Adrienne Murrell, investigations consultant, was also present and assisted me during the visit. Suzi Byrnes, owner, accompanied me during a walk-through of the family childcare home. During the visit, I spoke with Ms. Byrnes. Limited monitoring of child care requirements occurred during today’s visit. The following violation was cited during this visit: Violation Number Comment Rule 920 Records were not made available for review. A file for an 8 year old child was not available for review. G.S. 110-91(9) All violations documented above must be corrected immediately. A written, dated, and signed letter of compliance must be submitted to me within one week, by March 19 2026, describing how and when the violations were corrected and how compliance will be maintained in the future. Adrienne Murrell accompanied me on this visit. Suzi Byrnes accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Suzi Byrnes. Limited monitoring of child care requirements occurred during today’s visit. I monitored discipline, supervision, staff/child ratio, adequate/approved space, license posted, and permit restrictions. Violation Number Comment Rule 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. During the visit, attendance records were not available for review upon request. .1721(e)(6) 1414 Written permission to administer over-the-counter medications was not valid for up to 30 days at a time. A written permission form for over-the-counter medication was not provided for review upon request .1720(b)(4) 1423 Operator did not record required information when administering prescription or over-the-counter medication. There was no medication log available for review upon request. .1720(b)(13)(A-E) 1601 License was not posted in a place in the home where parents are able to view daily. The child care license, NC Child Care Law, emergency medical care plan and the written plan of care were not posted at the time of the visit. G.S.110-99(b) & .1702(i) 1889 Products that are labeled "keep out of reach of children" without any other warnings, were not stored on a shelf or in an unlocked cabinet that is five feet above the finished floor. During the visit, multiple bottles of vitamins and CBD butter were observed within reach of children in the home. These items were accessible to children at the time of the visit. .1719(a)(7) 2055 An individual sleeping space for resting such as a bed, crib, play pen, cot, mat, or sleeping bag with individual linens was not provided for each pre-school age child who was in care for four hours or more, or for all children if overnight care is provided. Upon arrival, I was invited in by the provider, Suzi Byrnes. She was caring for four children. Another child arrived a short time later. I observed the children playing with the toys and materials, toddlers eating a snack, being diapered, and playing in the outdoor learning environment. I conducted a walk-through of the indoor and outdoor learning environment. I reviewed the program records and the provider and children’s records. I observed the following violations today. I reviewed the items with Ms. Byrnes and provided technical assistance. Violation Number Comment Rule 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. "There were several diaper creams for children. There was no authorization from the parents. There were vitamin supplements as well without the authorization. I observed the provider adminster the vitamins orally to three children." 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) 1301 Operator did not complete the required number of on-going training hours as specified in rule. "The provider completed 3 hours of on-going training. She is required 8 hours annually." GS 110-91(11); 10A NCAC 09 .1705(b)(5) 1853 The operator did not conduct a monthly fire drill. "The last fire drill the provider conducted was in February 2023." .1719(a)(15) & .1721( e)(2) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. Upon arrival, I was invited in by the provider, Suzi Byrnes. She was caring for four children. Another child arrived a short time later. I observed the children playing with the toys and materials, toddlers eating a snack, being diapered, and playing in the outdoor learning environment. I conducted a walk-through of the indoor and outdoor learning environment. I reviewed the program records and the provider and children’s records. I observed the following violations today. I reviewed the items with Ms. Byrnes and provided technical assistance. Violation Number Comment Rule 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. "There were several diaper creams for children. There was no authorization from the parents. There were vitamin supplements as well without the authorization. I observed the provider adminster the vitamins orally to three children." 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) 1301 Operator did not complete the required number of on-going training hours as specified in rule. "The provider completed 3 hours of on-going training. She is required 8 hours annually." GS 110-91(11); 10A NCAC 09 .1705(b)(5) 1853 The operator did not conduct a monthly fire drill. "The last fire drill the provider conducted was in February 2023." .1719(a)(15) & .1721( e)(2) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. "The provider conducted the last quaterly shelter-in-place drill or lockdown drill was in February 2023." .1719(a )(16) & .1721(e )(7) The above violations must be corrected immediately. I must receive a compliance statement/ letter by email or mail by 3/26/2024. There may be a follow up visit if I do not receive the letter by 3/26/2024. Include in the letter the following information: 1. Your facility name and ID# 2. Visit date 3. Violation item number 4. How and when the violation was corrected (Be specific) 5. Falsification: If the compliance letter/statement states that corrections have been made when they have not, this may be considered falsification of information. Compliance: Child care providers are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain compliance history score of at least 75% for the past 18 months or during the length of time the facility operated, whichever is less. Failure to do so may result in an issuance of a provisional license or other administrative action. This is child care requirement Rule .1703. >Written plan of Care clarification: If you do not perform any routine tasks, mark to indicate that on your Written Plan of Care. >Diaper changing procedure clarification: Wipes can be used during the diaper changing procedure but do not replace handwashing at the end of the diaper change. >All medications for children require a parent’s written authorization prior to administering the medications. Over the counter and prescription medications including diaper cream, sun screen, vitamins, lip balms and so on require authorizations. Please refer to child care rule.1720 for details regarding the autnorization." >We talked about the options of maintaining children’s and provider records that are required. You may want to print out the File Checklists which are on the DCDEE website for the children and yourself and attach to each child’s and your file for reference. I reviewed today’s visit with Ms. Byrnes ussing the computer and provided an opportunity to ask questions regarding today's visit and interpretation and implementation of child care requirements.
