Your program currently operates with a five-star license, issued October 17, 2024, using the accreditation pathway with Cognia. The last annual compliance visit was conducted on April 3, 2026. The sanitation inspection was completed August 12, 2025, with a “Superior” classification. The last fire inspection was conducted on June 18, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-one percent as of March 23, 2026. The NC Secretary of State website was reviewed on March 23, 2026, and Brainworks Daycare LLC was listed as current- active. You visited each indoor and outdoor space with me. During today’s visit, I observed children in the indoor learning environments. Children were participating in general personal care routines and free play activities. Teachers were seen walking around the room engaging with children during play. I found supervision, ratios, and interactions to be in compliance. The following were also inspected during today’s visit: program requirements, equipment, furnishings, staff/child ratios, supervision, medication, medication administration, outdoor learning environments, and program records including staff records and children records. The following was cited during today’s inspection. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Teacher HR completed their first aid training after their first 90 days of employment. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Your program currently operates with a five-star license, issued October 17, 2024, with a Cognia accreditation that includes early learning standards. The last temporary time period visit was completed on September 17, 2024. The sanitation inspection was completed December 5, 2024, with a “Superior” classification. The last fire inspection was conducted June 27, 2024, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent as of April 3, 2025. The NC Secretary of State website was reviewed on April 3, 2025, and Brainworks Daycare, LLC was listed as current- active. You visited each indoor and outdoor space with me. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were monitored during today’s inspection. The caregivers were interacting and meeting the developmental needs for each of the children. During today’s visit, I also monitored staff records, children records, emergency drills, and all child care rules. Lunch was observed and consisted of macaroni and cheese, salad, tropical fruit, and milk. The following was cited during today’s inspection. This program currently operates with a temporary license effective April 16, 2024, to October 16, 2024, because of a Change of Ownership. Applicable childcare requirements were discussed with Amber Best, Administrator, and Anna Sharma, Owner. They both accompanied me as I conducted a walkthrough of the indoor and outdoor spaces used for child care. The children observed today were interacting with assigned staff, playing in free choice areas such as fine motor materials, books, blocks, and dramatic play materials. The children were observed during routine activities such as eating breakfast and hand washing. Teachers adequately supervised children. For breakfast the children were served papaya, cereal, and milk. During the visit three (3) children’s files and four (4) staff files were reviewed during the visit. All center program records were also monitored. It is the center’s responsibility to review all files for completeness. Overall attendance and enrollment were collected today. The attached space capacity, and enrollment worksheet reflects the enrollment and attendance information along with the staff that were present and caring for children today. The indoor and outdoor space calculations were checked during the visit. There were no changes to the space calculations and there were no renovations made to the classrooms or playgrounds. At the completion of the visit, the Visit Summary was reviewed with Mrs. Best and Mrs. Sharma. Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. This was not updated annually for one staff. The staff completed it during the visit so, it was corrected during the visit. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. This was not updated annually for one staff. The staff completed it during today's visit so, it was corrected during the visit. .0701(a) Two (2) violations were cited during today’s visit. They were corrected during the visit so they were not required to submit a compliance letter to me. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit may negatively affect your compliance score. Your compliance history is 96%. A copy was also given to you during today’s visit. Rated License Assessment: In order to maintain subsidy payments at the end of the temporary time period, the facility must transition to a three to five star rated license. Education Standards: The education of the Administrator, Lead Teachers, Teachers, and Group Leaders will be assessed for points for the star rated license. It is imperative to submit official transcripts directly to DCDEE and have employees submit all other information using “WORKS” (Workforce online reporting and knowledge system) no later than September 1, 2024. All staff education should be current before the fifth month of the temporary time period as a rated license will need to be processed before the end date of the temporary license. Program Standards: The center may receive 2 points in program standards by meeting enhanced staff/child ratios or enhanced space and by providing the required activity centers and supplemental centers. To receive more than 2 points in program standards, the facility will need to have an Environment Rating Scale (ERS) assessment and score at least a 4.0 on each of the scales. Be sure to visit www.ncrlap.org to familiarize you and your staff with the different assessments. Also, if you meet the requirements for a 4-5 star rated license and plan to provide care to 4 year old’s you will be required to have a curriculum that has been approved by DCDEE. To receive a three star license the facility must earn at least seven points. To receive a four star license the facility must earn at least ten points. To receive a five star license the facility must earn at least thirteen points. Technical Assistance: Quality Child Care: Providing quality child care is essential for children. There is ample research demonstrating that high quality child care experiences have many benefits to children and society and that quality child care is linked to children’s improved school performance. Classrooms with a planned curriculum have a more positive impact on child outcomes than one with no set curriculum. A comprehensive curriculum will address all areas of a child’s development including social and emotional, physical and health, cognition and general knowledge, and language and communication. For example, activities promoting literacy, math and science concepts, music and art are typically included. Child care licensing requirements are established to ensure a safe and healthy child care environment. It is your responsibility to maintain compliance with all applicable laws and rules at all times. Periodically review the child care law and rules.
