    Name of Operation: ONE TWO BUCKLE MY SHOE Facility ID: 68000257 Consultant: CAROLINE SYKES Operation Type: Family CC Home Case Number: Visit Date: 2/18/2025 Number Present: 5 Completed Date: 2/18/2025 Age: From 1 To 6 Total Minutes: 105 Time In: 09:15 AM Time Out: 11:00 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for all applicable Child Care Requirements. The program currently operates with a Five Star Child Care License that was issued on July 26, 2017. The last Annual Compliance Visit was conducted on March 5, 2024. Upon arrival to the home, I was greeted by the provider, Danise Webb, who was present with four preschool children. The children were observed engaging in indoor free play activities and teacher directed activities. During the visit I monitored indoor and outdoor spaces. In the home the playroom is the primary care area and included a variety of age appropriate toys and materials. The outdoor play area also contained a variety of toys. The provider’s license and parental information were posted on a bulletin board in the playroom. The provider’s file, children’s files, and program files were reviewed. A full assessment of applicable child care rules was conducted. A checklist was also used for monitoring purposes. The following violations were observed. Violation Number Comment Rule 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. Three children did not have a signed copy of the shaken baby syndrome and abusive head trauma policy on file indicating that parents had received and reviewed the policy. .1726(b)&(c) 2041 Application did not include the names and the phone numbers of the child's physician and the persons to be contacted in an emergency situation. One child's application did not contain the preferred hospital or child's physician's contact information. 10A NCAC 09 .1721(a)(3)(G)(H) The above violations must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by March 4, 2025. You can email the letter to me at caroline.sykes@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules.
