The facility currently operates with a Five Star Child Care License that was issued on March 9, 2018. The last annual compliance visit was conducted on May 7, 2025. Upon arrival, I signed in at the school’s main office and then proceeded to the Site Director’s office. The site director, Ms. Farrington, assisted me with the visit. The children were observed arriving in the gym and participating in a group game. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violation was observed: Violation Number Comment Rule 847 Parent's medication authorization did not include required information. A medication for one child did not have a FARE allergy form or medication permission administration form. 10A NCAC 09 .0803(4)(6-9) The above violations must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by November 24, 2025. You can email the letter to me at caroline.sykes@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number If a compliance letter is not received to my office by 5:00 PM November 24, 2025, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. TECHNICAL ASSISTANCE: A violation was cited regarding a medication for a child that did not have any documentation, permission or information for administering the medication. We discussed that all medications must have permission to administer paperwork to accompany them, to ensure proper dosage information is present and followed if the medication is administered. The staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for staff to be knowledgeable of all of the licensing requirements that apply to this program. Upon my arrival I was greeted by the Program Coordinator C. Farrington, she assisted me during today’s visit. The children were observed participating in free play outdoors on their designated playground and in the cafeteria. The last annual compliance visit was conducted April 27, 2023. The center is operating under a Five Star License that was issued March 9, 2018. The last sanitation inspection was conducted April 15, 2024 the center received a Superior rating. Ten children’s files were monitored. Staff files were not accessible. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 102 The license was not posted in a prominent place at all times.The children are using the cafeteria for their child care space and the license was not posted. G.S. 110-99(a1) 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center.The children are using the cafeteria for their child care space and the summary of child care laws was not posted. GS 110-102 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. The children are using the cafeteria for their child care space and the ratios were not posted. .0713(a)(10), (c) & (f)(3); .2818(e) 428 A current activity plan was not posted for each group of children for reference. The children are using the cafeteria for their child care space and a current activity plan was not posted. GS 110-91(12); .0508(a) 1739 All records required were not available for review by a representative of the Division. Staff records were not available for review. .2318(1-8) The above violation must be corrected immediately, and a letter must be sent to me no later than May 2, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. The license is located in a space that is not accessible, a request was emailed to inquire about getting a copy of the license so that it could be posted. I have emailed any applicable forms needed from the DCDEE website. UPDATES The Division of Child Development has announced their plan to resume the Star Rated License process. Facilities have been divided into 3 Cohorts. Each cohort will have a preparation year and an assessment year. Your facility is in Cohort 1. Your preparation year began July 1, 2023 and will end on June 30, 2024. Your assessment year begins July 1, 2024. The preparation year is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. It could be helpful to consider areas where refresher information might be helpful. These ideas can then be incorporated into ongoing training and/or professional development plans as you prepare for your assessment year when the regular star rated license reassessment process is required. Some preparation year ideas to consider are: 1. Review your previous SACERS assessments. 2. Look for assessment related resources at www.ncrlap.org 3. Seek out webinars and/or in-person trainings related to topics of interest. 4. Start a self-study process to become more familiar with the ERS or priority areas relevant to your program 5. Schedule staff meetings to discuss topics related to the Environment Rating Scales 6. Support and encourage staff during a self-study process. Connect them with resources at ncrlap.org 7.
