The last Annual Compliance visit was conducted on May 20, 2025. The last Sanitation Inspection was completed on December 17, 2025, and the program received a “Superior” classification. The last Fire Inspection was conducted on February 25, 2026, and the facility was approved. Upon arrival, I was greeted by the program owner, Ms. Nervo. She assisted me on a walkthrough of the facility where attendance was documented, and observations made. Infants were observed participating in routine care and floor play. Toddlers and preschoolers were observed engaging in indoor and outdoor play activities and eating morning snacks. A checklist was used to note the requirements I monitored today. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. Five employees files did not contain an annual staff evaluation and a staff development plan. 10A NCAC 09 .0514(f) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Two staff members did not complete the health and safety trainings within 5 years of previously completing them. .1103(b) The above violations must be corrected immediately. The last Annual Compliance visit was conducted on May 20, 2025. Upon arrival, I was greeted by the director of operations. She assisted me on a walkthrough of the facility where attendance was documented, and observations made. The younger class was observed engaging in outdoor activities. The older children were observed engaging in outdoor play. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were cited. Violation Number Comment Rule 541 The written feeding plan did not include the child's name, parent signature, and/or was not dated when received by the center. Two feeding plans in the infant classroom were not signed by the parent. .0902(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. A prescription medication in the preschool child was not stored in it's original labeled container. .0803(2)(a) 847 Parent's medication authorization did not include required information. A diaper cream in the Toddler classroom had an expired medication form. This item was corrected during the visit. 10A NCAC 09 .0803(4)(6-9) The above violations must be corrected immediately. TECHNICAL ASSISTANCE: A violation was cited regarding expiration dates on medication permission forms. We discussed the importance of ensuring medication forms are updated as required to ensure any changes in medication administration are noted. A violation was cited regarding prescription medication storage. An epi-pen was not stored in the original container with the prescription label. A pharmacy label was attached to the bag that the epi pen was stored in, but it only provided minimal information. We discussed the importance of prescription medications to be stored in their original container with the prescription label to ensure all dosing information is available in the event the medication needs to be administered. A violation was cited regarding infant feeding plans not being signed by parents. We discussed that the feeding plan should initially be signed by the parents. As updates are made, the teacher can update the form and review it with parents. We talked about two areas of standing water on the ground in the fenced playground area out front that should be investigated. It was unclear if they were coming from rain or not. The director of operations will monitor this situation. I shared information with the director of operations regarding the new QRIS. We discussed the possibility of the program participating in the QRIS in the future. I showed the program director how to navigate to the QRIS Modernization page on the DCDEE website. We looked at various resources on the site and discussed possible NCRLAP mock assessments. Please reach out with any questions you may have regarding QRIS. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements.
