in use for children under 12 months of age. .0606(g) 1739 All records required were not available for review by a representative of the Division. Records were not available for review for volunteers, subs and drop in children. .2318(1-8) The above violations must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by June 16, 2026. You can email the letter to me at caroline.sykes@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number If a compliance letter is not received to my office by 5:00 PM June 16, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. The staff were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/, on the home page under the ‘Services’ tab. TECHNICAL ASSISTANCE: A violation was cited regarding records not being made available for review. It is important that records for all staff, volunteers, substitutes and children, including drop-ins, be complete and made available for review. A violation was cited due to no infant sleep checks for children under 12 months of age. Three infants were present and there were no infant sleep checks for the children. One child was asleep during the visit. A violation was cited regarding safe outdoor environment. Wood paneling/particle board has been put up on the fence on the playground. There are rough edges and splintered areas on the wood that can be harmful to children. It is important to ensure the wood surface is smooth and does not cause potential injuries to children. It is important that program administrators/lead teachers are present at centers located in a residence on a regular basis. QRIS: We will schedule a time to discuss the new QRIS since the program administrator was not available today. Please review the QRIS Pathway to the Stars information and decide which pathway might work best for your program. The last sanitation inspection was completed December 20, 2024, with a “Superior” classification. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. We discussed the importance of lesson plans being changed weekly and posted to introduce children to new concepts and provide a variety of activities to enhance learning as developmentally appropriate. Violations regarding playground and fire inspections were cited. We discussed that playground inspections should be completed monthly to help prevent safety concerns. We also discussed that fire inspections must take place on an annual basis to ensure a safe environment for children in care. A violation was cited pertaining to aerosols and disinfectant substances not being stored properly in a locked cabinet or closet. We discussed the importance of lesson plans being changed weekly and posted to introduce children to new concepts and provide a variety of activities to enhance learning as developmentally appropriate. Violations regarding playground and fire inspections were cited. We discussed that playground inspections should be completed monthly to help prevent safety concerns. We also discussed that fire inspections must take place on an annual basis to ensure a safe environment for children in care. A violation was cited pertaining to aerosols and disinfectant substances not being stored properly in a locked cabinet or closet. The last Annual Compliance Visit was conducted on December 8, 2022. This center located in a residence currently operates with a Four Star Child Care License that was issued on October 31, 2017. The owner was not present. The owner’s daughter, K. Paisant, who is also one of the Lead Teachers assisted me during the visit. Children were observed participating in indoor and outdoor play. The last sanitation inspection was completed June 20, 2023 with a “Superior” classification. The last fire inspection was conducted January 6, 2022 and the facility was approved for daytime care only. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was conducted January 6, 2022. 10A NCAC 09 .0304(a) 807 A safe indoor and outdoor environment was not provided for the children. In the room where infants and one-year-olds are cared for, there were two (2) HVAC floor registers that were bent and could cause a child to be pinched or injured if they put there fingers under them. 10A NCAC 09 .0601(a) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last inspections were completed in 2022. .0605(q) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two staff members' (S. Paisant and K. Paisant) First Aid certification expired October 2023. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Two staff members'(S. Paisant & K. Paisant) CPR certification expired in October 2023. .1102(d) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Two (2) children's files did not have a medical exam or health assessment on file.
