The last annual compliance visit was conducted July 9, 2025. The sanitation inspection was completed May 6, 2026, with a “Superior” classification. The last fire inspection was conducted June 18, 2026, and your facility was approved for daytime care only. The center's compliance history was ninety-three (93) percent as of June 24, 2026 and was reviewed with the operator. The NC Secretary of State website was reviewed on June 25, 2026, and Kindercare Education LLC was listed as current- active. Chortia George, assistant director, visited each indoor and outdoor space with me. Children were participating in free play activities in the indoor and outdoor learning environments. Teachers were stationed throughout the learning environments supervising the children. I found supervision and staff/child ratios to be in compliance. Infants were observed during outdoor time. Sleeps checks were monitored and found to be in compliance. In addition, all program records were observed today including children records, staff records, emergence drills, all postings, and all child care requirements. The center does not provide transportation at this time. Lunch today consisted of whole wheat pancakes, scrambled eggs, green beans, fresh cantaloupe and milk. The following violations were cited during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The center had a fire inspection on 5/6/2025 and did not receive a new inspection until June 18, 2026. 10A NCAC 09 .0304(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. J. Your program currently operates with a five-star license, issued September 13, 2024, using the Accreditation Pathway. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-six percent as of February 9, 2026. The NC Secretary of State website was reviewed on February 12, 2026, and Kindercare Education LLC was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Rated license requirements were reviewed to ensure compliance was maintained with the current program and education components as well as the quality point. The following was cited during today’s inspection. Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Teacher WC did not have a updated health questionnaire on file. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Teacher WC did not have the required Emergency Information form on file. .0701(a) 1898 Staff did not complete the health and safety training within one year of employment. The allegation is as follows: There is a concern that the posted staff/child ratios are not being followed in the early morning and late afternoon hours. Staff/child ratio, group size, supervision, use of licensed space, space capacity, and license restrictions were monitored. The license was posted. Files for new staff were reviewed and contained all the required information. Upon arriving at the center, I spoke to the center administrator and informed them of the visit. I then conducted a walkthrough of the facility. I started in the infant’s classroom. The room had 14 children ranging from the ages of infants to one-year olds and three teachers. While the room was in ratio, they were over the maximum group size for the youngest child in care. If infants are present the maximum group size is 10. The group was in the middle of transitioning the one-year-olds over to the toddler room. I then inspected the toddler room. There were 10 children to two teachers. The children were having breakfast, and the teachers were passing out the food. The next room I inspected was the two and three-year old's classroom. There were 13 children to two teachers. They were finishing breakfast and transitioning to circle time on the rug. The three and four-year old's room was inspected and I observed 9 children to one teacher. The group was transitioning from breakfast to circle time. I then inspected the four and five-year old room. There were 13 children to one teacher. The class was in the process of finishing breakfast to transition to the next activity. I interviewed the administrator about the morning and afternoon schedule. They informed me that they recently had an administrator change and that they began the administrator role at the center on February 10, 2025. In their time in this new role, they have sent a letter out to the toddler parents stating that they need a stagger drop off to ensure that the center is following ratio requirements. They stated that this has been working since they made that change on March 3, 2025. Based on my interviews and observations the allegation of not following staff/child ratio was substantiated. I reviewed files for three new staff. Two of the teachers, JM and DT, started today due to staffing shortages and were pulled from another Kindercare Education LLC location. There is a concern of inadequate supervision in the classrooms for infants and three-year-old children. There is a concern related to sanitation and health in the classroom for infants. There is a concern that the posted staff/child ratios are not being followed. There is a concern related to enrollment and child files. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. Files for new staff were reviewed. Additionally, I observed the indoor and outdoor space used by the children. I observed each classroom and outdoor space. Children were participating in free play and group play activities. Today’s menu consisted of chicken nuggets, sweet potatoes, pineapple, and milk. Today I observed and inspected the following concerns: There is a concern that children were not attended to in a nurturing and appropriate manner or in keeping with the children's developmental needs in the classrooms for infants and three-year-old children. Today, children were participating in free play activities while outside. Teachers for the three- and four-year-old class were stationed throughout the outdoor space and helping the children play on the climbing structure. I also observed the one-year-old age group outside. Teachers were seen walking the outdoor play area. One of the children fell forward into a toddler walker and was stuck. The teacher did not respond to assist the child. I intervened and informed the teacher who then responded to the child and helped them back up. Based on the information provided and my observations the allegation of children not being attended to in a nurturing and appropriate manner is unsubstantiated, however the allegation of children's developmental needs not being met is substantiated. There is a concern of inadequate supervision in the classrooms for infants and three-year-old children. During the visit, I observed the one-year-old class in the outdoor learning environment. There were two one year old classes in the outdoor learning environment. I observed children on one side of the play area and the teacher on the other side of the play area talking to another teacher who was in a different outdoor play area. The teacher had their back to the children and could not see the children or hear the children from where they were positioned. Based on the observations of today’s inspection the allegation of inadequate supervision is substantiated. There is a concern related to sanitation and health in the classroom for infants. During today’s visit I observed the infant room. Teachers were engaged in feeding and interacting with infants. There was a concern around diapering and sanitation. During today’s inspection one of the teachers in the one-year-old classroom changed diapers without washing the child’s hand or their hands before and after diapering. Based on my observations the concern related to sanitation and health in substantiated. There is a concern that the posted staff/child ratios are not being followed. I observed the one-year-old classroom in the outdoor learning environment. One class had nine children with two teachers and the other class had six children with one teacher. The classes were not separated, they were intermingled and playing together. It could not be determined which child was in which class. There were fifteen children present and three teachers. Based on my observation the concerns related to ratio and groups size is unsubstantiated. However, separate outdoor play areas or time schedules were not provided for children under two when 15 or more children were in care. There is an additional concern related to enrollment and children files of children whose parents work at the facility. I was able to confirm that children records for the children were on file at the center and the children are enrolled. The allegations related to enrollment a children records is unsubstantiated. The following violations were observed and cited during today’s inspection. Violation Number Comment Rule 303 Children were not adequately supervised at all times. During the visit I observed the one-year-old class in the outdoor learning environment. During my observation, I observed children on one side of the play area and the teacher on the other side of the play area talking to another teacher who was in a different outdoor play area. The teacher had their back to the children and could not see the children or hear the children from where they were positioned. .1801(a)(1-5) 452 Separate outdoor play areas or time schedules were not provided for children under two when 15 or more children were in care. There were 15 one-year-old's and three teachers in the outdoor learning environment. .0605(c) 491 Caregiver did not respond at the earliest opportunity to an infant or toddler’s physical and emotional needs. During the visit, one of the children in the one-year old class fell forward into a toddler walker and was stuck. The teacher did not respond to assist the child. I intervened and informed the teacher, who then responded to the child and helped them back up. .0511(b)(1) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. A teacher in the one-year-old classroom did not wash the child's hands after diapering. 15A NCAC 18A .2803(c) 609 Staff did not wash their hands thoroughly before beginning work, before/after handling food, before bottle feeding or serving to other children, after toileting or handling body fluids, after diaper changing and after handling soiled items. The teacher in the one-year-old classroom did not wash their hands after diaper changing. 15A NCAC 18A .2803(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Teacher BB did not have a TB Test that was done within 12 months of being hired. They were hired October 28, 2024 and the TB Test was last completed August 8, 2023. .0701(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Teacher DW was employed on January 2, 2024. Criminal records check and qualification letter was not completed until January 22, 2024. Additionally, Teacher TT’s CPR certification did not certify them in First Aid. They will need to complete a First Aid course prior to their first 90 days of employment. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. The Fire inspector came out on March 19, 2024, to conduct a fire inspection and is returning soon to complete their inspection. The last fire inspection was March 30, 2023. Your program currently operates with a three-star license, issued April 20, 2022, earning 1 point in the education component, 6 points in the program standards component meting all enhanced requirements and 1 quality point by offering a staff benefits package and an infrastructure of parent involvement. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-three percent as of April 4, 2024. The NC Secretary of State website was reviewed on April 4, 2024, and Kindercare Education LLC was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were monitored during today’s inspection. The Infant 2 classroom did not have sleep check recorded. They did have sleep time records electronically on a tablet, but this information did not reflect visual checks. This was corrected during the visit by the teachers transcribing the information to the sleep checks and the teachers reported they would continue to do so going forward. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. Teacher DW was employed on January 2, 2024. Criminal records check and qualification letter was not completed until January 22, 2024. Additionally, Teacher TT’s CPR certification did not certify them in First Aid. They will need to complete a First Aid course prior to their first 90 days of employment. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Based on information provided, on March 4, 2024, a teacher used an electronic cigarette on the premises in the presence of children. There are 79 children and 11 teachers present during the time of today’s visit. During my visit I interviewed the administrator, Ms. Carima Neal, and informed her of the allegations. She reported she was informed by another teacher that while they were outside another teacher walked away from the children to another section of the playground and used an electronic cigarette/vape pen. Ms. Neal stated that there were approximately 12 children on the playground and two teachers. Ms. Neal informed me she spoke to the teacher about the allegations and the teacher admitted to using the vape pen on the playground. They informed her that it was done out of habit, and they were not thinking about it when they did it. Ms. Neal stated they placed the teacher on administrative leave the day of the incident and then terminated the teacher the following day March 5, 2024. Since the incident, the center held a staff meeting on March 19, 2024, where they discussed and reviewed the center’s tobacco policy as well as locking up personal items. I was able to view the agenda for the staff meeting as well as the sign in sheet where staff acknowledged their attendance. Based on the above information, the allegation of the use of an electronic cigarette by a staff member on the premises is substantiated. The following violations were cited during today’s inspection. Your program currently operates with a three-star license, issued April 20, 2022, earning 1 point in the education component, 6 points in the program standards component meeting all enhanced requirements and averaging a 5.6 score on the environmental rating scales and 1 quality point by offering a staff benefits package and providing an infrastructure of parent involvement. The last annual compliance visit was conducted September 23, 2022. The sanitation inspection was completed June 22, 2023, with a “Superior” classification. The last fire inspection was conducted March 30, 2023, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-five percent as of July 24, 2023. The NC Secretary of State website was reviewed on July 24, 2023, and Kindercare Education LLC was listed as current- active. You visited each indoor and outdoor space with me. Children enrolled were participating in free play activities as well as group activities. Teachers were observed sitting on the floor with the children reading books and engaging in free play activities. I found staff interactions and supervision to be in compliance. All program records were observed today including emergency drills, children files and staff records. Lunch consisted of pot stickers, peas, pears/pineapples, and milk. The following was cited during today’s inspection.
