The last Annual Compliance visit was conducted on January 23, 2025. The facility currently operates with a Five-Star Child Care License that was issued on November 18, 2024. The last Sanitation Inspection was completed on January 8, 2026, with a "Superior" classification. The last Fire Inspection was conducted on September 12, 2024, and the facility was approved for daytime care only. A fire inspection was not completed for the program in 2025. They have contacted the fire marshal to schedule an inspection and provided available dates as requested. They are awaiting follow-up from the fire department. Upon arrival, I was greeted by the program assistant director, Ms. Venable. The program director was absent today. Ms. Venable assisted me on a walkthrough of the facility where attendance was documented and observations made. The infants and toddlers were observed engaging in floor play, routine activities and taking naps. The older children were observed interacting in free play and teacher directed activities, and outdoor play. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire drill on record was in September of 2024. All staff should complete CPR/FA training within 90 days of hire and/or prior to the expiration date of their current training. It’s imperative that all children have a health assessment/medical exam completed and on file within 30 days of enrollment to ensure the health and safety of everyone. It is recommended that you require these exams or assessments to be completed and provided to you prior to enrollment, to prevent a lapse in the 30-day requirement after enrollment. The Travel Authorization form should be completed for each child in its entirety and kept on file. I recommend that you check every child’s file to confirm that the form has been completed. A violation was cited due to one child’s file not having the form completed and two other files were missing pertinent information on the form. Every child should have a signed statement regarding the parent/guardian’s receipt of the program’s Shaken Baby Syndrome and Abusive Head Trauma Policy. Kaye Adkins, Licensing Supervisor, accompanied me on today’s visit. The administrator, Jessica McNair was present and assisted me during the visit. Children were observed participating in indoor play activities. The last sanitation inspection was completed April 8, 2024 with a “Superior” classification. The last fire inspection was conducted February 17, 2023. The inspector returned to complete the 2024 inspection but was unable to do so due to the hood needing to be commercially cleaned. No follow up visit has been made at this point. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Fire inspection was not completed within 12 months. On April 4, 2024 the Chapel Hill Fire Department emailed the facility and told them that they must have the kitchen hood vent cleaned so that they could complete their inspection. The last fire inspection was on February 17, 2023. 10A NCAC 09 .0304(a) 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. Peeling paint on walls was observed throughout the facility particularly in the classroom bathrooms in spaces 5 and 6. 15A NCAC 18A .2825(a) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. The infant classroom did not have up to date sleep charts for each child aged 12 months or younger. .0606(g) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by August 21, 2024. The letter must address each violation and explain how it has been corrected. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ ,on the home page under the ‘Services’ tab. TECHNICAL ASSISTANCE A violation was cited regarding Infant Sleep Charts. The sleep times were being recorded on a tablet but did not include the required information such as teacher initials or position of sleep at time of check. We discussed the importance of sleep charts being maintained on paper as required to ensure a safe sleep environment for the children in care. Another violation was cited pertaining to the fire inspection being past due because the kitchen hood needs to be cleaned. We discussed the need to have the hood cleaned as soon as possible so that the fire inspector can return and complete the fire inspection. We also provided technical assistance on looking for local hood cleaning companies. The last annual compliance visit was conducted 2/8/2023. The last routine unannounced visit was 8/3/2023. The last sanitation inspection was completed 10/31/23. The last fire inspection was completed 2/17/23. Reminder: Contact the fire inspector to complete the fire inspection before 2/17/24. This facility operates with a 3-star license issued March 20, 2020. This facility is in COHORT 1. Beginning in July and throughout the next few months I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be between July 1, 2023 and June 30, 2024. Have all staff update WORKS accounts to reflect all currently completed coursework (DCDEE-WORKS) Determine whether you wish to have Environment Rating Scale (ERS) Assessments [ITERS-R, ECERS-R, SACERS-U, FCCERS-R] conducted Request technical assistance with your child care consultant and local partners Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) Reach out to your local Community College to discuss educational opportunities Review NCRLAP website ERS resources (www.NCRLAP.org). To help review ERS requirements and think carefully about the current characteristics and practices in your program and classrooms, check out the Thinking More worksheets. A worksheet is available for each subscale of each rating scale. • Consider requesting an ERS assessment (free of charge) during the preparation year. Remember the scores can be used in a variety of ways o ERS assessments can be requested during the preparation year and scores can be used as guidance to enhance program quality o If desired, assessment scores can be used during the preparation year if the operator wants to move forward with a rated license reassessment o Assessment scores can be saved to use during the reassessment year o Facilities can request another ERS assessment (free of charge) in your reassessment year for any ERS assessment scoring less than a 5.0 If you do not plan to have the ERS assessments, you would not need to prepare for the scales. Seven classrooms were monitored for staff/child ratios, group sizes, capacity, supervision, postings, health and safety and interactions/discipline. I reviewed parent permission to apply diaper cream for over 20 children. Most of the forms either had, ENOUGH written in the space for the amount to apply or the line was blank. We discussed to make sure parents write in an amount, example: dime size, pea size, quarter size. We also discussed posting or filing feeding schedules in a notebook for infants and toddlers that are younger than 15 months old. Once a child turns 15 months old, the feeding schedule is not needed. Safe sleep position checks were completed for all children under one year old. At least 30 days worth of the safe sleep checks were available for review. Primary caregivers were assigned in classrooms for infants and toddlers. The fire drills were removed for 2023 and a new sheet was started for 2024. You maintain copies of the fire and emergency drills in the office. Shelter in place or lockdown drills were conducted every 3 months and documented on the emergency drill log. The outdoor play area inspections were current and complete for 2023. A new sheet was started for 2024. Incident reports/log: The incident reports were logged on the incident report log and a copy was maintained with the log. You have been completing the pink incident report required for KinderCare. Begin using the incident report provided by DCDEE for all incidents. Anytime an injury requires professional medical attention, the form should be submitted to your consultant. EPR plan: The EPR plan was update May 2022. It was archived in May 2023 so you will have to go in to the NC Risk Management portal to request it. Once it is accessible, you will update it and print it. Be sure to update it within the year or it will archive again. Ready to go file: The Ready to Go file was not prepared. A violation was cited for not having the ready to go file. You had the checklist for required items. The file should remain current at all times and be ready to grab and go whenever needed. Twenty children’s records were reviewed today. Two children did not have a medical on file within 30 days of enrollment. Staff files and the training worksheets were reviewed today. Three teachers had expired ITS-SIDS trainings. The emergency medical care plan was posted but was not on the form provided by DCDEE. You completed the emergency medical care plan on the form from the DCDEE website and posted it during the visit. This information is important to keep current so staff know their responsibilities during and emergency. This information is reviewed during orientation and annually for all staff. The three star license was posted along with the NC Summary of the Law and safe arrival and departure procedures. The outdoor play areas were monitored. You stated that the outdoor play areas will be renovated soon. The plan was for that to occur last summer. Transportation is not provided. No buses on the premises. Violation Number Comment Rule 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. Three teachers that work in the classroom for infants did not have current ITS-SIDS training. .1102(f) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Two children did not have a medical on file. GS110-91(1) 1821 The EPR Plan did not include the date of the last revision of the plan. The plan was last updated May 2022. .0607(d)(8) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The plan was not update since May 2022. .0607(e) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Several teachers did not update the health and safety trainings within the 5 year time frame. .1103(b) The violations cited today should be corrected as soon as possible. Mail or email me a corrective action letter stating when the violations were corrected, how the violations were corrected and how you plan to prevent repeat violations in the future.
