The facility currently operates with a Five Star Child Care License that was issued on March 6, 2020. The last annual compliance visit was conducted on April 29, 2025. Upon arrival, I signed in at the school’s main office and then proceeded to the preschool classrooms where I was greeted by the staff. Children were observed engaging in teacher directed activities and transitioning to naptime. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were cited. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted in either classroom. GS 110-91(12); .0508(a) 1041 Prior to employment a Criminal Background Check was not completed. A substitute teacher in one of the classrooms does not have a completed criminal background check and qualification letter. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. A substitute teacher in one of the classrooms did not have a valid qualification letter on file. G.S. 110-90.2(b) & (d) & .2703(e) The above violation must be corrected immediately. A Corrective Action Letter must be drafted and sent to me within 14 business days. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. TECHNICAL ASSISTANCE: A violation was cited related to lesson plans not being posted in the classrooms. One classroom had a lesson plan posted that was not current and the other classroom did not have a lesson plan posted. We discussed the importance of ensuring that lesson plans are current and posted in the classroom at the beginning of each week. A violation was cited pertaining to a substitute teacher in one of the classrooms not having a current criminal background check. We discussed the importance of ensuring any person working with children in the NC PreK classrooms must have a criminal background check through the NC DCDEE system. L. Joshi was asked to leave the classroom immediately. The lead teacher and I discussed ratios to ensure it was maintained with the substitute leaving. I also spoke with the PreK office at the central office to let them know that I had asked Ms. Joshi to leave. We also discussed that anyone working in the classrooms must have a background check with qualification letter, regardless of if they count in ratio or not. On April 30, 2025, an staff member was observed on video footage sitting for most of the time during recess and not interacting with children. In addition, two staff members were observed on their phones, not adequately supervising children. .1801(a)(1-5) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. Upon my arrival I was greeted by the teachers they assisted me during today’s visit. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were observed participating in free play indoors, napping and leaving with parents. The last annual compliance visit was conducted May 24, 2023. The center is operating under a Five Star License that was issued March 23, 2017. The last sanitation inspection was conducted April 18, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. New staff and three children’s files were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. There was no fire drill documented for October 2023. .0604(t); .0302(d)(5) 807 A safe indoor and outdoor environment was not provided for the children. On the playground there is a rope climber, one of the ropes on the bottom is broken. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space 130, there was a cabinet, that was unlocked with the key in the lock, that contained several different cleaning products. .2820(b) 853 Incident logs were not completed and maintained as required. Neither of the classrooms had an incident log. .0802(g)(1-6) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not have documentation of the training on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff did not have documentation of the training on file. .1102(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff did not have the training on file. .1102(g) The above violation must be corrected immediately, and a letter must be sent to me no later than May 19, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. Facilities have been divided into 3 Cohorts. Each cohort will have a preparation year and an assessment year. Your facility is in Cohort 1. Your preparation year began July 1, 2023 and will end on June 30, 2024. Your assessment year begins July 1, 2024. The preparation year is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. It could be helpful to consider areas where refresher information might be helpful. These ideas can then be incorporated into ongoing training and/or professional development plans as you prepare for your assessment year when the regular star rated license reassessment process is required. Some preparation year ideas to consider are: 1. Review your previous ECERS-R and ITERS-R assessments. 2. Look for assessment related resources at www.ncrlap.org 3. Seek out webinars and/or in-person trainings related to topics of interest. 4. Start a self-study process to become more familiar with the ERS or priority areas relevant to your program 5.
