The facility currently operates with a Five Star Child Care License that was issued on April 12, 2019. The last annual compliance visit was conducted on March 12, 2025. Upon arrival, I checked in at the school’s office and proceeded to the classrooms, where I was greeted by the teachers, who assisted me during the visit. I completed a walk-through of the classrooms where attendance was documented, and observations made. Children were observed eating lunch, engaging in group movement activities, participating in routines and transitioning to nap. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. The children's files reviewed today did contain parent acknowledgement of having received a summary of the NC Child Care Laws and Rules. GS 110-102 1203 Operational policies were not discussed with parents on or before the child's first day and/or they were not notified in writing of all changes. The children's files reviewed today did not contain parent acknowledgement of receipt of operational policies. 10A NCAC 09 .0514(b) 1314 Emergency information did not name child's health care professional. Upon my arrival I signed in at the office and proceeded to the classrooms. Some of the classroom teachers were present and assisted me during today’s visit. One classroom did have a substitute today. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed in free play and eating lunch. The last annual compliance visit was conducted March 25th, 2024. The center is operating under a Five Star License that was issued April 12, 2019. The last sanitation inspection was conducted March 11th, 2025, the center received a Superior rating. Fire drills were monitored. All staff and four children’s files were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. Some of the files for staff were not made available for review. Marcia Humphrey, Lead Licensing Consultant, accompanied me on the visit. Upon arrival, we were greeted by Dr. Sophia Mapp, Preschool Director for Chapel Hill Carrboro Schools. She escorted us to the new outdoor learning environment, where we took measurements of the space. The space is approved for 26 children at 100 square foot per child. Dr. Mapp will contact me with the information regarding the manufacturer's stated age range for the equipment in use. Children were observed engaging in indoor activities in the classrooms. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were cited. Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. There was not thermometer in the refrigerator in space M6 where adult and children's items were stored. 15A NCAC 18A .2806(j)(2) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space M6 there were 6 electrical outlets that were not covered. 10A NCAC 09 .0604(c) 875 The center did not follow all procedures to prevent the spread of communicable diseases described in 15A NCAC 18A .2800. Upon my arrival I signed in at the office and proceeded to the classrooms. The classroom teachers, were present and assisted me during today’s visit. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed resting and preparing to go home. The last annual compliance visit was conducted April 27, 2023. The center is operating under a Five Star License that was issued April 12, 2019. The last sanitation inspection was conducted October 16, 2023 the center received a Superior rating. Fire drills were monitored. All staff and six children’s files were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 209 Children used space that was not approved. The outdoor play area was located behind the two classrooms, this space was not approved for outdoor play. GS 110-91(1)&(4-5) 434 The materials and equipment indoors and outdoors were not sufficient to provide a variety of play experiences to promote the children's emotional and social development, health and physical development, approaches to play and learning, language and communication development, and cognitive development. There was not enough gross motor materials and equipment to facilitate gross mother play. 10A NCAC 09 .0509(2) 807 A safe indoor and outdoor environment was not provided for the children. There were 6 large adult size picnic tables in the middle of the space used for outdoor play that could be a potential hazard for the children 10A NCAC 09 .0601(a) 813 Electric fans were not mounted out of reach or did not have a mesh guard to prevent access. There was a white fan that was sitting on top of a shelf that was not out of reach or covered by a mess cover. 10A NCAC 09 .0604(d) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The outdoor play area was not enclosed by a fence. GS 110-91(6); .0605((i) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Several staff did not complete this training. .1102(g) The above violation must be corrected immediately, and a letter must be sent to me no later than April 8, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. Your assessment year begins July 1, 2024. The preparation year is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. It could be helpful to consider areas where refresher information might be helpful. These ideas can then be incorporated into ongoing training and/or professional development plans as you prepare for your assessment year when the regular star rated license reassessment process is required. Some preparation year ideas to consider are: 1. Review your previous ECERS-R and ITERS-R assessments. 2. Look for assessment related resources at www.ncrlap.org 3. Each cohort will have a preparation year and an assessment year. Your facility is in Cohort 1. Your preparation year began July 1, 2023 and will end on June 30, 2024. Your assessment year begins July 1, 2024. The preparation year is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. It could be helpful to consider areas where refresher information might be helpful. These ideas can then be incorporated into ongoing training and/or professional development plans as you prepare for your assessment year when the regular star rated license reassessment process is required. Some preparation year ideas to consider are: 1. Review your previous ECERS-R and ITERS-R assessments. 2. Look for assessment related resources at www.ncrlap.org 3. Seek out webinars and/or in-person trainings related to topics of interest. 4. Start a self-study process to become more familiar with the ERS or priority areas relevant to your program 5. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. Upon my arrival I signed in at the office and proceeded to the classrooms. The classroom teachers, were present and assisted me during today’s visit. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed resting and preparing to go home. The last annual compliance visit was conducted April 27, 2023. The center is operating under a Five Star License that was issued April 12, 2019. The last sanitation inspection was conducted October 16, 2023 the center received a Superior rating. Fire drills were monitored. All staff and six children’s files were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 209 Children used space that was not approved. The outdoor play area was located behind the two classrooms, this space was not approved for outdoor play. GS 110-91(1)&(4-5) 434 The materials and equipment indoors and outdoors were not sufficient to provide a variety of play experiences to promote the children's emotional and social development, health and physical development, approaches to play and learning, language and communication development, and cognitive development. There was not enough gross motor materials and equipment to facilitate gross mother play. 10A NCAC 09 .0509(2) 807 A safe indoor and outdoor environment was not provided for the children. There were 6 large adult size picnic tables in the middle of the space used for outdoor play that could be a potential hazard for the children 10A NCAC 09 .0601(a) 813 Electric fans were not mounted out of reach or did not have a mesh guard to prevent access. There was a white fan that was sitting on top of a shelf that was not out of reach or covered by a mess cover. 10A NCAC 09 .0604(d) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The outdoor play area was not enclosed by a fence. GS 110-91(6); .0605((i) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Several staff did not complete this training. .1102(g) The above violation must be corrected immediately, and a letter must be sent to me no later than April 8, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement.
