    Name of Operation: MCDOUGLE ELEMENTARY PREK/HEAD START Facility ID: 68000381 Consultant: CAROLINE SYKES Operation Type: Center Case Number: Visit Date: 3/4/2026 Number Present: 19 Completed Date: 3/4/2026 Age: From 3 To 5 Total Minutes: 120 Time In: 11:00 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to conduct the Annual Compliance monitoring with Rated License Assessment. The last Annual Compliance Visit was conducted on March 11, 2025. The center currently operates with a Five Star Child Care License that was issued on March 26, 2018. Upon arrival, I signed in at the school’s main office and then proceeded to the preschool classrooms, where I was greeted by the teachers. The lead teachers in each classroom assisted me during the visit. Attendance was documented and observations made. Children were observed participating in outdoor activities, preparing for and eating lunch, and preparing for nap. The last Fire Inspection was conducted in October 20, 2025. The last Sanitation Inspection was completed September 17, 2025, with a “Superior” classification. A full assessment of child care requirements was conducted during this date. I monitored program requirements, including postings such as, permit with restrictions maintained, summary of laws, daily schedule, First-Aid posters and the staff/child ratio charts. I monitored program records, a sampling of children’s records, staff personnel files, medical files and on-going trainings. The following violations were cited during the visit. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One of the children's files reviewed did not contain acknowledgement of receipt of the NC Child Care Law. GS 110-102 1203 Operational policies were not discussed with parents on or before the child's first day and/or they were not notified in writing of all changes. One child's file reviewed did not contain acknowledgement of receipt of operational policies. 10A NCAC 09 .0514(b) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. None of the children's files reviewed contained emergency medical information. .0802(c) 1314 Emergency information did not name child's health care professional. None of the files reviewed contained the responsible party's choice of health care professional. .0802(c)(2) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. One of the files reviewed did not contain a signed and dated statement by parent that discipline policy waws received. .1804(c) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. One of the children's files reviewed did not contain a signed and dated statement acknowledging receipt of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma Policy. .0608(b)(1-6) The above violations must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by March 18, 2026. You can email the letter to me at caroline.sykes@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number If a compliance letter is not received to my office by 5:00 PM March 18, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. TECHNICAL ASSISTANCE: Violations were cited regarding children’s files. The following documents must be included in the children’s files- A signed and dated statement by parents acknowledging receipt of Summary of the Child Care Laws and Rules Acknowledgement by parents of receipt and discussion of operational policies Signed and dated acknowledgement by parents of receipt of the discipline policy Signed and dated acknowledgment by parents of Prevention of Shaken Baby Syndrome and Abusive Head Trauma Policy. These acknowledgement of receipt of these documents should be in each child’s file. A violation was cited regarding missing emergency medical care information including the responsible party’s choice of health care professional. It is important that this information be included in the child’s file in the event an emergency arises. RATED LICENSE INFORMATION: We discussed the QRIS Pathways to the Stars. The program will apply for Pathway 3 using their Head Start certification. I have received the application and staff child ratio form. Please send me the Request for Accreditation and Head Start Pathway form. Once this is received, I will submit the rated license packet for review. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care laws and rules.
