Name of Operation: MI ESCUELITA SPANISH IMMERSION PRESCHOOL Facility ID: 68000562 Consultant: CAROLINE SYKES Operation Type: Center Case Number: Visit Date: 6/25/2026 Number Present: 33 Completed Date: 6/25/2026 Age: From 1 To 5 Total Minutes: 190 Time In: 09:20 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for applicable Child Care Requirements and complete the Rated License Assessment. The facility currently operates with a Five Star Child Care License that was issued on November 20, 2020. The last annual compliance visit was conducted on July 9, 2025. The Program Director was not present upon my arrival but arrived later. The assistant director was present and assisted me during the visit. A walk-through of the facility was completed, where attendance was taken and observations made. Children were observed interacting in teacher directed and free play indoor and outdoor activities. The last sanitation inspection was competed on March 3, 2026, with a “Superior” classification. The last Fire Inspection was completed on November 24, 2025, and the facility was approved for daytime care only. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violation was cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A piece of the plastic border/edging that encircles the climbing structure on the outdoor learning environment is broken, leaving a sharp jagged edge. 10A NCAC 09 .0601(a) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by July 9, 2026. The letter must address each violation and explain how it has been corrected. Please send this to caroline.sykes@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number If a compliance letter is not received to my office by 5:00 PM July 9, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. TECHNICAL ASSISTANCE: A violation was cited regarding safe outdoor environment. We discussed the importance of ensuring there are not sharp edges or other hazards on the outdoor learning environment. The plastic border on the outdoor learning environment that surrounds the climbing structure has a broken piece, causing sharp edges. This needs to be removed/replaced to ensure safety. We also discussed emergency medications and topical medication forms. RATED LICENSE ASSESSMENT: The program plans to apply for the star rated license via Pathway 2, Classroom and Instructional Quality. They meet the 5 Star Education Requirements with 50% of their Lead Teachers and 50% of other educators. They meet the Family and Community Engagement foundational practices and 4 additional practices. The administrator is developing a CQI Plan for the facility and the staff have completed their individual CQI/Staff Development plans. They meet the approved curriculum and formative assessment component by using the Creative Curriculum for curriculum and assessments. They share the assessment information with families twice annually. RESOURCES: -The Health and Safety Trainings are offered on the DCDEE Moodle format. The facility currently operates with a Five Star Child Care License that was issued on November 20, 2020. The last annual compliance visit was conducted on July 16, 2024. The Program Director, Ms. Marie Ann, was present and assisted me during the visit. A walk through of the facility was completed, where attendance was taken and observations made. Children were observed interacting in free play, teacher directed activities and music class. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were cited. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Two creams in Space 1 did not have current medication permission forms completed by parents. 10A NCAC 09 .0803(1)(a & b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. An Epi Pen in Space 1 was not in the original labeled container. .0803(2)(a) The above violations must be corrected immediately, and a letter must be sent to me no later than February 27, 2025 stating the item number and how the item was corrected.
