Name of Operation: FRANK PORTER GRAHAM SCHOOL-AGE CARE PROGRAM Facility ID: 6855079 Consultant: ANITA GRAHAM Operation Type: Center Case Number: Visit Date: 5/7/2024 Number Present: 43 Completed Date: 5/7/2024 Age: From 5 To 12 Total Minutes: 120 Time In: 03:00 PM Time Out: 05:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to complete monitoring for Annual Compliance Rated License visit. The facility currently operates with a five (5) star license that was issued on March 9, 2018. The program scored a 5.03. The program earned 6 points in education, 6 points in program standards and the quality point for program options met. Caroline Sykes, Child Care Consultant accompanied me on today’s visit. The director, Ms. Diane Christian greeted us and assisted us during the visit. Attendance was taken and observations made. Children were observed having in outdoor free play, participating indoor free play, routines and departing for the day. The center's last fire inspection was conducted on November 21, 2023. The center's last sanitation inspection was conducted on October 19, 2023 and a 'Superior' classification was received with four demerits. A full assessment of child care requirements was conducted during this date. I monitored program requirements, including postings such as, permit with restrictions maintained, summary of laws, daily schedule, First-Aid posters and the staff/child ratio charts. I monitored program records, a sampling of children’s records, staff personnel files, medical files and on-going trainings. The following violation was observed cited and a technical assistance was offered. The violation cited was corrected during the visit. Violation Number Comment Rule 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. This was corrected during the visit. .0701(d) The staff were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of the licensing requirements that apply to your program. It is always your responsibility to maintain compliance with all of the applicable laws and rules . The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Please be aware that new rules came into effect in January of this year. TECHNICAL ASSISTANCE: -10A NCAC 09 .0701 HEALTH STANDARDS FOR CHILD CARE PROVIDERS, SUBS : (d) Staff medical statements, proof of a tuberculosis test or screening, and completed health questionnaires shall be included in the staff member's medical file, which must be maintained separately from the staff member's individual personnel file in the center. You must use your NCID to access Moodle and register for the trainings. -For other upcoming training opportunities please visit CCSA’s website at: https://www.childcareservices.org/programs/ccrr/professional-development/ -Child care programs can order free posters from the NC Child Care Health and Safety Resource Center at https://healthychildcare.unc.edu/ . We discussed the rated license process and that this facility is categorized as Cohort 1, meaning the program is approaching the end of the planning year and the assessment year begins July 2024. We discussed that your program plans to wait until the assessment year to apply for your assessment. You will be given the application for assessment for submission to NCRLAP upon your request. I will submit the application at that time. AGENCY NEWS: Child Care Licensing rules and regulations are established to ensure a safe and healthy environment for children in care.
