The facility currently operates with a Four Star Childcare License that was issued on September 27, 2019. The last annual compliance visit was conducted on May 7, 2025. The administrator, Ms. Alston, was not present upon my arrival but arrived later. Mr. Stan, bookkeeper, and she assisted me during the visit. Children were observed participating in indoor teacher directed and free play activities and transitioning to outdoor play. The last sanitation inspection was completed March 23, 2026, with a “Superior” classification. The last fire inspection was completed March 27, 2026, and the facility was approved for daytime care only. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violation was observed. Violation Number Comment Rule 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. Three staff files did not contain a current annual staff evaluation and/or staff development plan. 10A NCAC 09 .0514(f) The above violation must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by April 21, 2026. You can email the letter to me at caroline.sykes@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number If a compliance letter is not received to my office by 5:00 PM April 21, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. It’s important that all staff have an evaluation and development plan each year to ensure personal and professional growth and development and support in their position with the program. We discussed the QRIS Pathway to the Stars. The program director is researching the Program Assessment and Classroom and Instructional Quality pathways and will decide which one will work best for the program. We reviewed the components of each pathway, along with DCDEE WORKS registration for staff. All staff should continue to register and review their WORKS accounts to ensure current education information is included. RESOURCES: -The Health and Safety Trainings are offered on the DCDEE Moodle format. The administrator, Ms. Alston, was present and assisted me during the visit. Children were observed participating in indoor teacher directed and free play activities and outdoor play. The program recently moved back into their permanent space after a temporary emergency relocation. The last sanitation inspection was completed May 5, 2025, with a “Superior” classification. The last fire inspection was completed April 30, 2025, and the facility was approved for daytime care only. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member, P. Ramsey, did not complete first aid training within the first 90 days of employment. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member, P. Ramsey, did not complete CPR training within the first 90 days of employment. .1102(d) 1757 A valid qualification letter was not on file and available to review at the facility. Two employees, S. Cooper and C. Riggsbee, qualification letters had expired and need to be renewed. G.S. 110-90.2(b) & (d) & .2703(e) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. S. Cooper and C. Riggsbee need to update their criminal background check, and a copy of their qualification letters should be sent to me by May 22, 2025. If I do not receive a copy of their qualification letters by this date, they should not be allowed to work in the program until their letter is received. A violation was cited regarding staff taking the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. We discussed the importance of ensuring each staff member completes the training within the required timeframe so that they are informed in the topic area and aware of the current best practices when it comes to recognizing and responding to suspicions of child maltreatment. A violation was cited pertaining to employees completing CPR/FA training within 90 days of employment. We discussed the importance of having all staff complete CPR/FA training within 90 days to ensure they are aware of the current best practices in the event an emergency arises and they need to administer CPR or First Aid. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On June 6, 2024, a staff member used an open hand and hit a two-year-old child on the top of the child's head. No marks or bruises were observed. G.S. 110-91(10) 1424 School-aged children were not adequately supervised. We were greeted by the center director Ms. Alethea Alston. Caroline Sykes, Childcare Licensing Consultant accompanied me during today's visit. The last Annual Compliance Visit was conducted on June 15, 2023. The center currently operates with a Four Star Child Care License that was issued on September 27, 2019. Ms. Alston assisted us during today's walk-through. The children were observed participating in different free play activities with their caregivers and then preparing to go outside. The annual compliance checklist was completed during today's visit. The last sanitation inspection was completed October 10, 2023 with a “Superior” classification with 14 demerits. The last fire inspection was conducted April 15, 2024 and the facility was approved for daytime care only. The last fire drill was completed on April 30, 2024. The last Shelter-in-place was completed on March 5, 2024. The last outdoor play inspection was completed on April 25, 2024. The following violations were cited: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In the infant classroom space #3, the activity plan was not completed to address each child's developmental domain during today's visit. GS 110-91(12); .0508(a) 807 A safe indoor and outdoor environment was not provided for the children. The school age bathroom has chipping paint that is accessible to children present in care. 10A NCAC 09 .0601(a) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. RATED LICENSE INFORMATION Rated License assessments have resumed, and your facility is in Cohort 2. Your preparation year will begin July 1, 2024 and will end on June 30, 2025.
