    Name of Operation: LITTLE GENIUS LLC Facility ID: 68000574 Consultant: CAROLINE SYKES Operation Type: Center Case Number: Visit Date: 4/10/2025 Number Present: 64 Completed Date: 4/10/2025 Age: From 0 To 5 Total Minutes: 105 Time In: 09:30 AM Time Out: 11:15 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for all applicable Child Care Requirements. The last Annual Compliance Visit was conducted on October 25, 2024. The center currently operates with Three Star Child Care License that was issued on August 9, 2023. The director was present and assisted me during the visit. She accompanied me on a walk-through of the facility where attendance was documented and observations made. Children were observed participating in indoor and outdoor play. The last sanitation inspection was completed March 11, 2025 with a “Superior” classification. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Two children in Space 3 and one child in Space 4 had medical action plans that had expired. 10A NCAC 09 .0803(1)(a & b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. One prescribed medication in Space 3 and one prescribed medication in Space 4 were not stored in their original labeled container. .0803(2)(a) The above violations must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by April 24, 2025. You can email the letter to me at caroline.sykes@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number If a compliance letter is not received in my office by 5:00 PM April 24, 2025, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/, on the home page under the ‘Services’ tab. TECHNICAL ASSISTANCE: A violation was cited regarding prescription medications having expired or outdated medical action plans. We discussed the importance of ensuring action plans are updated regularly so that the most current medical information pertaining to the allergy or illness is available. A violation was cited regarding prescription medications not containing the original packaging with the prescription label. We discussed the importance of having the prescription label for all prescription medicines to verify that dosing information, expiration dates, etc. are accurate. RESOURCES: -The Health and Safety Trainings are offered on the DCDEE Moodle format. The last Annual Compliance Visit was conducted on January 4, 2023. The center currently operates with Three Star Child Care License that was issued on August 9, 2023. The Department of the Secretary’s website was used to check the status of the corporation. The facility’s owner is current/active. The director was present and assisted me during the visit. She accompanied me on a walk-through of the facility where attendance was documented and observations made. Children were observed participating in indoor and outdoor play. The last sanitation inspection was completed November 1, 2023 with a “Provisional” classification. The last fire inspection was conducted October 20, 2022 and the facility was approved for daytime care only. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violation was observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection on file was dated October 20, 2022. 10A NCAC 09 .0304(a) The above violation must be corrected immediately, and I must receive a letter no later than November 16, 2023 stating how the item was corrected. This letter should be e-mailed to me at janet.p.jones@dhhs.nc.gov . If I do not receive your letter by November 16, 2023 and/or if you do not address the violation, a return visit to your program will be made to monitor for compliance and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Facilities have been divided into 3 Cohorts. Each cohort will have a preparation year and an assessment year. Your facility is in Cohort 3. Your preparation year will begin July 1, 2025 and will end on June 30, 2026. Your assessment year begins July 1, 2026. -The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. To take the exam, go to DCDEE Moodle and select Early Childhood Professional Development/Child Care and Development Fund (CCDF)/North Carolina Early Childhood Equivalency Exam.
