    Name of Operation: FRANK PORTER GRAHAM ELEMENTARY PRE-K Facility ID: 68000417 Consultant: CAROLINE SYKES Operation Type: Center Case Number: Visit Date: 4/30/2025 Number Present: 27 Completed Date: 4/30/2025 Age: From 3 To 5 Total Minutes: 115 Time In: 11:50 AM Time Out: 01:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for all applicable Child Care Requirements. The last Annual Compliance Visit was conducted on May 7, 2024. The program currently operates with a Five Star Child Care License that was issued on March 28, 2017. The last sanitation inspection was completed April 28, 2025 with a “Superior” classification. The last fire inspection was completed on October 15, 2024 and the facility was approved for care. Upon arrival, I signed in at the school’s main office and then preceded to the preschool classrooms where I was greeted by the staff. The classroom teachers assisted me during the visit. Currently the program consists of two classrooms. Children were observed eating lunch, participating in teacher directed and free play activities and preparing for nap. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violation was cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The outdoor learning environment is in need of the following maintenance: New mulch, grass cut, broken drainage pipe repaired or replaced. 10A NCAC 09 .0601(a) 1314 Emergency information did not name childs health care professional. Once child's file did not include the name of the preferred health care provider or hospital. .0802(c)(2) The above violation must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by May 14, 2025. You can email the letter to me at caroline.sykes@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number If a compliance letter is not received to my office by 5:00 PM May 14, 2025, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. TECHNICAL ASSISTANCE: A violation was cited regarding safe indoor and outdoor spaces. The outdoor learning environment needs mulch, grass cut and a broken drainage pipe repaired. We discussed the importance of ensuring the playground is a safe space for the children. We also discussed ensuring that monthly playground inspections continue to be completed, and playground needs reported on the inspection forms. A violation was cited regarding children’s files not containing preferred medical facilities and/or hospitals for the children. We discussed the importance of having this information on file in the event an emergency happens. RESOURCES: -The Health and Safety Trainings are offered on the DCDEE Moodle format as well. The last Annual Compliance Visit was conducted on May 10, 2023. The center currently operates with a Five Star Child Care License that was issued on March 28, 2017. Caroline Sykes, Child Care Consultant, accompanied on today’s visit. Upon arrival, we signed in at the school’s main office and then proceeded to the preschool classrooms where we were greeted by Ms. Raquel Carney. There are two Pre-K classrooms at this location. Ms. Palacio is the lead teacher in the second classroom. During the visit attendance was taken and observations made. Children were observed participating in indoor free play, story time, teacher directed activities and routines The last Fire Inspection was conducted November 21, 2023 and the facility was approved for daytime care only. The last Sanitation Inspection was completed November 6, 2023 and achieved a “Superior” classification. A full assessment of child care requirements was conducted during this date. I monitored program requirements, including postings such as, permit with restrictions maintained, summary of laws, daily schedule, First-Aid posters and the staff/child ratio charts. I monitored program records, a sampling of children’s records, staff personnel files, medical files and on-going trainings. The following violations were observed. The following violation was observed cited and a technical assistance was offered. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Clorox wipes and Lysol wipes were observed maintained in an unlocked cabinet. This was corrected during the visit. .2820(b) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. This was corrected during the visit. .0701(d) The above violation were corrected immediately during the visit. Cleaning materials, Detergents (in all forms, including pods, Automatic dishwasher detergents (liquid or solid, including pods), Aerosol cans, Paints, solvents, paint removers, Floor polish or stripper, furniture polish, Liquid soap, hand sanitizers, disinfectants, Pesticides, Health and beauty products, Lawn care chemicals. -10A NCAC 09 .0701 HEALTH STANDARDS FOR CHILD CARE PROVIDERS, SUBS : (d) Staff medical statements, proof of a tuberculosis test or screening, and completed health questionnaires shall be included in the staff member's medical file, which must be maintained separately from the staff member's individual personnel file in the center.
