Name of Operation: MARY SCROGGS ELEMENTARY PRE-K/HEAD START Facility ID: 68000434 Consultant: CAROLINE SYKES Operation Type: Center Case Number: Visit Date: 3/10/2025 Number Present: 24 Completed Date: 3/10/2025 Age: From 3 To 5 Total Minutes: 130 Time In: 11:35 AM Time Out: 01:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to conduct the Annual Compliance monitoring. The last Annual Compliance Visit was conducted on April 10, 2024. The center currently operates with a Five Star Child Care License that was issued on January 21, 2020. Upon arrival, I signed in at the school’s main office and then proceeded to the preschool classrooms. In the first classroom I was greeted by Ms. Campbell. There are two Pre-K classrooms at this location. Ms. Reismann is the lead teacher in the second classroom. During the visit, attendance was taken and observations made. Children were observed participating in outdoor play, preparing for and eating lunch, and preparing for nap. The last Fire Inspection was conducted November 22, 2024 and the facility was approved for daytime care only. The last Sanitation Inspection was completed December 3, 2024 and achieved a “Superior” classification. A full assessment of child care requirements was conducted during this date. I monitored program requirements, including postings such as, permit with restrictions maintained, summary of laws, daily schedule, First-Aid posters and the staff/child ratio charts. I monitored program records, a sampling of children’s records, staff personnel files, medical files and on-going trainings. The following violations were observed Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. There was no file available for one staff member in space SCR-A. G.S. 110-91( 9) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by March 24, 2025. The letter must address each violation and explain how it has been corrected. Please send this to caroline.sykes@dhhs.nc.gov and include the following: -Facility name -Facility ID number -Each item number The staff were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. TECHNICAL ASSISTANCE: A violation was cited regarding a staff member's file not being available in the classroom. We discussed the importance of all staff members having a file consisting of the required documents onsite. Some documents may be kept at the central office, as long as there is an Off Site Records Form that includes all staff members names. RESOURCES: -The Health and Safety Trainings are offered on the DCDEE Moodle format. Ms. Jacobs is the lead teacher in the second classroom. During the visit attendance was taken and observations made. Children were observed participating in indoor free play, including gross motor activities. They were observed having lunch and resting. The last Fire Inspection was conducted November 29, 2023 and the facility was approved for daytime care only. The last Sanitation Inspection was completed September 12, 2023 and achieved a “Superior” classification. A full assessment of child care requirements was conducted during this date. I monitored program requirements, including postings such as, permit with restrictions maintained, summary of laws, daily schedule, First-Aid posters and the staff/child ratio charts. I monitored program records, a sampling of children’s records, staff personnel files, medical files and on-going trainings. The following violations were observed The following violation was observed cited and a technical assistance was offered. Violation Number Comment Rule 1068 On-going training documentation did not include all applicable information: subject matter, topic area in G.S. 110-91(11), name of training provider, date training was provided, number of hours of training, and name of staff. Training logs were not current and/or completed for each staff member. .1106(a) The above violation must be corrected immediately, and I must receive a letter no later than April 24, 2024 stating how and when the item was corrected. This letter should be e-mailed to me at anita.c.graham@dhhs.nc.gov . If I do not receive your letter by April 24, 2024 and/or if you do not address violation entirely, a return visit to your program may be necessary to monitor for compliance with childcare requirements and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. The staff were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of the licensing requirements that apply to your program. It is always your responsibility to maintain compliance with all of the applicable laws and rules . The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Please be aware that new rules came into effect in January of this year. That record shall include the subject matter, topic area in G.S. 110-91(11), training provider, date provided, hours, and name of staff who completed the training. This documentation shall be on file and current. (b) Each center shall have a record of countable technical assistance in which each staff member participates, including copies of documentation, observations and evaluations provided by the person who has been endorsed by the NC Institute for Child Development Professionals as a technical assistance provider. That record shall include the dates and times of technical assistance with staff, names of all participating staff members and the name of the technical assistance provider. This documentation shall be on file and current.
