The last Annual Compliance visit was conducted on January 16, 2025. The facility currently operates with a Five-Star Child Care License that was issued on January 2, 2019. The last Sanitation Inspection was completed on March 21 2025, with a "Superior" classification. Upon arrival, I was greeted by the program director, Ms. Sasha. She assisted me on a walkthrough of the facility where attendance was documented, and observations made. The infants were engaging in floor play, and napping. The toddlers were observed engaging in indoor and outdoor activities and daily routine diaper changes During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violation was observed. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The lesson plan in space 2 had a date of April 2025. The lesson plan in space 4 had a date of March 2025. These items were corrected during the visit. GS 110-91(12); .0508(a) The above violation was corrected during the visit; therefore, a corrective action letter is not required. Technical Assistance: A violation was cited regarding activity plans. We discussed the importance of activity plans being current and posted in the classrooms for parents and other visitors or substitute teachers to view. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to always maintain compliance with all the applicable laws and rules. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ on the home page under the ‘Services’ tab. RESOURCES: -The Health and Safety Trainings are offered on the DCDEE Moodle format as well. The last Annual Compliance visit was conducted on February 1, 2023. The facility currently operates with a Five-Star Child Care License that was issued on January 2, 2019. The last Sanitation Inspection was completed on August 1, 2023, with a "Superior" classification with (14) demerits. The last Fire Inspection was conducted on July 19, 2023, and the facility was approved for daytime care only. The last Fire Drill was conducted on December 12, 2023. The last Lock Down/ Shelter-in-Place was on December 15, 2023. The last Playground Inspection was conducted on December 22, 2022. Upon arrival, I was greeted by the director, Sasha Simmons. I stated the purpose of my visit and was assisted on a walkthrough of the facility where attendance was documented, and observations made. The infants were eating breakfast, daily routine diaper changes and toddler class were singing songs. A checklist was used to note the requirements I monitored today. The North Carolina Department of the Secretary’s website was used to check West Ministries School Inc. The status of the corporation on January 12, 2024, was current/active. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records, Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections and Transportation. This facility does not provide transportation. The following violations were observed during today’s visit. Violation Number Comment Rule 541 The written feeding plan did not include the child's name, parent signature, and/or was not dated when received by the center. Five feeding schedules were not signed by parents. .0902(a) 833 The EMC plan did not give the procedures to be followed . The Emergency Medical Care plan was not reviewed on an annual basis with staff. 10A NCAC 09 .0802(a) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last playground inspection was conducted on December 22, 2023. Director was unaware of the monthly requirement for this inspection. .0605(q) 865 A child was given over the counter medication without written authorization from a parent to follow the direction of the NC State Health Director during a public health emergency and/or the authorization did not contain the required information. A and D diaper cream and Boudreaux's But Paste was used for two children without a permission form signed by the parent in space 3. 10A NCAC 09 .0803(9)(a-d) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Two classrooms did not have documentation of visual checks while infants were sleeping. .0606(g) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. The EPR review was not completed on an annual basis with the staff. .0607(f) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member did not have this training within 90 days of hire. .1102(g) The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received by my office on, or before January 31, 2024. The letter should include: Each item number of noncompliance cited. Describe how and when the violations were corrected. Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov . If a compliance letter is not received to my office by 5:00 PM January 31, 2024, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. Technical Assistance: Today, we discussed that playground inspections must be completed once every month. You stated that you were unaware that playground inspections had to be completed monthly. All infant feeding schedules must have the parent and teacher signature on them. Please make sure teachers are checking permission forms on a weekly basis for dates expiring. If the permission form is not legible, please have the parent sign a new one. ERR and Medical review with staff must be completed on an annual basis. Infants must be checked for the rise and fall of the chest every fifteen minutes. This must be documented from the time the child falls asleep until he/she wakes up. All lesson plans must be posted and dated by Monday morning for the week. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program.
